摘要:
Serum 25-hydroxycholecalciferol (25-OH-D3) was determined in 20 normal persons and in 50 patients with renal failure. Low serum levels were found in patients with markedly impaired renal function while patients on chronic hemodialysis exhibited normal values. Since uremia is assumed not to influence the resorption and hepatic conversion of vitamin D the decreased 25-OH-D3 levels are discussed to be due to low vitamin D intake on protein-restricted diet.

摘要:
The metabolic activity of bone marrow cell suspensions was studied by the modified Lajtha method on samples from normal subjects, from nondialyzed and dialyzed uremics and from patients with hypoplastic anemia. It was found that: (1) the metabolic activity of bone marrow cells of the uremic nondialyzed patients is reduced as compared with the controls, and (2) the metabolic activity was reduced when marrow cells from normal subjects were incubated with creatinine, with methylguamdine or with uremic serum. These findings suggest that creatinine and methylguamdine play an important role in disturbing the cellular metabolism in uremia and they confirm that the anemia of chronic renal failure is largely due to metabolites which interfere in the maturation and proliferation of the cells.

The following conelusions can be drawn from this study of the clinical and morphological features of perimembranous glomerulonephritis (PGN) in 100 patients and in 31 patients who had serial renal biopsies: (1) PGN appears in every age group and is especially frequent between 31–40 and 51–60 years; it occurs more often in males than in females; (2) PGN begins in most cases idiopathically without preceding diseases; (3) the predominant early symptoms of PGN are edema and/or proteinuria; (4) the nephrotic syndrome is the most striking symptom when PGN is fully developed; (5) Hypertension is frequently observed in patients with PGN; the combination of the nephrotic syndrome and hypertension is observed more frequently than the nephrotic syndrome and normal blood pressure; the combination of hypertension, hematuria and the nephrotic syndrome is also frequently observed; (6) proteinuria is most marked in the early stages of PGN and decreases as the transformations of the glomerular basement membrane progress; (7) PGN is a proliferative glomerular disease, but the proliferation is mild and may only be a proliferation of the mesangial cells; (8) the prognosis of PGN is unfavorable; clinical improvement can occur without an improvement in the morphological features of the glomerular basement membr

摘要:
Four crude glycoproteins were isolated from supernatant of a Wistar rat kidney cortex homogenate. Three were soluble in a saline solution at pH 7.5; a fourth was prepared from a trypsin hydrolysate. These glycoproteins appear to be specific components of renal cortical tissue. One of them (GP1) is similar to rat GBM collagen-like fraction; the others (GP2, GP3, GPT) are non-GBM renal glycoproteins. There is a discussion of their origin which compares the results of the current study with those obtained during previously published studies, especially those dealing specifically with GBM.

Postmortem tissue analysis revealed significant potassium deficits in 58% of 27 patients maintained on chronic hemodialysis for periods of 2–75 months against dialysates containing 0–2.6 mEq/l of potassium. Potassium depletion was observed with all dialysate potassium concentrations but was greatest in patients dialyzed against potassium-free baths. The potassium deficit occurred primarily in skeletal muscle, myocardium, and brain. The present studies compare clearance (K) for varying molecular weight (MW) solutes, Urea (U), Uric acid (Ua), BSP and B12 in hollow fiber kidneys (HFK) before and in reused hollow fiber kidneys (RHFK) after five hemodialyses. In 10 RHFK, 3 % H2O2 75 ml. RHFK with H2O2 appeared cleaner than with W. KU was 168 ± 6,134 ± 12, and 152 ± 10 ml/min; KUa was 144 ± 11, 89 ± 12, and 101 ± 6 ml/min; KBSP was 50 ± 7, 29 ± 7, and 42 ± 6 ml/min, and KB12 was 15 ± 2,7 ± 2 and 11 ± 2 ml/min with HFK before, RHFK with H2O2, and RHFK with W, respectively. The percentage decrease in K was greater for higher MW solutes. HFK cleaning with H2O2 before dialysis did not change solute transport markedly. The data suggest: (1) there is relatively greater decrease in middle molecule clearance than small molecule in RHFK, and (2) with comparable FBV, clearance is better (p < 0.01) for RHFK with W than RHFK with H2O<

The ultrafilterable fraction (UF/P) of 131labeled o-iodohippurate (OIH) was measured in vitro and the effects of varying plasma OIH concentration (pOIH), plasma protein concentration (pPr) and plasma pH (ppH) determined. Dialysis coils were perfused with saline and human plasma at 38°C and pOIH varied (n = 46; pPr = 6.3–7.1 g/100 ml; ppH = 7.2–7.6), pPr varied (n = 126; pOIH = 50 mg/l00 ml; ppH = 7.2–7.6), or ppH varied (n = 121; pOIH = 84–106 mg/l00 ml). In this report, we present the case history of a patient who developed a brief period of oliguria following acute tubular necrosis. Obstructive uropathy and renal underperfusion were ruled out as etiologic causes of oliguria. While glomerular filtration rate (GFR) returned rapidly toward normal, an unusually prolonged and massive diuresis and saluresis (as high as 45 liters/24 h and 5,700 mEq Na/24 h) ensured for almost 1 month; following this, remission was completed over a matter of days. We believe this case dramatically illustrates the importance of functional maturation of tubules in recovery from acute tubular necrosis, and the protective effect against excess fluid and salt losses afforded by decrease in GFR usually seen in this syndrome.
