By means of renal biopsy and light, immunofluorescence, and electron microscopy, a diagnosis of membranous nephropathy (MN) was made in 100 patients. The nephrotic syndrome was present in 83 of these patients. 65 of the patients were men and 35 were women. The average period of follow-up was 99.8 months. As judged by the incidence of death and of improvement or complete healing, the women fared better than the men, whether given high-dose alternate-day prednisone therapy or not. The incidence of improvement or complete healing in the patients given prednisone was higher than that reported for patients who were not given corticosteroids. We have shown that occurrence of MN is more frequent in women than men and the course of MN is more benign in women than in men; alternate-day prednisone therapy appears to be beneficial in patients with MN.

The aim of this study was to investigate the effect of amino acid (AA) supplementation on the loss and its time course of free AA, during hemodialysis. 2 uremic patients on regular hemodialysis received an increasing number of essential AA tablets during the dialysis. The loss and its time course of 6 essential and 7 nonessential free AA during dialysis was assayed in the dialysate. The loss per dialysis treatment of the administered essential AA increased almost proportionally with the doses of AA tablets. For the nonessential AA, no definite relationship between doses of AA tablets and losses was observed. At all levels of supplementation, methionine was retained best: 90% of the amount present in the tablets. Threonine showed the lowest retention: 15-55%.

The reciprocal of serum creatinine concentration (1/Cr) is often used to predict glomerular filtration rate (GFR). Serum creatinine also varies with age, size, and muscle mass, and so it may inaccurately estimate GFR. The reciprocal of serum β2 -microglobulin (1/β2µ) has been proposed as an alternative estimator of GFR. This study compares 1/Cr and 1/β2µ as predictors of GFR as measured by 125I-iothalamate clearance (Ciot) and creatinine clearance (Ccr) in 29 subjects with a wide range of age, size and kidney function, and including 12 chronic hemodialysis patients. 1/β2µ was a better predictor of Ciot (r = 0.90) and Ccr (r = 0.87) than 1/Cr (r = 0.50 and 0.78) was. In fact, in the non-dialysis population, 1/β2µ predicted Ciot (r = 0.86) about as well as Ccr predicted Ciot (r = 0.87). β2µ was less dependent on body size than Cr and unlike Cr, was not influenced by dialysis and did increase as GFR decreased with age. β2µ can be useful as an alternative clinical estimate of GFR, particularly when Cr is considerably influenced by factors other than re

Peripheral and bone marrow eosinophils were determined in a group of patients on chronic hemodialysis and in predialysis uremics. Healthy subjects were taken as controls. Increased number of eosinophils in bone marrow were found in the predialysis uremic group and this finding was even more accentuated in the dialyzed patients. Marrow eosinophilia correlated linearly with serum creatinine levels in the predialysis group. The peripheral eosinophil count was normal in the uremic group and in part of the hemodialysis group. Marked and progressive peripheral eosinophilia was evident in a distinct subgroup of the dialysis patients. Marrow eosinophilia seems to be part of the uremic syndrome. The possible nature of the factor(s) involved in the derangement of eosinophil homeostasis is discussed.

The thiobarbituric acid (TBA) assay was applied to uremic blood. Significant TBA-reactive material was found in patients with chronic renal failure compared to normal controls. The presence of TBA-reactive material correlated with the degree of renal failure, and was removed by hemodialysis. The reactive material is not 2-deoxyribose or sialic acid. In addition, the presence of TBA-reactive material in uremic blood is not abolished by prior aspirin ingestion, suggesting it is not related to active prostaglandin synthesis. The possible origin of the TBA-reactive material in uremic blood is discussed.

To assess the relevance of HBe/anti-HBe system and DNA polymerase activity in patients on regular dialysis treatment, we prospectively studied 38 patients on haemodialysis, who were chronic carriers of HBs Ag (range 6–66 months), and 26 HBs Ag negative dialysis patients as controls. HBe Ag was present in 74%, DNA polymerase in 60%, and anti-HBe in 13% of HBs Ag positive patients. After a mean follow-up of 23.5 months, only 2 patients, who had been HBe Ag positive, had cleared HBs Ag, and 1 of them had turned to anti-HBS positivity. Among the patients who were still HBs Ag positive, only 1 had lost HBe Ag, without developing anti-HBe. Throughout the study, SGOT and SGPT levels were significantly raised in HB virus carriers as compared to controls. In the HBs Ag positive group, the presence of HBe Ag and/or DNA polymerase characterized a subgroup with the most striking abnormalities in enzyme levels. The causes of death in the HBs Ag positive group were not related to liver disease. Viral replication usually takes place in HB virus carriers on haemodialysis and is associated with biochemical evidence of liver-cell damage.
