The efficacy of sulphasalazine in the treatment of psoriasis was assessed by a double-blind trial performed on 17 patients. Treatment with sulphasalazine for 4 weeks caused a significant improvement in total body surface area of psoriasis and amount of scale, but produced no significant reduction in erythema or thickness of the plaques. Acetylator status was performed on patients drawn from both the double-blind trial and a preliminary open trial to assess if therapeutic response might be related to this factor. There was no correlation between overall response of psoriasls to sulphasalazine and the acetylator status of the patient. It is concluded that sulphasalazine is a safe, cheap and effective systemic therapy for psoriasis. Although side-effects are relatively common, they occur early and are reversible on discontinuation of the drug.

The use of ultrasound-induced hyperthermia as adjuvant therapy was evaluated in seven patients with chronic palmoplantar psoriasis in a bilateral paired comparison study. Treatments consisted of twice-weekly 30-min applications of an ultrasound transducer operating at 8 MHz. The intensity of the device was varied between 0.30 and 0.54 W/cm2to achieve temperatures of 42–44C at a depth of 2–3 mm beneath the skin surface as monitored with a thermocouple probe. The mean clinical score for the treatment sites was 6.0 1.6 before treatment and 2.3 0.70 after hyperthermia treatment. The mean clinical score of the control site was 5.6 1.76 and remained unchanged during the treatment period. There were no sidesffects. At the 3 month follow-up three patients continued to show clinically less severe psoriasis on the hyperthermia treated area compared with the control area.

The effect of corticosteroid treatment in the prevention of postherpetic neuralgia was evaluated in a 5-year retrospective study on 28 patients who had been given steroid treatment and 20 patients who were given non-steroid therapies. Tbere was no statistically significant difference in the occurrence of postherpetic neuralgia between the two groups. Steroids did not shorten the duration of pain in the group that did not develop postherpetic neuralgia. Based on these results we do not recommend the use of corticosteroid treatment in the prevention of postherpetic neuralgia.

Each of 21 patients with histologically proven basal cell carcinomas received three injections of 1.5 106U recombinant interferona-2b per week for 3 weeks. Excision biopsies were performed 3 months after completion of treatment. Eight out of 19 biopsies revealed no evidence of residual tumour and 12 out of 20 patients experienced some side-effects considered mild by both patient and investigator in most cases.

The mechanism of minoxidil on hair growth is unknown. The casual level of scalp sebum was used as an assay for androgenic activity in the pilosebaceous unit. There was no change in casual scalp sebum levels of 24 males treated with 2 ml 2% minoxidil solution applied topically twice daily for 6 months regardless of a positive response of hair growth which occurred in six of the patients.

Dithranol was prepared in water-miscible O/W cream formulations at 1, 2 and 4% concentrations. These creams were stable after up to 15 months storage at room temperature. When used for short-contact (10 to 20 minutes) therapy in 29 psoriasis patients, the results with 4% cream were similar to the published results with 1% dithranol in Lassar's paste or in petrolatum. Irritation was minimal. Electron microscopic studies before and after treatment revealed no ultrastructural alterations, in particular no damage to the mitochondria.

A total of 17 patients with nummular psoriasis were treated with 0.1% betamethasone valerate stick on the right-hand-side of the body and with 0.1% betamethasone valerate ointment on the left twice a day for 4 weeks. The trial was performed open, and the patients were Bssessed at the beginning and at the end of the study. One patient dropped out because of lack of compliance. The study included an overall clinical and photographic assessment, assessment of the lesion area, scaling, colour and thickness, the amount of betamethasone valerate used during the trial, and a questionnaire on patients' opinions concerning the hygienic and cosmetic qualities of the stick. No significant difference was found in scaling and colour, but lesions studied were thinner when treated with the steroid stick. In most patients the overall clinical and photographic assessments showed that there was a better effect with the 0.1% betamethasone valerate stick than with 0.1% betamethosone valerate ointment. From a hygienic and cosmetic point of view, patients preferred the stick formulation to the ointment. The amount of betamethasone valerate used in the trial was the same on both halves of the body.

Gold salts given by injection were commonly used in the treatment of discoid lupus erythematosus (DLE) some years ago. The introduction of a new orally administered, gold preparation has renewed interest in gold treatment. Eight patients with longstanding disfiguring DLE, unresponsive to conventional therapy, were treated with auranofin 3 mg twice daily. Three patients improved and in one of these a complete resolution of the skin lesions occurred. Adverse reactions were limited to loose stools in two patients. The different therapies for discoid lupus erythematosus are briefly mentioned.

A case of sporotrichosis, which showed resistance to treatment with oral potassium iodide and local beat treatment but which responded to itraconazole, is reported. The patient presented an ulcerated, infiltrated, large plaque with scaly crusts on the left cheek and several reddish nodules on the neck. Two weeks after the oral administration of itraconazole 100 mg/day, the lesions were dramatically flattened, and 1 month later a negative fungus culture was obtained. No recurrence was observed at 3 months after stopping treatment. Itraconazole proved to be an adequate alternative- treatment in iodine-resistant sporotrichosis.
