Psoriasis commonly affects the scalp and may represent a Koebner response to colonization with the Pityrosporon yeast microorganism. To test if treatment with topical selenium sulphide could induce longer remission times after healing of psoriasis of the scalp, we have performed a randomized double-blind placebo-controlled study of a shampoo containing selenium sulphide, proved to be active against the yeast. One group of 16 patients was treated with shampoo alone and one group of 36 patients was treated with shampoo in combination with Grenz rays (4 Gy weekly for 6 weeks), known to have a beneficial effect on psoriasis of the scalp. The results indicated that there may be a longer remission time with selenium sulphide shampoo in combination with Grenz rays compared to placebo shampoo and Grenz rays, although this difference was not statistically significant. The differences in severity scores before and after treatment between the selenium sulphide and placebo shampoo groups and between the selenium sulphide shampoo with Grenz rays and placebo shampoo with Grenz rays groups, was not statistically significant.

The effect of ebastine 10 mg once daily was studied in a double-blind comparison with placebo in 204 patients with chronic urticaria. Symptom severity, as assessed by both investigator and patient, was significantly reduced by ebastine after 7 and 14 days of treatment. The drug was well tolerated in the majority of patients, with the overall incidence of side-effects not significantly different to that of placebo. It is concluded that ebastine represents an effective and well-tolerated alternative to other non-sedating antihis-tamines in the treatment of chronic urticaria.

Hyperkeratosis of the follicular channel is the most commonly demonstrated abnormality in comedonic acne. Several reports have shown the effectiveness of azelaic acid against comedoaes in acne. The aim of this study was to investigate the activity against comedones of 20% azelaic acid (AZA) cream compared with 0.05% retinoic acid (RA) cream and placebo cream using clinical and scanning electron microscopy (SEM) evaluation. Skin surface biop sies with cyanoacrylate glue were performed before and at the end of four months of treatment, and SEM was used to count the number of comedones. The placebo group did not show a reduction in the number of comedones, but there was a significant reduction in the AZA and RA groups. AZA induced a reduction in follicular hyperkeratosis of a similar magnitude to that induced by RA.

In an open, multicentre study, patients with one to ten plantar warts on each foot were randomized prior to treatment for up to 17 weeks to receive either a keratolytic agent alone (Promicid gel) or the same agent covered by an occlusive plaster (Promicid occlusive dressing). A total of 170 patients completed the study. A statistical comparison of the two treatments showed that the use of a keratolytic agent and occlusion was significantly more effective than the use of the keratolytic agent alone. Treatment results were generally better for patients under the age of 14 years, and occlusion was particularly helpful for patients of 14 years of age or older. Both treatments were well tolerated. There were fewer sidesffects in the group treated with the keratolytic agent and occlusion.

The clinical efficacy and safety of 0.5% augmented betamethasone dipropionate ointment (Diprolene) and 0.05% diflorasone diacetate ointment (Psorcon) were evaluated in a multicentre randomized double-blind trial. A total of 164 patients with psoriasis from four geographic regions of the US were treated in this 2-week clinical study. One half received Diprolene ointment and the other half, Psorcon. After 2 weeks, both ointments significantly improved psoriasis (p<0.001), but Diprolene ointment held a slight, but significant, advantage over Psorcon ointment in clinical efficacy. There was a high patient preference for Diprolene ointment which led to fewer adverse reactions.

The first case of cutaneous cryptococcosis in a renal transplant patient successfully treated with fluconazole is reported. A 59-year-old renal transplant female patient receiving immunosuppressive therapy developed cutaneous cellulitis on the posterior aspect of the right thigh, which ulcerated. Diagnosis of cutaneous cryptococcosis was confirmed by culture and histopathological and mycological examination. There was no sign of involvement of other organs. Fluconazole 300 mg/day orally was started; after 93 days the dose was increased to 400 mg/day. Clinical, mycological and histopathological cure was obtained after 182 days of treatment. In spite of immunosuppressive therapy, fluconazole was successful in treating the disease, and there was no relapse over the following year. Treatment had no adverse influence on renal function.

The human skin blanching assay remains in widespread use as a reliable, qualitative, comparative indicator of topical corticosteroid availability and potency. The experimental refinements promulgated by certain researchers in this field have yielded a versatile bioassay for the accurate assessment of new drugs or delivery vehicles. With the increasing appearance of generic topical corticosteroid formulations which compete with trade-oame equivalents, the vital importance of this assay in regulatory affairs and assessing bioequivalence has been re-emphasized. It is stressed that if the blanching assay is to be used in this sphere, then multiplereading trials must be conducted; important registration or clinical decisions cannot be made with any validity from short-term assessments.

Chloracne is a rare but serious manifestation of systemic poisoning with polyhalogenated aromatic chemicals. It is alrqost invariably due to occupational exposure, often as a result of an industrial accident. Comedones, sometimes with inflammatory lesions, are the principal skin signs and most accurately reflect systemic toxicity. Hepatotoxicity, neuropathy and lipid abnormalities also occur. The skin lesions are resistant to most conventional acne therapies. Topical tretinoin may be of some benefit for comedones. Light cautery of comedones is successful and, although time consuming for the patient, many comedones can be treated in a 15 minute session. Limited experience with oral isotretinoin suggests that it is not very effective in chloracne. An outbreak of chloracne requires detailed investigation of the industrial process responsible to eliminate further exposure. The prognosis for mild cases of chloracne is good provided contact with the chloracnegen ceases.
