Sixteen patients with generalized vitiligo were treated with oral L-phenylalanine and UVA (PAUVA). The patients were treated twice weekly for six months, taking 50 mg/kg body weight of phenylalanine 45 minutes before exposure to UVA radiation. The UVA dose was carefully graduated and increased in increments of 30–50% every third treatment according to the patient's response. Another ten patients with generalized vitiligo were treated with UVA alone and acted as a control group. None of the patients in either group showed significant improvement. No side-effects were observed during the study.

Cutaneous CD30+lymphomas are a group of lymphoproliferative disorders classified recently. Within this group the form of primary cutaneous CD30+large cell lymphomas, characterized by the onset of solitary or localized nodules, seems to have a favourable course, sometimes with spontaneous regression. Thus, a nonaggressive treatment is a first-choice therapeutic approach. Dermatological radiotherapy using contact, soft or half-deep X-rays was performed on eight patients in the period 1991–1995, with a median total dose of 20 Gy per field of irradiation. The mean follow-up was of 24.75 months. All the lesions treated healed completely and up to now no relapse has been observed in the irradiated areas. Radiotherapy was always well tolerated and the cosmetic results were good in six patients (75%) and acceptable in two (25%). The preliminary results in this small series of patients support the choice of dermatological radiotherapy in the treatment of primary cutaneous CD30+large cell lymphomas with solitary or few lesions.

The cause of folliculitis decalvans is still debated.Staphylococcus aureusis often cultured from the lesions, but it is not the sole factor involved in the pathogenesis of the disease. Folliculitis decalvans is known for its resistance to treatment, resulting in an unfavourable prognosis. We report a case that resolved after two cycles of therapy with rifampin. We discuss the mechansims of action of rifampin and the pathogenesis of folliculitis decalvans.

A 61-year-old male patient with lichen planus with typical cutaneous and oral mucosal lesions is reported. The oral lesions became erosive and bullous. Various therapies were attempted without success. Finally, cyclosporin A was initiated in increasing doses up to 4 mg/kg per day. The previously therapy-resistant oral lesions resolved completely within 3 months. Cyclosporin should be considered as a therapy for serious erosive bullous oral lichen planus.

Bullous hemorrhagic cellulitis is a severe bacterial infection characterized by the rapid development of hemorrhagic bullae, skin necrosis and possible progression to necrotizing fasciitis. It is caused by a number of organisms including group A beta hemolytic streptococcus, gram negatives and anaerobes. The pathogenesis of group A beta hemolytic streptococcal infection, apart from the inherent strain virulence, is felt to be in part due to tumor necrosis factor (TNF) alpha production by the host. We present a case of bullous hemorrhagic cellulitis due to group A beta hemolytic streptococcus successfully treated with the addition of IV steroids and pentoxifylline to the antibiotic regimen. Both corticosteroids and pentoxifylline inhibit TNF alpha activity and its secretion by macrophages.

Vitamin D3analogues interfere with various aspects of cutaneous inflammation, keratinization and epidermal proliferation, and are effective in chronic plaque psoriasis. Recently their clinical efficacy has been investigated in other skin disorders. So far, the vitamin D3analogue calcipotriol has proved to be effective in pustular psoriasis, congenital ichthyosis, inflammatory linear verrucous epidermal naevus, Graver's disease and pityriasis rubra pilaris. Information on the effect of other vitamin D3analogues in these conditions is limited.
