A total of 32 percutaneous renal biopsies were examined from patients with rheumatoid arthritis. The renal biopsies in these patients present a very heterogeneous picture; there were 9 cases of chronic interstitial nephritis and 4 cases of amyloidosis. In one patient whose renal biopsy showed glomerulitis, a muscle biopsy resulted in a diagnosis of polyarteritis nodosa. 11 biopsies showed a normal histology, and 6 showed arteriosclerosis of the small arteries. Finally, 1 patient showed so-called membranous glomerulonephritis; this may have been the coincidental presence of 2 diseases. We were unable to confirm the existence of a vascular rheumatoid lesion, as previously reported in the litterature. Employing a technique which permitted a quantitative evaluation of the nuclear distribution in the glomeruli, this distribution was examined in the biopsies from 28 patients with rheumatoid arthritis and in 11 patients with gallstones, but with normal renal function and normal blood pressure. The presence of a glomerular hypercellularity in rheumatoid arthritis, as has previously been claimed was not confirmed. It was likewise not possible to demonstrate local hypercellularity in the glomeruli of patients with rheumatoid arthritis and normal renal biopsies.

The diuretic and saluretic response to the intravenous infusion of a 5% salt solution (5 g of NaCl/m2 of body surface area) was examined in a group of normotensive subjects, in patients with uncomplicated essential hypertension, in normotensive women after toxemia of pregnancy and in subjects with ‘labile’ hypertension, who were normotensive at the time of the study. It was found that the 2 latter groups reacted to the procedure in a very similar manner as the subjects with hypertension, i. e. with a diuretic as well as saluretic response of rapid onset, in contrast to the gradual and much lesser rise in water and salt excretion observed in the normal control subjects. The diuretic response in all 3 experimental groups was more pronounced than the saluretic response. Angiotensin II-amide administered intravenously at 0.2 µg/kg·min together with isotonic saline to rats with either DCA-induced or renal hypertension had the same diuretic and natriuretic effects as in normal rats. In renal hypertension (induced by a clip on one renal artery leaving the other kidney untouched) the normal overall diuretic response appeared to be due to the clamped kidney only. In the unclamped kidney of renal hypertensive rats the tubular diuretic and natriuretic response to angiotensin was abolished. Hydrochlorothiazide (5 mg/kg i.v., followed by 1 mg/kg •min) also failed to elicit its normal natriuretic response in the unclamped kidney.
