摘要:
The effect of hemodialysis on myocardial function in 10 pediatric chronic hemodialysis patients was assessed utilizing echocardiography. Parameters of cardiac function were measured prior to and immediately after dialysis. A significant decrease in cardiac output was observed post-dialysis. Unlike that observed in adult patients a real improvement in myocardial function as indicated by an increase in the mean velocity of circumferential fibre shortening was evident post-dialysis.

摘要:
An unusual case of rickets associated with hypercalciuria is described. In addition to proteinuria, the patient had phosphaturia, aminoaciduria, renal glucosuria and impaired renal concentration but no renal tubular acidosis. Studies did not support the diagnosis of primary hyperparathyroidism. The findings in the patient were very similar to those in 4 previously reported cases and are suggestive of a new combination of multiple renal tubular defects.

摘要:
Gastrointestinal protein loss was studied by intravenous application of 51Cr-albumin and the measurement of fecal excretion of 51Cr. As compared to controls, fecal excretion of 51Cr was largely increased in 28 patients with nephrotic syndrome due to glomerular disease and in 11 patients with hypoproteinemia in the course of different gastrointestinal disorders. It was not increased in nephrotic patients without primary glomerular disease and in renal insufficiency without nephrotic syndrome. Controversial data of the literature are discussed with regard to different methods and patient groups.

摘要:
Phenytoin binding to uremic plasma was studied in vitro. Inhibition of binding independent of albumin concentration was demonstrated. Acidification from pH 8 to pH 3 produced a small decrease in binding by normal plasma but no change or a small increase in binding by uremic plasma. After plasma was acidified to pH 3.0, passed through an anion-exchange resin and realkalinized to pH 8, the binding by uremic plasma was restored to near normal. These studies indicate that the uremic abnormality causing impaired plasma drug binding is reversible and supports the concept that a competing ligand, tightly bound at physiologic pH, is responsible for impaired drug binding.

Plasma renin activity and extracellular fluid volume were determined in 34 normotensive and in 35 sustained essential hypertensive patients with normal renal function, balanced sodium intake and urinary output. In normotensives, plasma renin activity was negatively correlated to extracellular fluid volume (r =-0.54; p =0.001). The 95% confidence limits of the normotensive curve was used as nomogram to classify the hypertensive patients into two groups: those (23 cases) that fell within the limits of the normal curve (group I) and those (12 cases) that were below these limits (group II). In comparison with group I, group II was characterized by: (i) similar values for age, blood pressure, inulin clearance and extracellular fluid volume and (ii) significantly but lower values (p < 0.001) for plasma renin activity with maintenance of the relationship between extracellular fluid volume and renin. The study strongly suggests that (i) the hypertensives of group I had no abnormal regulation of the renin-angiotensin system in comparison with the control subjects and (ii) the hypertensives of group II had an extracellular fluid volume-renin relationship set for lower values of renin.

摘要:
The clinical, laboratory and EEG findings of 4 uremic patients on hemodialysis who accidently developed acute hypercalcemia were reviewed. An acute central nervous system syndrome developed, associated with the clinical changes of disorientation, dysarthria, seizures, myoclonic jerks, hallucinations, irritability, confusion, memory and judgment defects plus bizarre behavior. The EEG findings demonstrated diffusely severe slow background activity in all tracings. In addition, the EEG abnormalities as well as the clinical findings disappeared when serum calcium returned to normal. Hypercalcemia, a reversible condition, seems to have been the cause of this clinical syndrome which should be differentiated from dialysis dementia, a condition known to be irreversible and fatal.

摘要:
In a renal failure patient, asthma developed after 1 year hemodialytic treatment. The attack was exclusively associated with each hemodialysis. Skin test to the usual allergen such as house dusts or pollens was negative. The provocation test by inhaling acetate solution or oral intake of acetic acid was positive resulting in an increase in the pulmonary resistance, or asthmatic attack. This asthma disappeared completely after substitution of acetate in the hemodialysis by the use of a bicarbonate dialysate. Acetate might therefore precipitate the attack of asthma.

In rats kept under starvation for 25 h after bilateral nephrectomy VO2 is reduced to about 50% of the normal value. The body temperature is conspicuously low (33–34°C), the blood pH about 7.1. Raising the body temperature to normal values by keeping the rats in a thermostat does not influence the low metabolic rate. The prevention of acidosis by repeated application of bicarbonate in the postoperative period has no effect on VO2. Tube feeding with glucose + oil or glucose + oil + casein does not improve the metabolic rate. However, the total glycerol plasma levels, already high in the state of starvation, are still more elevated. It is concluded that the primary effect of acute uremia is to suppress the metabolism, as a consequence of which the body temperature is lower
