The dialysis nurse has a unique role in the care of her patients. The uniqueness of her role is directly related to the special stress long-term hemodialysis mediates upon the patient. This stress often promotes significant psychological regression, with its attendant problems of immature behavior, dependency and even overt psychiatric symptoms. The nurse, because of her special relationship to the patient, is the first and often the only professional to deal with these problems. How well she can deal with this responsibility depends on several variables. The role she learned in her training will dictate her propensity to innovatively respond to the patient. A pleasant relationship with the medical staff will greatly sustain the personal comfort necessary for such demanding work. Participation in patient selection helps her understand the purpose and nature of her commitment.

We attempted to assess the significance of the serum transferrin level as an index of the nutritional state of patients with chronic renal failure. The transferrin concentration was measured by radial immunodiffusion in serum from 79 healthy subjects and 54 patients with chronic renal failure. Serum transferrin levels were low in patients with both mild and severe renal failure; the lowest levels were found in patients in a poor nutritional state.

Studies of a 60-year-old woman with renal tubular acidosis, a Fanconi syndrome and nonmyelomatous hypergammaglobulinemia are described. They demonstrate bicarbonate wasting in association with a normal capacity for acidification of the urine when serum bicarbonate was educed by ammonium chloride loading. The presence of a 1:2,500 latex titer, hypergammaglobulinemia, positive Schirmer’s test, leukopenia and interstitial nephritis suggest that the patient has a disorder closely related to Sjögren’s syndrome. A case of Wegener’s granulomatosis in a 51-year-old white male is described. His initial clinical course was characterized by severe renal and pulmonary insufficiency. Renal biopsy showed severe proliferative glomerulonephritis with crescent formation plus intraglomerular fibrin deposition. Because of the seriousness of the renal lesion along with the demonstrated fibrin deposition, the patient was treated with heparin in addition to azathioprine and methylprednisolone. A prompt improvement in renal function was noted following heparin administration. After 9 months, heparin was stopped while azathioprine and prednisolone were continued. Renal function deteriorated and then improved after reinstitution of heparin therapy. The initial chest roentgenogram and pulmonary function tests were compatible with Goodpasture’s syndrome. A technique of estimating the mean hydrostatic coil pressure of the blood path (MCP) by abruptly stopping blood flow and recording the maximum outlet pressure was evaluated. It was found that this technique underestimates MCP by differing amounts depending on blood flow rate and hematocrit. Differences between estimated and measured MCP in different coil types at a variety of flow rates and hematocrits are reported. The small magnitude of the differences permits close estimates of MCP using a simple correction factor. Precise estimates of MCP, however, require measurement of hematocrit and flow rate with the technique.
