The results of clinical, functional and renal biopsy follow-up studies performed in 2 series of adult patients with glomerulonephritis are reported. The first series included 35 cases which were divided into 4 groups according to the histological picture, namely cases with ‘minimal’, ‘proliferative’, ‘membranous’ and ‘chronic’ lesions. These cases were treated with Azathioprine alone (16 cases) or combined with prednisone (19 cases) for 1 to 2 years. A true recovery-rate (based on the disappearance of the clinical, biochemical and renal biopsy lesions) was found in 31,4% of the cases. The second series consisted of 15 immunosuppressive-resistant cases. A lymphatic depletion was obtained by Thoracic Duct Fistula, and was estimated as satisfactory in 11 of the cases with an average output per fistula of 50 × 109 lymphocytes and 36,91 g of immunoglobulins. Post-fistula immunosuppressive therapy was in each case the same as the pre-fistula one. Three cases completely recovered, 4 cases improved, 4 cases were unchanged. The effect of calcium infusion on serum levels of calcium and phosphorus and on the fractional excretion of phosphorus (Cp/Ccr) was studied in eight normal subjects and in 35 patients with varying degrees of renal failure. The increment in serum calcium in patients with advanced renal failure and radiographic evidence of osteitis fibrosa was significantly less than that observed in patients with a comparable renal function but without evidence for such bone disease. The increase in serum phosphorus usually observed after calcium infusion in normal subjects was also seen in patients with chronic renal disease but was less marked as renal failure worsened. In patients with advanced renal failure and osteitis fibrosa serum phosphorus fell after calcium infusion. The fall in Cp/Ccr following calcium infusion in patients with mild renal failure was similar to that observed in normal subjects (50 to 90%), but was diminished in patients with advanced renal failure; in the latter group the maximal decrements in Cp/Ccr were less than 25% in most patients irrespective of the presence or absence of osteitis fibrosa and overt secondary hyperparathyroidism. The results indicate that in patients with advanced renal failure: (1) the usefulness of changes in Cp/Ccr following calcium infusion as a diagnostic or prognostic guide is questionable ; and (2) the difference in the changes in serum calcium and phosphorus after calcium infusion may prove to be helpful in differentiating between uremic patients with and without marked osteitis fibrosa.

Chez le rat, la morphologie des cellules en aval de l’anse de Henlé est modifiée lorsque l’animal est placé dans une situation physiologique où la réabsorption des bicarbonates est accrue. L’indice intercalaire ou nombre de cellules par section du tube distal ou de la portion corticale du collecteur augmente après administration per os de 3,45 mM de Na H CO3, de K H CO3 et de N H4 CO3 et à un moindre degré après charge osmotique. L’acétazolamide empèche l’effet du Na H CO3. L’acidose gazeuse provoque un accroissement deux fois plus important encore de l’indice intercalaire. En outre, toutes les cellules claires sont modifiées, prenant certains caractères des cellules intercalaires. On en conclut que cellules claires et intercalaires ne sont que deux aspects fonctionnels d’une même lignée et que les changements morphologiques constatés correspondent à l’augmentation de la «réabsorption» des bicarbonates. Glomerular filtration rate, the renal clearances of sodium, potassium and chloride, and several parameters of urinary acidification were measured separately in the “clamped” and “undamped” kidney of rats after acute (normo-tensive rats) or chronic (hypertensive rats) clamping of one renal artery. Both acute and chronic constriction of one renal artery resulted in a decrease, in the clamped kidney, in GFR, the excretion of sodium, potassium, chloride, ammonium and bicarbonate ions, and the absolute rate of bicarbonate reabsorption. Bicarbonate reabsorption factored by the volume of glomerular filtrate was significantly, though slightly, increased after chronic constriction of a renal artery. Urinary pCO2 was higher than plasma pCO2 in the undamped kidney (positive A pCO2) but not in the clamped kidney (null or negative A pCO2) of renal hypertensive rats. Intravenous injection of carbonic anhydrase abolished the positive A pCO2 present in the undamped kidney. Injection of acetazolamide induced a positive A pCO2 in the clamped kidney. The present data confirm the existence of a glomerular-tubular balance for bicarbonate reabsorption, and possibly for hydrogen secretion, which remains active after constriction of the renal artery. The slight enhancement of bicarbonate reabsorption per unit volume of glomerular filtrate as well as the disappearance of the A pCO2 in the clamped kidney may be due to a relative excess of luminal (brush border) carbonic anhydrase and/or to a prolonged contact time of tubular fluid with this

Proximal and distal tubular segments of the subcapsular nephron and segments of the ascending limb of Henle’s loop were dissected freehandedly from lyophilized kidney section of moderately dehydrated rats after furosémide (100 mg/kg body weight). An 80% inhibition of Na+K+ ATPase activity after furosémide was obtained by quantitative histochemical measurement in the ascending limb of Henle’s loop and in the distal convoluted portion only. These results demonstrate: First, that furosémide acts mainly within the ascending limb of Henle’s loop and the distal convoluted portion where the Na+K+ ATPase likely mediates sodium transport. The nature of the stimulus(i) that initiates renal hypertrophy and hyperplasia is unknown. This investigation assays plasma of uninephrectomized rats for activity of renotropin(s), a humoral factor(s) which has been postulated to control compensatory renal growth. Plasma taken from rats 24 hours after uninephrectomy when compared to plasma taken from sham operated animals stimulates thymidine incorporation into DNA and uridine incorporation into RNA in kidney slices but not in liver slices. Stimulation is not seen in plasma from bilaterally nephrectomized rats. It is concluded that the assay described shows the presence of a circulating renal growth factor which is probably produced in response to a decrease in renal mass.
