Survival after serum creatinine exceeded 10.0 mg/l00 ml was increased if one peritoneal dialysis was performed. After one peritoneal dialysis ‘survival’ of 62 patients to the present time, death or the next dialysis averaged 119 days; median survival was 28 days. Characteristics of prolonged survivors were persistent reduction of serum creatinine below the predialysis concentration, tubulointerstitial diseases, i.e. polycystic kidneys, pyelonephritis or obstructive nephropathy, potentially reversible complications such as urinary tract infection or extracellular volume depletion, and high urine volume, and low blood pressure. The authors report on the myocardiopathy of seven patients with end-stage renal disease, characterized by fever, in four patients; tachycardia, gallop rhythm, pericardial rub, in most of patients; signs and symptoms of cardiac failure and increased cardiac area and alterations of the EKG in all patients. Good remission of the symptoms was achieved in two patients by hemodialysis and by successful renal transplantation in the others. The likely etiologic factors of the syndrome are discussed.

In an attempt to alter the natural course of glycerol and uranyl nitrate induced acute renal failure in the rat, the vasodilator, prostaglandin A1 was administered systemically at varying times during the induction phase of renal failure. According to the method used, no significant beneficial effect could be demonstrated in either experimental model.

The relationship between plasma potassium concentration and the renin-angiotensin-aldosterone system was evaluated in ten patients with chronic renal failure (creatinine clearance 10–56 ml/min). Under basal conditions and following various stimulation maneuvers, normokalemic patients demonstrated normal plasma renin and aldosterone levels. Five of six patients with hyperkalemia had diminished function of the renin-angiotensin-aldosterone system; their ability to conserve sodium during salt depletion was less than that of normokalemic patients. The data suggest that the maintenance of plasma potassium levels in these patients is dependent on the presence of a normally functioning renin-angiotensin-aldosterone system; aldosterone activity may be an important determinant of sodium conservation in patients with renal failur

Despite recent attempts to improve classification and to provide data on pathogenic mechanisms, there is a paucity of information concerning the clinical aspects of bilateral renal dysplasia. Therefore, the clinical features and long-term prognosis of eight patients with bilateral renal dysplasia have been evaluated. Three important observations were made, namely (1) a large proportion of patients had an impairment of urinary concentration and acidification, (2) the clinical course was characterized by progressive renal deterioration in patients with bilateral involvement, and (3) four children with renal dysplasia had the clinical features of Laurence-Moon-Biedl syndrome.

A method of graded sieving for counting of glomeruli in removed kidneys is described. The number of nephrons (glomeruli) in six normal kidneys deriving from autopsies of three children 22–73 months of age was 1.82 ± 0.35 × 106. The results give a little higher number than most previous studies with which they are compared. In the kidneys of two 10- and 15-month-old patients with the congenital nephrotic syndrome of the Finnish type the mean number of nephrons was 3.17 ± 0.52 × 106 which is on the average 73 % higher than in the normal kidneys studied. The number of nephrons in kidneys of five other patients showed an average of at least 3.42 ± 0.30 × 106 the counting method being a little different. The presence of testosterone is a prerequisite for the development of chloroform-induced renal tubular necrosis in mice. The purpose of this study was to check whether methyltestosterone exerts the same influence. Castrated males and females were treated with one of these compounds: those treated with testosterone developed renal tubular necrosis after chloroform injection, those which received methyltestosterone were free of this lesion.
