A plasma factor has been hypothesized to initiate compensatory growth of the remaining kidney following unilateral nephrectomy. Evidence supporting the existence of this factor has been obtained in studies of hamster renal cortical cells grown in monolayer tissue culture. These cultured cells resemble renal epithelium in electron micrographs. The incorporation of 3H-uridine into RNA was significantly increased in those cultures incubated in plasma from uninephrectomized hamsters (UP) above those containing sham plasma (SP) at 10 and 15 % plasma in the medium. The uptake of uridine into the cy toplasmic fraction of those cultures incubated for 4, 6, 8, 10 and 24 h and in the nuclear fraction for 2, 4, 6, 8 and 24 h in UP was significantly greater than those incubated in SP. Plasma from partially hepatectomized hamsters did not increase the growth rate of this tissue culture over those cultures incubated in SP. Dialysis of UP does not affect its growth characteristics. Therefore, a renotrophic factor is present in the plasma of uninephrectomized hamsters which increases the uptake of uridine into renal tissue cultures.

(1) Differential protein clearances have been measured in 20 patients suffering from toxaemia of pregnancy. The proteins measured were albumin, transferrin, IgG, ceruloplasmin, IgA, C′3 complement, α2-macroglobulin (α2M) and IgM. (2) Selectivity of proteinuria was high in 14 cases, where no protein molecule with a molar weight greater than 180,000 was found. (3) In 6 cases, selectivity was low with a high differential protein clearance of α2M. (4) IgM was not detected in the urine in any patient. C′3 was found in one patient’s urine only. Low selectivity was related to a total proteinuria of at least 6 g/day. The anomalies in differential clearances of the individual proteins are discussed. Differential renal protein clearances were measured in twelve patients suffering from renal amylloidosis complicating familial mediterranean fever. Seven proteins examined included transferrin, IgG, IgA, IgM, C′3, ceruloplasmin and α2M. Eight out of twelve patients had non-selective proteinuria, three had moderately selective proteinuria, and one had highly selective proteinuria. A comparison was made between the estimation of selectivity by simple inspection of differential protein clearances and by three different methods involving measurement of the slope of the regression line between the logarithm of differential protein clearance and the logarithm of the molecular weight of the various proteins. The fluorescent antibody method has been employed for the identification of HL-A antibodies on the surface of human peripheral blood lymphocytes. Identification of weakly reactive or non complement-fixing HL-A antibodies may augment present methods for the detection of pre-existing humoral sensitization to donor allo antigens in potential transplant recipients.

The effects of high doses of frusemide on plasma and muscle electrolyte composition were investigated in 20 patients with chronic renal failure (CRF), in 5 of them complicated by superimposed cardiac decompensation (CRF + CD). In patients with uncomplicated CRF, short-term frusemide therapy resulted in a decrease of muscle water content, while no significant effect on muscle electrolyte and protein nitrogen contents was observed. On the contrary, in patients with CRF + CD, muscle water (both in the extracellular and intracellular phases), sodium and chloride concentrations fell significantly as a result of frusemide therapy. Muscle potassium remained unchanged, even if related to the cell protein content, in both groups of patients. These results support the conclusion that, in patients with uncomplicated CRF, short-term frusemide therapy acts primarily on plasma and extracellular volume with little or no modification of intracellular fluids and tissue electrolyte composition.

Changes in colloid osmotic pressure (COP), osmolality, blood urea nitrogen, glucose, electrolytes and arterial blood gases were correlated with the symptoms that occurred during 46 dialyses in 10 stable, chronic hemodialysis patients who had symptoms attributable to the disequilibrium syndrome. The onset and severity of symptoms were consistently related to fluctuations in COP. Saline administration failed to relieve them, whereas the continuous infusion of salt-poor human serum albumin (1 g/kg) or mannitol (1 g/kg) successfully controlled the COP fluctuations and most of the symptomatology. The relationship of the COP fluctuations to existing theories on the pathogenesis of the disequilibrium syndrome were discussed.

A reliable method for the determination of cadmium in blood and urine has been developed, and the concentrations in normal subjects and in patients with chronic renal failure have been determined. Dependancy on, and variations with, age, sex and smoking habits in both health and disease have been investigated. The increases of blood cadmium levels during dialysis are shown to be due to the contamination produced by the zinc-oxide plasters used in the construction of the dialysis coils. The correlation of cadmium concentrations with renal disorders, and any associated secondary bone diseases is discussed.
