Uraemic pruritus is an important cause of morbidity in patients on chronic haemodialysis. Topical agents such as menthol and phenol may provide considerable relief in pruritus associated with other disorders. Sarna lotion (Stiefel Laboratories) is a preparation containing 0.5% each of camphor, menthol and phenol. A double-blind, randomized, crossover trial was conducted to compare the efficacies of Sarna lotion and Eurax lotion, which contains 10% crotamiton, in the symptomatic relief of uraemic pruritus. Pruritus was assessed using a four-point verbal rating scale and a visual analogue scale. Assessments were made before treatment, after 4 hours, and after 7 days of treatment with each lotion, with a washouot period of 7 days between treatments. A total of 31 haemodialysis patients with uraemic pruritus were studied. One patient withdrew from the trial after an adverse reaction to Eurax. With both Sarna and Eurax, there was significant improvement in the degree of pruritus after 4 hours, and after 7 days of treatment. For both lotions, after 7 days of treatment, 76.7% of patients reported some improvement in their degree of pruritus. Of these, about 50% felt that their itch was‘much improved’or‘very much improved’. There was no significant difference in efficacy between the two lotions after 4 hours and after 7 days of treatment.

An open comparative study was conducted with EMLA cream versus infiltration with prilocaine to determine if EMLA cream was an adequate topical local anaesthetic for the taking of skin biopsies by excision or punch. Of the 51 patients, 34 were allocated to the EMLA group and 17 to the infiltration group. Eight patients had a punch biopsy and 43 had an excision biopsy, 27 in the EMLA group and 16 in the infiltration group. There were no statistically significant differences between visual analogue scale (VAS) pain scores of all patients who received EMLA and those of all patients who received infiltration analgesia. For the EMLA group the mean VAS score was 25 mm on a 100 mm scale. Seven patients in the EMLA group required further infiltration analgesia. The infiltration group had a mean VAS score of 19.1 mm. This study shows that EMLA cream, at the dose and with the application time used, provides effective local analgesia for skin biopsies taken by punch or scalpel.

A few open studies on small groups of patients have reported the efficacy of oral vitamin D3in the treatment of psoriasis. To further characterize the therapeutic potential of this agent a placebo-controlled double-blind study was undertaken on a larger group of patients. Vitamin D3(the active 1-α-hydroxy form) 1μg/day and a placebo in a similar presentation were given in coded vials to 50 patients. Slight improvement was observed in 9 (45%) patients receiving vitamin D3and 8 (38%) patients receiving placebo showed similar improvement, indicating that this regimen is not effective in the treatment of moderate to severe psoriasis.

Patients who had been treated with at least one 16-week course of isotretinoin (1 mg/kg) for acne vulgaris were interviewed and examined in order to establish whether there was an increased incidence of eye problems in those who also used contact lenses. Of the total of 157 patients, 11 (7%) wore contact lenses. Significantly more of the patients (63%, 7/11) using contact lenses developed dry eyes, than those not wearing lenses (28%, 41/146). There was no increased risk of infection in the contact lens wearers. Most of the patients (82%, 9/11) who wore contact lenses were able to continue as long as they reduced the duration of use and applied lubricating eye drops. Soft lenses produced fewer problems. We recommend the institution of lubricating eye drops as a matter of course when prescribing isotretinoin to subjects who wear contact lenses, and we emphasize the need for elementary hygiene when handling lenses.

A detailed method of patient skin self examination for the dysplastic naevus syndrome using full-scale colour total body photographs is described. The patient and the physician share the same set of photographs to monitor the syndrome. Any new lesions or changing lesions are reported by the patient to the physician for further evaluation. This method has an advantage over previous recommendations because patient use of the photographs provides an objective visual baseline for comparison. Overall costs of caring for patients with the dysplastic naevus syndrome through this method are likely to decrease since biopsies can be limited to new lesions or lesions demonstrating a changing pattern.

A double-blind placebo-controlled trial of oral paraaminobenzoate (Potaba) was carried out with 25 patients suffering from lichen sclerosus et atrophicus. Of the 21 patients who completed the two month trial, six showed some improvement on Potaba against seven on the placebo, an insignificant difference.

An open trial of liothyronine 20μg twice daily was carried out in 26 women, selected from our general medical and endocrine clinics, with severe or moderately severe acne. They were on no other treatment for acne during the trial. Endocrine tests showed mild hyperprolactinaemia which was reduced, but not significantly, during treatment. Response scores were assessed at intervals of 3 months for a total period of 1 year. Little change occurred in the first two months, but the score was significant at 3 months (t-test p<0.001) and progressed most between 6 and 9 months. The explanation for the response is uncertain and requires further work.

Forty patients were enrolled in a double-blind comparison of a topical preparation of the non-steroidal antiinflammatory agent biphenylacetic acid (5%) and placebo in mild to moderate facial acne vulgaris. There was no significant improvement in acne grade and counts of inflamed and non-inflamed lesions in either treatment group indicating that topical 5% biphenylacetic acid cream is of no value in the treatment of acne vulgaris.

Numerous drugs have been reported to cause oral lichenoid reactions. Whilst gold is known to cause a cutaneous lichenoid eruption, oral lesions are rare. We report a case in which mucosal lesions were the presenting feature, and discuss the possible aetiology.
