An evaluation of routine electron microscopy in the examination of renal biopsies was performed. Two renal pathologists, one with and the other without benefit of ultrastructural studies, independently evaluated 213 consecutive renal biopsies. In 11% electron microscopy indicated a substantially different diagnosis and in 36% ultrastructural studies provided important information by substantiating the diagnosis made by light microscopy, establishing a clinically relevant subclassification or documented changes which occurred during the natural course of disease or as a consequence of treatment. The examination of renal biopsy material by electron microscopy in consecutive routine cases contributed to diagnosis and/or patient management in 48% of cases.

The urinary bicarbonate excretion following administration of sodium bicarbonate was examined in 25 necrokidney-transplanted patients 14–60 days after the operation, and in 10 patients with mild, chronic renal diseases. The effects on plasma and erythrocyte potassium and magnesium concentrations of low ( < 0.2 mEq/l) as compared with orthodox (1.50 mEq/l) dialysate magnesium concentrations have been studied in six patients on maintenance haemodialysis. On low magnesium dialysis, plasma magnesium concentrations were significantly decreased from hypermagnesaemic to normal levels but the high erythrocyte magnesium concentrations were unchanged. The plasma potassium decrease, usual during each dialysis, was unaffected. In contrast, erythrocyte potassium concentrations, which did not change during individual dialyses, were high when orthodox magnesium was present in the dialysate, and normal during low magnesium dialysis. Low magnesium dialysis has biochemical advantages in that it corrects hypermagnesaemia and maintains normal erythrocyte potassium concentrations which otherwise would be increased.

After intravenous pyruvate administration, the rate of decay of the blood pyruvate was significantly lower in the chronic uremic patients than in the healthy subjects. This difference could not be ascribed to a reduced renal excretion of pyruvate. Lower blood 2-oxoglutarate levels were found during the test in chronic uremic patients. The impaired glucose tolerance observed in chronic uremia could in part depend on a reduced pyruvate oxidation in the citric acid cycle.

Since indomethacin diminishes the intensity of proteinuria in glomerulonephritis, experiments were undertaken to identify the morphological elements that might explain the reduction in the permeability of the glomerular capillary wall. A case of lithium intoxication, manifest by cardiovascular and neurological disturbance, is presented. The importance of this case lies in the induction of oliguric renal failure and the consideration of whether or not lithium has a prime nephrotoxic effect, or whether the renal failure was secondary to the ischaemia produced by the profound cardiovascular disturbance.

The dialysability of the contrast medium meglumine sodium diatrizoate was investigated in 8 patients on regular dialysis treatment. The dialysance was 52.9 ± 8.0 ml/min with a blood flow of 172 ± 41 ml/min using a modified Kiil kidney. The half-time concentration was 4.5 h. A case of acute renal failure during intermittent rifampicin therapy (600 mg once a week) is reported. The anuria persisted for 6 days and was followed by prompt recovery. A renal biopsy showed tubular necrosis. The immuno fluorescence for IgG, IgM, IgA, B1C and fibrinogen was negative. The authors suggest that the incidence and severity of acute renal failure, as a clinical manifestation of rifampicin hypersensitivity, could be dependent on the interval between two successive doses of the drug.

This is a case report of Waldenström’s macroglobulinemia associated with mesangio-capillary glomerulonephritis, angiitis and myositis. The renal lesion was a major problem. The confinement of lymphadenopathy to the abdominal cavity with absence of an M component in the serum electrophoretic pattern hindered precise antemortem diagnosis. The accompanying mild increase in serum IgA mimicked Aleutian mink disease. The possibility of dysproteinemia playing a role in the renal alteration is suggested.
