In several studies, dietary fish oil has been found to be beneficial in psoriasis, although the results have been controversial. To overcome the slow kinetics and limited availability of oral supplementation, a patient with acute relapsing psoriasis received anω-3 fatty acid-based lipid emulsion (100 ml/day containing 2.1 g eicosapentaenoic acid (EPA) and 2.1 g docosahexaenoic acid) for 10 days. Clinical symptoms were assessed using the PASI. The PASI score dropped from 18.4 to 4.2. In addition leukotriene generation of ionophore-stimulated neutrophils showed a more than tenfold increase in EPA-derived lipoxygenase product formation. Because of a renewed exacerbation 1 year later, a second trial was started with double the previous dose over a period of 15 days. Probably because of a worse clinical condition, the patient responded more slowly than during the first trial, and the increase in EPA-derived metabolites was more retarded. Modulation of eicosanoid metabolism by intravenousω-3 fatty acid supplementation appears to have a rapid and reproducible beneficial effect on inflammatory skin lesions in acute relapsing psoriasis.

The clinical efficacy of antidandruff shampoos containing antifungal agents is correlated with their anti-Malasseziaactivity. Other ingredients of the shampoos also affect the efficacy and tolerability of the treatments. This study was conducted to quantify the effect of eight proprietary antidandruff shampoos on the human stratum corneum. Corneosurfametry and squamometry were used to rate the interaction between the shampoos and the stratum corneum. Significant differences in mildness were disclosed between some test shampoos and SLES-7EO selected as a mild reference surfactant. The activity against corneocyte clumping also showed significant differences among the test products. It is concluded that the formulation of shampoos for dandruff should not be overlooked.

A group of 34 women with vulvar vestibulitis syndrome were treated with topical ketoconazole cream three times a day. Improvement was seen in 15 out of the 34 patients for 8 months (44%) as measured using an electronic force probe.

摘要:
In addition to the importance of bioavailability to reliable therapy, the three leading tetracyclines used in the treatment of acne have dissimilar tissue distributions and lipophilic characteristics which may impart different antimicrobial efficacy. The purpose of this study was to compare the in vivo antimicrobial effects of minocycline, doxycycline and tetracycline. The results of this study confirm that normal dosages of minocycline hydrochloride capsules in fast-release powder form produced significanctly superior in vivo reductions inPropionibacterium acnescompared to that produced by normal dosages of doxycycline monohydrate and tetracycline hydrochloride. In addition, minocycline produced much greater residual antimicrobial activity than the other two drugs after the end of the treatment period.

In a prospective double-blind left-right comparative study, a cream containing 13-cis-retinoic acid 0.1% (13-cis-RA) and its cream base were applied to a test patch of skin of seven patients with Darier's disease for periods ranging from 3 to 10 weeks. A half-side reduction in papules was observed in four patients. The treatment had no effect on scaling and increased erythema in two patients. Clinical improvement of papules was statistically significant, whereas scaling and erythema did not change significantly. Side-effects of the study medication were only minimal and easily controlled by reduction of the application frequency. Biopsies for immunohistochemical examination were taken from representative skin lesions, one before and one from each side after treatment. Keratins 4 and 13 were induced by the treatment with topical 13-cis-RA in three and two patients, respectively. In conclusion, topical 13-cis-RA 0.1% cream is a well-tolerated therapy for Darier's disease. Induction of keratins 4 and 13 seems to offer an immunohistochemical tool for the investigation of the mode of action of 13-cis-RA.

Balneological therapy with a bath oil containing polidocanol (Balneum Hernial Plus) was used to try to reduce uraemic pruritus in 30 haemodialysis patients. All patients were given oil baths every 1 or 2 days, and the treatment was continued for 4 weeks. After termination of the treatment the pruritus had disappeared completely in 26 patients, and had improved markedly in 3 and moderately in 1 patient. The treatment was well tolerated and no adverse effects were noted. At follow-up, 4 weeks after termination of the treatment, the pruritus had disappeared in 10 patients and improved in 14, and the treatment had failed in 6 patients. Of the 30 patients, 25 judged this treatment to be very good or good, 3 considered it as moderate and 2 stated that it was without effect. This study showed benefits of balneological therapy with a bath oil containing polidocanol in the treatment of uraemic pruritus. Continuous therapy is recommended, as recurrences after discontinuation of the treatment were observed.

The efficacy of the combination of 20% azelaic acid cream and 0.05% tretinoin cream in the treatment of melasma was compared with that of 20% azelaic acid cream alone in an open-label randomized study (n= 50). Over the 6-month treatment period both treatments yielded approximately 73% therapeutically meaningful (rated as‘good’or‘excellent’) results. Tretinoin augmented the depigmenting effect of azelaic acid as shown by a faster response and a more pronounced improvement with the combination treatment during the first 3 months, and by a higher rate of excellent results at the end of therapy. Both treatment regimens were well tolerated, and no hypochromia, photosensitivity reactions or exogenous ochronosis were observed.

Outpatient UVB phototherapy and inpatient tar therapy are commonly used treatments for psoriasis. In practice, the treatments often represent therapeutic alternatives, but while the use of UVB radiation has been the subject of many randomized controlled trials, the use of tar is purely empirical. We therefore conducted an audit to compare the efficacy of these two treatments for psoriasis. The case notes of 59 patients were reviewed and compared. Tar therapy had a significantly shorter overall treatment time than UVB therapy (3.4 vs 7.8 weeks,P<0.0001), even though the patients treated with tar had a significantly higher disease severity score on admission (2.7 vs 1.9,P0.0001). Tar treatments were given significantly more frequently (6.4 per week) than UVB treatments (3.0 per week). The number of UVB treatments given during a completed course was not significantly different from the number of tar treatments. No difference in the outcome of the treatments was observed. The results suggest that both tar and UVB radiation will continue to be of clinical relevance to psoriasis therapy. Tar therapy seems to be faster, but this may be due to the higher number of weekly treatments. For hospitalized patients tar therapy seems to be superior to phototherapy, at least when UVB is not administered on every weekday. For outpatients it seems most reasonable to offer UVB treatment, because a complete treatment course involves fewer visits than tar therapy. In addition, phototherapy is cosmetically well accepted by nearly all patients and in that respect is superior to tar therapy.

This study was designed to evaluate the combined effects of the use of an emollient in conjunction with a potent topical corticosteroid and to compare the effects with the use of the same steroid alone in the treatment of patients presenting with chronic lichenified eczema. In this comparative randomized study, 40 patients (16 males and 24 females) were included and divided into two equal groups. Patients in group A were given clobetasol propionate ointment and those in group B clobetasol propionate with an emollient lotion, both for twice-daily application. The patients were followed up weekly for a period of 4 weeks and assessed clinically for pruritus, scaling and lichenification on a scale of 0–3. The mean sums of scores on week 0, 1, 2, 3 and 4 were 6.44, 5.05, 4.72, 3.62 and 2.61 in group A patients and 8.10, 5.42, 3.01, 1 and 0 in group B patients, respectively. Thus there was a highly significant difference in the clearance of lesions (P<0.001) in favour of group B (steroid with emollient) and this was achieved in a much shorter time.
