The local and general cellular immune response and the sequence of antibody production was studied in experimental retrograde pyelonephritis.The rats developed an acute urinary tract infection with high bacterial counts of O4 E. coli in the urine (106-8/ml) and renal tissues (104-8/g). The kidneys and bladder walls were infiltrated with inflammatory cells. There was an in variable brisk general immunocellular response with direct(IgM) plaque counts rising to 6× 102 antibody-producing cells (APC)/106 spleen cells at 4–6 days, and indirect (IgG) plaques reaching a peak at 15–20 days. The immunocyto-adherence assay attained a maximum of 104 APC/106 spleen cells at 10 days. Similar changes occurred in lymph tissue from the renal hilum and pelvis. There was also an abundant but variable local production of specific O4 antibody (mainly IgG and IgM) by numerous cells in the kidney and the bladder wall. 13 patients with oliguric acute renal failure were given ethacrynic acid to induce diuresis. Histologic evidence of acute tubular necrosis was present in all patients who died. Seven patients responded with sustained diuresis, six patients did not. The most important factor determining the efficiency of treatment was the period of time between the onset of the oliguria and the time the ethacrynic acid was given. If this delay exceeded 24 h, only one patient responded; but all patients treated within 22 h responded. Permanent hearing loss in one patient and transient loss in another were the major side effects. Ethacrynic acid profoundly influences the natural course of acute tubular necrosis if given before 24 h of oliguria.

Percutaneous biopsies were performed in 14 unselected addicts, ranging in age from 21 to 52 years, who had no manifest cardiac, hepatic or sickle-cell disorder. Urinary findings ranged from normal to a trace of protein with a few red blood cells and a few white blood cells; Australian antigen was negative. All were normotensive and blood chemistries were within normal range. By light microscopy, 12 cases exhibited slight nonspecific increase in mesangium. Three cases had, on immunofluorescent studies, mesangial deposits -predominantly of IGm – and two cases had basement membrane deposits of IGg. Electron microscopy revealed focal foot process fusion, increase in mesangium and small electron-dense deposits in six cases. In view of the propensity of addicts to develop variegated renal disorders, the asymptomatic renal lesions herein described may represent early manifestation of kidney disease as a consequence of drug usage and deserve further investigatio

Intravenous pyelograms were performed on 22 kidney donors, both before and from 2 weeks to 4 years after nephrectomy. A significant increase in kidney size was demonstrated by four different measurements. There was no significant difference in the increase in kidney size between the donors over 40 and those under 40 years of age. A significant increase in glomerular filtration rate per kidney was also found, but this increase could not be correlated with the increase in kidney size.

This paper describes a study undertaken to determine the prevalence of a family history of renal disease among patients with chronic renal disease of unknown aetiology. The results showed that 18.2% of these patients had a positive family history whereas only 6.1% of control persons investigated in a similar manner gave such a history. For hypertension the difference was less marked, 24.2% compared with 15.2%. These results indicate that renal disease occurring familially may be more common than previously thought.
