Monitoring of plasma proteinases, proteinase inhibitors and other selective plasma proteins was evaluated in patients undergoing Y-graft aortofemoral bypass operation. Fast-reacting acute-phase proteins (C-reactive protein, antichymotrypsin, α1-acid glycoprotein) and slow-reacting proteins (haptoglobin, α1-antitrypsin) increased significantly 48–120 h after operation. By contrast, no significant increase was found between plasma ceruloplasmin levels before clamping and after declamping. Activity and concentration of α2-macroglobulin decreased postoperatively and remained significantly lowered throughout the observation period. Plasma levels of granulocyte elastase were elevated significantly 1 h after declamping, whereas trypsin-binding capacity decreased immediately after the release of the clamp. Aprotinin pretreatment caused higher trypsin-binding capacity of the plasma, significantly lower ‘unspecific’ proteolytic (azocasein-hydrolyzing) activity and significantly lower non-TCA precipitable low molecular weight plasma protein concentration. The effects of ketone bodies and glucose on nitrogen balance and liver protein synthesis were studied in rat after experimental trauma. Energy was delivered intravenously as either monoglyceryl acetoacetate (MA; 87.5% of total energy input) or glucose (G). The isocaloric infusions (132 kcal/kg/day) were started after recovery from anaesthesia and continued for 2 days. The liver protein synthesis was then measured in vitro by perfusion with 14C-leucine. The infusion of MA resulted in a more negative cumulative nitrogen balance (MA: -2.31 ± 0.26 g N/kg, G: -1.32 ± 0.43 g N/kg/48 h; mean ± SD) and liver protein synthesis (MA: 43.4 ± 17.2, G: 71.1 ± 15.2; arbitrary units, mean ± SD). A technique for auxiliary heterotopic transplantation of 60% of the liver has been developed in the pig to study acute and chronic rejection. Transplantations were performed in 13 non-tissue-typed donor-recipient combinations without immunosuppressive medication. Three pigs died in the 1st postoperative week from technical problems. In the remaining 10 animals acute rejection of the graft was not found, but signs of chronic rejection developed in 6 animals. It is concluded that auxiliary partial liver transplantation is technically feasible in the pig. Although the auxiliary liver graft is subject to immune attack, long-term graft survival without immunosuppressive medication can be achieved.

Intralipid was given to adult rats for 4 days either by intragastric or intravenous infusion. The villus height, protein content and the levels of sucrase, lactase and aminopeptidase activities were determined in the jejunum. The results were compared to values found in control rats fed orally with an equilibrated isocaloric diet. Intragastric or intravenous infusion of Intralipid caused (a) a 50% reduction in the protein content of the brush border membrane, (b) a significant drop of the specific (expressed per milligram protein) and segmental (expressed per centimeter intestinal length) activities of the disaccharidases, (c) the maintenance of aminopeptidase activity and (d) the lengthening of the villi in the jejunum. The major loss in disaccharidase activities was obtained when fat was given intravenously. Arguments favoring a hormone-mediated effect of the infused lipids on the intestinal function are presented and discussed.

A quantitative spectrophotometric assay for endotoxin, utilizing Limulus amebocyte lysate and a chromogenic peptide substrate, was standardized for the application on lymph and plasma from the rat. The influence of inhibitors and unspecific amidolytic activity, the optimal incubation times, the adhesion of endotoxin to platelets and glass surface, the variation in endotoxin standard as well as the effects of cold storage on endotoxin activity were settled. In vitro added as well as endogenously derived endotoxin was studied. The method was found to be reliable when in practical use on these media.

The effect of an enzymatic preparation for wound cleaning (Varidase®) on the mechanical properties of absorbable sutures (Dexon®) was studied in vitro and in vivo in a rat model. In vitro the sutures demonstrated a significant decrease in strenght (breaking strength and energy absorption) and extensibility after 12 days of incubation in saline. Incubation in Varidase, however, further decreased the mechanical properties significantly. The stiffness of the sutures was independent of treatment and time. In vivo changes of mechanical properties of the sutures resemble those of the in vitro study, except for a decrease in stiffness of the sutures. The sutures in a primary closed wound had the same strength (energy absorption) as the sutures of an open wound treated by saline, while the sutures of an open wound treated by Varidase tended to have a decreased strength (p = 0.08). This study supports the hypothesis that an enzymatic process may be involved in the degradation of Dexon. We have developed a personal procedure for the radioimmunoassay of gastrin (lower detection limit 3 pg/ml). Using 5-fold concentrated lyophilized salivary samples, we are able to detect the basal content of gastrin immunoreactivity in saliva of normal fasting people (mean ± SD: 1.38 ± 0.61 pg/ml) compared to plasma gastrin (mean ± SD: 28.88 ± 11.00 pg/ml). We found that after meal stimulus, the value of salivary as well as plasma gastrin concentration increased (the correlation coefficient r among salivary and plasma gastrin being 0.899; p < 0.001).
