Of 165 patients with dermatitis herpetiformis OH) assessed for autoimmune status, autoantibodies were present in 101 (61.2%) and autoimmune disease in 20 (12%) on prekntation; a further nine patients (5.5%) had developed autoimmune disease at follow up (mean 8.1 years). Autoimmune status did not correlate with severity of small-bowel enteropathy as assessed by microscopy or by intraepithelial lymphocyte count. There was also no correlation with age, sex, duration of rash before presentation or age at onset of rash. Of 162 patients followed up (mean 8.1 yrs), 156 were on definitive treatment. In all treatment groups there was an overall trend to development of autoimmune disease/autoantibodies in a minority of patients. Autoantibodies/ autoimmune disease developed in subjects initially autoantibody-negative in 5/26 (19%) on a strict gluten-free diet, 5/16 (31%) on drug therapy plus gluten avoidance and 2/19 (11%) on drug therapy alone. Treatment with a gluten-free diet did not result in resolution of autoimmune disease or disappearance of organ-specific antibodies in DH. These results indicate that development of autoimmune disease is independent of both gluten intake and severity of bowel disease, but is more likely to be due to an underlying immunological mechanism which is probably genetically determined.

A double-blind trial of 0.75% metronidazole gel versus oral oxytetracycline 250 mg twice daily in the treatment of rosacea was undertaken. Over a 9-week study period, both treatments were shown to be highly effective as shown by a reduction in numbers of papules and pustules, severity of erythema and overall subjective assessment by both observer and patient. Topical metronidazole gel may be regarded as an effective alternative to oral oxytetracycline in the treatment of rosacea.

In a multicentre double-blind randomized parallel-group study, 48 patients with mycologically proven tinea pedis were treated with either oncedaily applications of terbinafine 1% cream or placebo for 2 weeks. The mycological cure rate at follow-up 4 weeks after completing treatment, was 81% for terbinafine compared with 45% for placebo (P= 0.01, Fisher's exact test, one tailed). Only one adverse event, an erythematous rash in a terbinafine-treated patient, was thought to be related to treatment. Topical terbinafine 1% cream used once daily for 2 weeks was shown to be an effective and well-tolerated treatment for tinea pedis.

We studied the efficacy and tolerance of topical 0.5% podophyllotoxin in the treatment of molluscum contagiosum. In this open study, 24 patients applied the drug on a daily basis with temporary withdrawals of treatment when irritation occurred. All lesions cleared after less than 15 applications of podophyllotoxin over a period of less than 3 weeks. No systemic adverse effects occurred. This method of treatment of molluscum contagiosum was reliable and safe.

Over a 21-month period 76 patients with 94 chronic leg ulcers were recruited from community- or hospital-based practice and randomized to Comfeel Ulcer Dressing (Coloplast Ltd) or Jelonet (Smith and Nephew). In the Comfeel group, 14 of the 46 ulcers (30.4%) healed, compared with three of the 48 Jelonet-treated ulcers (6.3%) in the 12-week assessment group. Comfeeldressed ulcers also showed a significantly better response. Patients expressed favourable views on the cosmetic and waterproof presentation of Comfeel Ulcer Dressings. We feel that the carboxymethylcellulose, hydrocolloid Comfeel Ulcer Dressing offers an advantage in treating chronic leg ulcers in both community- and hospital-based patients.

A total of 14 patients (ten men and four women) ranging from 14 to 53 years of age entered a 4 week study. They were instructed to take 27 g fish oil containing 4.3 g eicosapentaenoic acid daily and received selective ultraviolet phototherapy five times weekly. One patient had severe geographic psoriasis and 13 had plaque-type psoriasis, All patients used only emollients as local therapy. Clinical evaluations were performed, and plasma samples obtained, before, during and after the study. The plasma lipids were extracted and the major lipid classes isolated by thin-layer chromatography. The fatty acid composition was analysed by gas chromatography. Levels of eicosapentaenoic acid and other n-3 fatty acids were found to be increased in all patients. This elevation vaned between different patients and lipid classes investigated. After treatment, three patients were clinically clear, three had marked improvement (80–90% clearing), and five showed minimal improvement. Three were unchanged. Further studies are needed to evaluate the presumed synergistic effect of fish oil and selective ultraviolet phototherapy.

Significant advances in the treatment of vascular naevi have occurred with developments in dermatological lasers. This review discusses these developments and includes basic laser physics, new laser technology, clinical studies and improvement in technique. It also summarises the most appropriate laser use for various patients.

Numerous treatments are available for psoriasis, each of which has limitations due either to ineffectiveness or to adverse side-effects. This article examines the use of 5-lipoxygenase inhibitors in the treatment of psoriasis. Experimental evidence has identified perturbations in arachidonic acid metabolism as a possible cause of psoriasis. This evidence is briefly reviewed. Also reviewed are several clinical trials that have investigated the effectiveness of 5-lipoxygenase inhibitors in the treatment of psoriasis. These studies indicate that, while Slipoxygenase inhibitors appear to show some effectiveness, with one study showing similar efficacy to fluocinolone, the use of Slipoxygenase inhibitors at present appears limited by its side-effect profile. Suggestions for future studies using 5-lipoxygenase inhibitors are made.
