Emollients are well known to benefit dry and inflamed skin. We wished to evaluate the therapeutic effects of various emollients on acute and chronic ultraviolet (UV) radiation photodamage. Albino hairless mice were treated with petrolatum, Vaseline petroleum jelly (VPJ), mineral oil, lanolin, cold cream and sun protection factor-2 (SPF-2) sunscreen. The UV source was a bank of Westinghouse FW tubes (peak emission at a wavelength of 313 nm). Chronic studies assessed protection against tumorigenesis and dermal damage. Protection against sunburn-cell formation was also assessed. Striking inhibition of tumorigenesis was found with VPJ (by about 95%) add lanolin (by about 54%) whereas mineral oil greatly enhanced tumorigenesis (by about 40%). VPJ was also protective against dermal damage and it reduced the number of sunburn cells. With an SPF of about 2, it is likely that some of the VPJ protection was due to sunscreening. However, the fact that VPJ applied post-UV-irradiation reduced the incidence of tumours by 21%, together with the suppression noted with the emollient lanolin which is not a sunscreen, suggests other mechanisms.

Soap substitutes are widely prescribed in the belief that they improve symptoms in a wide range of skin disease. There is currently a lack of clinical evidence to support this belief. We report a study performed on 38 subjects suffering from atopic dermatitis, psoriasis or senile xerosis, in whom the response to emulsifying ointment BP and Wash E45 was assessed. The investigator was blinded as to the treatment allocated to each subject. Visual analogue scales were used by subjects and the investigator to assess improvement in symptoms and signs. Subjects were asked whether or not the treatment had been beneficial, and overall benefit was also assessed by the investigator on a scale of 0 to 4. Subjects were asked to grade the efficacy of the medication as a cleanser using a scale of 1 to 5. No differences in response could be demonstrated between the two preparations, although Wash E45 was found to be more effective as a cleanser. All parameters of severity of the skin condition improved during the study with both soap substitutes.

A double-blind study was performed on 85 patients with alopecia areata in order to assess the capacity of topical cyclosporin A to produce hair regrowth. No difference was found between the cyclosporin and the placebo. The failure to find detectable blood levels of the drug suggests that its transcutaneous absorption is poor.

A randomized trial was conducted to determined whether hydrocolloid occlusion plus shaving was superior to shaving alone on healing and relief of symptoms of radiation-induced plantar hyperkeratosis. A total of 30 consecutive patients took part in the study. During 12 weeks, six shavings were performed every 14 days in both groups. The patients were randomized to receive shaving alone or shaving with DuoDERM occlusion in addition. Outcome measures were the size of the hyperkeratosis, a patient-performed discomfort score, and a patient-performed overall judgement of the trial. The results demonstrated benefit of occlusion on the reduction in the size of the hyperkeratosis (P= 0.02) and decreased discomfort (P= 0.005). Complete remission of the hyperkeratosis was shown in 86% and 47% of the patients in the occlusion group and conventional group, respectively (P= 0.07). In the occlusion group, 71% of the patients were without symptoms compared with 33% in the conventional group (P= 0.09). Overall trial judgement slightly favoured occlusion (P= 0.09). The results of the present study suggest that hydrocolloid occlusion plus shaving might be a useful treatment for radiation-induced plantar hyperkeratosis.

Scalp psoriasis is frequently recalcitrant to topical therapy. In this study the efficacy and safety of a super-potent (augmented betamethasone dipropionate 0.05% lotion) and a potent (fluocinonide 0.05% solution) topical corticosteroid were evaluated in the treatment of severe scalp psoriasis. A total of 169 subjects with severe psoriatic scaling of the scalp accompanied by induration, erythema, and pruritus/burning were enrolled in this randomized double-blind parallel-group trial of 3 weeks duration. Subjects applied the study medication twice daily and were evaluated five times during the study. Evaluations of individual disease parameters, the global response and the percentage improvement showed that both medications were effective in the treatment of severe scalp psoriasis. There was a significantly greater proportion of patients (P<0.01) completely cleared of scaling at the end of therapy in the augmented betamethasone dipropionate group. There was a low incidence of treatment-related sidesffects in both groups, with a greater number occurring in the fluocinonide group.

The efficacy of two oral antifungal drugs, itraconazole and griseofulvin, in tinea corporis and tinea cruris was compared in a doubleblind trial. Patients were treated with itraconazole (n = 15, 100mg/day) or griseofulvin (n = 14, 500mg/day) for 2 weeks. The clinical outcome was in favour of itraconazole. After 2 weeks 60% of the patients treated with itraconazole were cured, and 21% of those treated with griseofulvin. Equivalent figures 2 weeks after the end of the treatment, were 67% and 29%, respectively. Both treatments were well tolerated.

Perifolliculitis capitis abscedens et suffodiens is a rare condition characterized by recurrent interconnected abscesses and patchy alopecia of the scalp. Numerous treatments have been used in this condition with varying degrees of success. We report a case successfully treated with isotretinoin.

Two patients with major aphthosis and three with minor aphthosis which proved resistant to corticosteroids were treated with oral potassium iodide at doses ranging between 600 and 900 mg/day. Healing began after 3 days and was complete in a week. Tolerance was excellent despite the disagreeable taste of the drug.

We report the case of a 78-year-old woman who presented with adult linear IgA disease (LAD). Control of her disease was difficult due to intolerance of dapsone and the poor response to other sulphone drugs. Immunosuppressive therapy with azathioprine and corticosteroids was ineffective, whereas the combination of methotrexate and low-dose prednisolone led to remission. Methotrexate was discontinued after 18 months due to deteriorating liver function, and the subsequent relapse was controlled with cyclosporin. To our knowledge the use of methotrexate or cyclosporin have not previously been described in LAD but our experience suggests that they are more effective agents than azathioprine.
