In slow-healing wounds, i.e. those with little granulation tissue with deep necrotic tissue and exudation, complicated by inflammation, there is depolymerization and degradation of local mucopolysaccharides, especially hyaluronic acid. A local deficit of hyaluronic acid leads to insufficient regeneration of connective tissue, poor angiogenesis and deficient differentiation of histiocyte and fibroblast populations. Exogenous hyaluronic acid applied to a debrided wound keeps it moist and ensures a high concentration at the site of action. Hyaluronic acid stabilizes the clot matrix, attracts inflammatory, mesenchymal and epithelial cells into the wound area and enhances angiogenesis of the clot tissue. It also exerts an antiexudative, vasoprotective and fibrogenic action in inflamed tissues of slow-healing wounds. In this multicentre controlled study we evaluated the efficacy and safety of hyaluronic acid in 50 patients with venous leg ulcers in comparison with Dextranomer, the product of choice for this indication in France. Objective parameters, including the appearance and dimensions of the ulcer, improved significantly in both treatment groups. However, there was a faster and greater reduction in the ulcer dimensions following treatment with hyaluronic acid. Both treatments improved surrounding erythema, pain, oozing and necrosis but only hyaluronic acid caused a significant decrease in oedema. A greater number of positive efficacy judgements were seen in the hyaluronic acid group. The local tolerability of the two treatments was excellent. Hyaluronic acid is therefore a safe and effective treatment for patients with venous leg ulcers.

At the Dead Sea spas, which are 400 m below sea level, both the ultraviolet radiation from the sun and the mineral-rich waters of the lake have special features. The separate influence of these two factors was studied in 81 patients with psoriasis. The condition was assessed during 4 weeks of treatment using the psoriasis area and severity index (PASI). In subjects who only bathed in Dead Sea water the improvement was 28%, in those who only sunbathed it was 73%, and in those who did both it was 83%. The main therapeutic factor was thus shown to be sun exposure, possibly enhanced by bathing in Dead Sea water. Similar degrees of improvement in spring, summer and autumn led to speculation about the optimum dose of solar radiation needed.

摘要:
The efficacy and cosmetic qualities of betamethasone valerate and mometasone furoate cream applied once a day were evaluated in 30 patients with symmetrical atopic dermatitis. While objective evaluation and patient opinion indicated that the two treatments were equally effective, betamethasone was significantly superior with respect to several cosmetic properties and was greatly preferred for its overall cosmetic quality. The possible benefit of the patients' preference for betamethasone for treatment compliance and treatment results is discussed.

BACKGROUNDCandidaonychomycosis with chronic paronychia is a dermatological disorder that is difficult to treat. Amorolfine nail lacquer, a new topical antifungal agent, has proven effective in the treatment of onchomycosis of short duration without matrix involvement.OBJECTIVE We evaluated the effectiveness of amorolfine nail lacquer in the treatment of long-termCandidaonychomycosis with chronic paronychia.METHODS A group of 32 patients with 57 onychomycotic fingernails with matrix involvement were treated with twice-weekly applications of amorolfine 5% nail lacquer which was also inserted in the paronychial space. The responsible fungi wereCandida albicans(25),C. tropicalis(7),C. parapsilosis(2), and no pathogens were isolated from nine patients who were treated on clinical grounds only. Five clinical features (hyperkeratosis, onychodystrophy, onycholysis, paronychia and tenderness) were scored. Photographs were taken each time the patients attended for the three required follow-up assessments. Patients were not requested to discontinue wet work or to wear gloves.RESULTS After an average 40 weeks of treatment (14–67 weeks), 90% of the nails were either cured or showed only minor signs of residual dystrophy. Tenderness and chronic paronychia responded particularly well and quickly to the treatment.CONCLUSION Amorolfine 5% nail lacquer is effective and is the treatment of choice in chronicCandidaonychomycosis with paronychia and nail matrix involvement.

This study demonstrated that twice daily application of 2% salicylic acid lotion impregnated into pads was significantly better than placebo in mild to moderate acne. Significant benefit from the active therapy was evident at 4, 8 and 12 weeks when total lesion counts were assessed. There was a significant benefit from the active therapy over placebo in noninflamed lesions at 8 and 12 weeks and inflamed lesions at 12 weeks. These results fit with the suggestion that salicylic acid has a primary affect on comedogenesis.

Chitin is a polymer of D-glucosamine, beta-poly-N-acetyl D-glucosamine. The chitin derivative, 70% deacetylated chitin (DAC-70), shows immunopotentiating activity. DAC-70 ointment 3.5% in a white petrolatum base was used to treat defined areas of four patients with senile exfoliative erythroderma following eczema. The effects of DAC-70 were compared with those of glyteer (0.2% soybean tar ointment) with 0.1% dexamethasone. In all patients erythroderma ceased spreading within 1 to 2 weeks in all treated areas. Thus, topically applied 3.5% DAC-70 may be of similar value to glyteer in the treatment of exfoliative erythroderma following eczema.

Chitin is a polymer of D-glucosamine, beta-poly-N-acetyl D-glucosamine. The chitin derivative, 70% deacetylated chitin (DAC-70), is immunopotentiating. The present study was conducted to determine the immunoregulatory events in skin following topical application of DAC-70 to DNCB-sensitized contact dermatitis in guinea pigs. On the 3rd day of treatment, DNCB-induced erythema was reduced in all five guinea pigs treated with 10% or 3.5% DAC-70 ointment. On the 7th day of treatment, ATPase-positive Langerhans cell (LC) density in epidermal sheets was 1021 + 131/mm2(mean + SD) for 10% DAC-70-treated skin, and 1000 + 130/mm2for 3.5% DAC-70-trèated skin, both values exceeding 782 + 65/mm2for normal epidermis. The value in DAC-70-treated samples was significantly greater (P<0.01) than in normal epidermis. DAC-70 thus appears to have an immunoregulatory effect by increasing the number of epidermal LCs.

We describe four patients with Crosti's lymphoma, a non-Hodgkin's B-cell lymphoma of follicular centre origin, treated with intralesional natural interferon alpha (IFN). In all patients the lesions cleared. Two patients had a clinical relapse but their lesions cleared after a second course of therapy. Intralesional injections are thought to have a direct action on tumoral cells as well as a systemic effect. Given the high rate of relapse after radiotherapy (75%), natural IFN is a promising therapeutic alternative.

Allergic contact dermatitis (ACD) can be persistent and difficult to treat. Indeed, in ACD from chromate, less than 20% of cases are clear after 10 years. Oral cyclosporin A has been used in severe ACD. There are few reports regarding the use of azathioprine. We report two cases of ACD from chromate and two from sesquiterpene lactone which responded well to azathioprine therapy. None of the cases had responded adequately to potent topical steroids or allergen avoidance. Our cases illustrate that azathioprine 0.2 mg/kg daily is a simple, cheap and effective treatment, both in the short and long term. Azathioprine is cheaper and has fewer side-effects than cyclosporin and so may be preferable as an initial systemic therapy. Azathioprine may be effective in other forms of allergic contact dermatitis.
