摘要:
The bacterial concentrations of the municipal water increased by more than 39-fold when subjected to reverse osmosis; then decreased by greater than 200-fold within the reservoir and water supply system of the hemodialysis center. The bacterial concentrations of dialysate solutions in contact with proportioning single-pass artificial kidney machines were as low or lower than the water from the hemodialysis center system ( < 10 CFU/100 ml.). The complete opposite was observed in the recirculating single-pass artificial kidney machines where bacterial concentrations in the dialysate solution reached levels greater than 1.0 × 106 CFU/100 ml

摘要:
The role of metabolic acidosis in the genesis of renal osteomalacia was investigated by studying bone mineralisation and resorption rates with a combined isotope and balance technique in six patients, before and after the administration of alkali. Correction of blood pH was achieved in five cases and was associated with a significant rise in the bone mineralisation rates and a significant positive trend in the calcium balances. It is suggested that acidosis contributes to the pathogenesis of osteomalacia in renal failure by slowing skeletal mineralisation, possibly by inhibiting bone alkaline phosphatase.

17 patients with recurrent calcium-containing-renal calculi were studied using the short NH4CI test and one subject with ‘incomplete renal tubular acidosis’ was identified. In retrospect the only clue to this diagnosis was a fasting, morning urine pH exceeding 6.0 units. Fasting morning urine pH, which is usually less than 6.0 in subjects who acidify normally, is proposed as a simple screening test for ‘incomplete RTA’. Modified high dose NH4CI tests and Na2SO4 tests were performed in this subject and other patients with either complete or incomplete distal renal tubular acidosis. 摘要:
Two cases of pericardial pseudocyst which developed in the course of maintenance hemodialysis are reported. Both patients were usually free of complaints, although there was evidence of the previous pericarditis with recent accumulation of pericardial effusion. As a possible mechanism leading to pericardial pseudocyst formation, distension of a weak area of the thickened pericardium due to an increase in intrapericardial pressure was assumed. Therefore, pericardial pseudocyst is one of the likely complications of patients on maintenance hemodialysis.

In this study, rats recovering from glycerol-induced acute renal failure were found to be protected from mercury-induced nephropathy, and HgCl2 poisoning protected rats from developing myohemoglobinuric renal failure. In view of the widely disparate nature of the renal failure models used, refractoriness appears to relate to an altered sensitivity of the organism itself rather than reflecting resistance to a particular nephropathic challenge. Renal renin content of the rats at the time of rechallenge was normal or high, a finding which contrasts sharply with that of chronically saline-loaded animals which also are refractory to ARF but have a maximally suppressed renal renin content. Renal renin depletion is not essential to the prevention of acute renal failure in the rat.

摘要:
Five patients with Salmonella typhimurium infection after a renal cadaveric graft are described. Salmonella were isolated from the urine of four patients, from the stool of one patient, from the blood of two patients, from the hip joint of one patient and from the cerebrospinal fluid of one patient. Infections were difficult to eradicate and necessitated prolonged antibiotic treatment. Renal function only deteriorated after the infection in some patients; salmonellosis could have triggered the rejection of the graft. Impaired cell-mediated immunity due to immunosuppressive drugs may be considered to be a predisposing factor for this kind of infection. Higher humoral antibody titers against Salmonella were found in the patients most clinically ill.

摘要:
Two and three Dow Cordis Hollow Fiber and Gambro Lundia artificial kidneys were used for high efficiency dialysis in large patients, intoxications, and for rapid ultrafiltration. BUN clearances of up to 240 ml/min were achieved. One episode of severe neuropathy occurred during high efficiency dialysis, and hypotensive episodes were more common. High efficiency dialysis has certain indications particularly in intoxication cases. However, because of possible side effects more experience is needed before its ultimate place in the treatment of uremia can be ascertained.
