Serial assays of plasma fibrinolytic activity, euglobulin lysis activity, fibrinogen, and plasminogen were carried out in 13 children with acute post-streptococcal glomerulonephritis (APGN). Observations were started 2 to 16 days after the apparent onset and were continued until the clinical recovery. A marked increase in plasma overall fibrinolytic activity was found in 12 out of 13 patients during a relatively early stage of the disease. The fibrinolytic activity was due to an increase in plasma activators level as it was shown by the elevated activity of the euglobulin fraction of the plasma. Fibrinogen was increased in most patients and plasminogen level was lowered in some of them. The increase in plasma fibrinolytic activity was no longer detectable during the subsiding phase of the disease. Plasma fibrinolytic hyperactivity probably relates with the removal of fibrin from the glomeruli.

Le pH et la P CO2 urinaires sont étudiés au cours d’une surcharge par perfusion intraveineuse de bicarbonate de sodium chez neuf insuffisants rénaux chroniques dont I’acidose a été préalablement traitée les jours précédents, et chez dix sujets normaux à titre de groupe témoin. Le pH urinaire augmente dans les deux groupes mais reste inférieur chez les urémiques. La P CO2 urinaire reste voisine de la P CO2 artérielle chez les urémiques alors qu’elle s’élève franchement chez les sujets normaux. La non-augmentation de la P CO2 de I’urine chez les urémiques n’est due ni à la moindre valeur du pH urinaire, ni au débit urinaire plus faible chez ces sujets. II est impossible d’exclure les rôles éventuels de la diurèse osmotique, d’une altération du système de I’anhydrase carbonique, d’un abaissement de la P CO2 papillaire chez les insuffisants rénaux. La moindre élévation du pH urinaire de ces sujets est dûe à la dilution de I’urine déterminée par la diurèse osmotique. En effet, le débit des bicarbonates urinaires rapporté à la filtration glomérulaire est voisin dans les deux groupes alors que la fraction excrétée de I’eau filtrée est très supérieure chez les urémiques. Ammonium chloride was intravenously infused in a group of sea-leve. residents and in a group of high-altitude natives living at 4300 m above sea level. Blood pH, and buffer base; plasma bicarbonate, chloride, sodium and potassium; and urine pH, ammonium and titratable acidity were determined before and after the ammonium infusion. Blood pH, pCO2 blood buffer base, serum bicarbonate and chloride, which in basal conditions differ in both groups, changed in proportion to the basal figures both at the end of infusion and during recovery. Urine pH, ammonium and titratable acid responded similarly in both groups. The results are interpreted as indicating that the high-altitude native is in a different state of acid-base equilibrium. His lower blood bicarbonate concentration does not diminish his overall body defense capacity against metabolic acidosis.

Leaks in the Twin Coil Artificial Kidney Machine were investigated for possible contamination of the patient’s blood by the dialysate solution using bacteriological techniques. Our results show that as little as 0.001 cm3 of dialysate is sufficient to produce overwhelming growth in Transfuso Vac pint bottles of blood. Thus, we would have been able to detect even the smallest contamination of blood by dialysate fluid. Although blood in itself has an effect on bacteria, this effect appears easily overcome by introduction of a very small amount of microorganisms. Over a period of 11 months, 58 leaks were detected. 摘要:
Plasma renin activity (p.r.a.) was studied in 37 patients with acute renal insufficiency and in 20 healthy subjects. Significant increase of the p.r.a. in the oliguric phase of acute renal failure could be established. The p.r.a. decreased in the diuretic phase of this syndrome. No straight correlation between elevated p.r.a. on the one hand and the degree of hypertension and plasma sodium and potassium on the other hand could be found. The possible role of renin in the development of acute renal insufficiency is discussed and the importance of p.r.a. estimations in the differentiation of acute renal failure from exacerbated chronic renal disease is underlined.

The role of the concentrating mechanism in the potentiation of acute renal failure by dehydration has been studied using rats with complete diabetes insipidus. These rats were shown to be continually dehydrated even when allowed free access to water. Despite massive volumes of highly dilute urine at the time that hemoglobinuria was induced by glycerol injection, these animals developed acute renal insufficiency whose severity was intermediate between that of non-dehydrated and severely dehydrated normal rats treated similarly. Micropuncture experiments showed the appearance of the kidney in vivo and the functional abnormalities of the three types of rats to be qualitatively identical. It appears that dehydration’s effect on hemoglobinuric acute renal failure is independent of the concentrating mechanism and correlates well with plasma volume depletion. 摘要:
Inhibition of renal hippurate transport in vitro has been reported previously. Since many assessments of renal blood flow and tubule function are dependent on hippurate transport, the question of whether inhibition by substances present in azotemia occurs in vivo is important. These studies show that in azotemia, induced by two different methods, renal hippurate accumulation is inhibited in the kidney of rats in vivo. As in the in vitro model, oral neomycin feeding can reduce some of this inhibition. The similarity between the in vivo and in vitro models suggests that a common mechanism for inhibition of hippurate transport is present in both models.
