A group of 18 adult patients, with chronic, severe and therapy-resistant atopic dermatitis, were treated with 3 mg/kg per day cyclosporine for 1–8 months (mean 5.3 months). All patients were treated in the period from September-October to April-May. During the summer months only exposure to sunlight was recommended. In 15 patients improvement in clinical signs and pruritus, and a beneficial impact on the quality of life were observed. Side-effects occurred in 6 patients, but they did not lead to suspension of therapy. No changes in blood pressure were observed. No renal or hepatic side-effects were recorded. During follow-up (ranging from 17 to 42 months, with a mean of 23.8 months), 11 patients were in good health and the dermatitis was controlled with topical nonsteroidal products and exposure to sunlight. Four patients developed a relapse for which therapy with cyclosporine was again started. Cyclosporine can be successfully used in the treatment of adult patients with chronic, severe and therapy-resistant atopic dermatitis and it appears to be effective even at a dose of 3 mg/kg per day. Low-dose long-term treatment can lead in some patients to complete remission of the disease. The proposed therapeutic regimen (treatment from September-October to April-May and only exposure to sunlight during the summer months) is innovative and is particularly suited to the Mediterranean and similar climates.

INTRODUCTION Numerous trials using interferon in the treatment of HPV infections have been reported. IFN-βhas antiviral, antiproliferative and immunopotentiating activities. The aim of this randomized open study was to evaluate the effectiveness of intramuscular IFN-βin reducing relapses versus both electrocautery and combined treatment, i.e. IFN-βplus electrocautery, in a group of male patients affected by genital warts.PATIENTS AND METHODS A total of 84 male patients (mean age 32 years) with recent genital warts were randomly enrolled into three groups: 33 patients were treated with electrocautery; 25 patients received only intramuscular IFN-β(3 000 000 IU daily for 10 days); 26 patients were treated with both electrocautery and intramuscular IFN-β(3 000 000 IU daily for 10 days). The patients were checked monthly. Residual disease was treated with electrocautery until complete clearance.RESULTS After 1 month 42% of the patients treated with electrocautery, 29% of those treated with IFN-βand 39% of those treated with combined therapy had been cured. Residual disease rates gradually decreased in all groups during the follow-up, but more quickly in the patients who had received IFN-βtherapy. No side-effects after IFN-βtreatment were observed.CONCLUSION IFN-βmonotherapy is not the first choice treatment for genital warts, though it may be useful in patients with relapsing condylomata acuminata resistant to other therapy.

We report the clinical results of 8-MOP bath PUVA in 42 psoriasis patients. The PASI was used to assess disease severity and improvement after topical PUVA therapy. PASI reduction was 72.51% for the whole treated group, with a greater response in those patients with palmoplantar psoriasis and guttate psoriasis. We also observed a better response in patients with skin phototypes II and III. Treatment was well tolerated although some adverse effects were observed.

The neuropeptide substance P has been implicated in the pathophysiology of pruritus. Capsaicin, the pungent agent in red pepper, is known as a potent substance P depletor. Therefore, the influence of capsaicin on pruritus was studied in ten patients suffering from atopic dermatitis. The patients applied 0.05% capsaicin cream four times daily to one arm for 3 weeks in an open left versus right comparison. Every week skin lesions were evaluated by a physician, and pruritus was assessed by the patients. Four out of ten patients reported a marked relief of pruritus in the capsaicin-treated area only. A relief of pruritus by both capsaicin and vehicle was reported by two patients. Capsaicin was of no benefit in two patients. All patients reported a transient burning sensation after application of capsaicin, which gave reason to discontinue capsaicin treatment in two cases. We conclude that for some patients with atopic dermatitis, capsaicin can be helpful in reducing pruritus.

During July and August 1994, 45 psoriatic volunteers were treated at the Dead Sea for 28 days, using one of three sun-exposure schedules. The mean percentage improvement in the Psoriasis Area and Severity Index (PASI) was as follows: group 1–3.0 h maximum daily sun exposure: 91%; group 2–4.5 h maximum daily sun exposure: 87%; group 3–6.0 h maximum daily sun exposure: 88%. One year before, 16 of the patients from groups 1 and 2 had had Dead Sea treatment with a daily sun-exposure time of 5.5 h; there was no significant difference in the degree of improvement between the 2 years. It was concluded that for treatment in July and August the daily sun exposure need not exceed 3 h.

INTRODUCTION Topical tretinoin (Retin-A), although currently recommended for the treatment of acne, has also been reported to be of benefit in a wide range of skin diseases including plane warts, ichthyosis, lichen planus and photodamaged skin. The objective of this study was to evaluate the efficacy and safety of topical tretinoin (0.05% cream) in the treatment of plane warts in Arab children.METHODS A group of 25 children with plane warts treated with 0.05% topical tretinoin cream (applied once daily for 6 weeks) were compared with a control group of 25 untreated children. Patients were reviewed weekly and a final assessment was done at 12 weeks.RESULTS After 12 weeks, clearance of warts was observed in 84.6% of the treated group as compared with 32% of the control group. The difference was statistically significant (P<0.01). The drug was well tolerated. Even though 42.3% of the treated group had some redness and peeling, only one patient discontinued treatment.CONCLUSION Topical 0.05% tretinoin cream appears to be an effective and safe treatment for children with plane warts. This was a pilot study that needs confirmation by a large double-blind, placebo-controlled trial.

Five older women with severely photodamaged facial skin were treated for 16 weeks with 0.1% tretinoin cream and 0.05% diflorasone diacetate cream. Striking clinical improvement occurred in all subjects; notably, there was a reduction in wrinkling and bleaching of dyspigmentation. Histologically, there was complete correction of epidermal atrophy and arypia. Basal keratinocytes, originally crowded with hyperpigmented melanosomes, lost most of the pigment. Dermal cellularity increased and there was also a modest increase in the microvasculature. The dermal matrix (collagen, elastin, and glycosaminoglycans) was not changed. Irritant effects were minor. Atrophy did not occur. Evidently tretinoin counteracted the atrophic potential of the potent corticosteroid but did not interfere with its anti-inflammatory effect.

A total of 144 patients with at least one viral hand wart were treated with liquid nitrogen cryotherapy in an open, multicentre, prospective study. The aim of the study was to determine the optimum interval between cryotherapy treatments. An effort was also made to determine whether the cure rate was related to the age of the patient, the duration of the disease and/or the total number of warts. The patients were grouped according to age, duration of disease and number of warts, and an index finger wart not previously treated with cryotherapy was the basis for randomization of the patients for treatment at 2-, 3-or 4-week intervals until cure was achieved or a maximum of six treatments had been given. Cure rates were evaluated 3 and 6 months after the initiation of treatment. The cure rates of the index wart were 71% and 65% evaluated after 3 and 6 months, respectively. After 6 months, cure rates of 63%, 70% and 63% were seen for index warts treated with cryotherapy at 2-, 3-and 4-week intervals, respectively. If the index wart was not cured after four or five freezings, further cryotherapy was unlikely to be successful. For the wart disease of the patients in general, the cure rates were 22% and 29% after 3 and 6 months. The best cure rates were found among young patients with few warts of short duration.

Three cases of lymphocutaneous sporotrichosis were successfully treated with oral terbinafine at a dose of 125 mg once daily rather than 250 mg twice daily as reported previously. The clinical response was rapid in two patients. The duration of treatment was 8, 10 and 29 weeks, respectively, for the three patients treated. No major side-effects were noted. There was no relapse on subsequent follow-up. Terbinafine at a dose of 125 mg once daily thus proved to be effective and safe in the treatment of lymphocutaneous sporotrichosis.
