Abnormality of absolute values of median sensory conduction velocities and differences in conduction velocity between median and ulnar nerves are used as criteria for abnormality in the diagnosis of carpal tunnel syndrome. Detailed studies of sensory conduction in various median and ulnar branches and segments indicate that conduction velocities measured and calculated by standard methods are different in the different nerves, segments and branches. In particular, the velocity in the middle digit branch of the median nerve is slower than that in the index, and that in the 3–4 palmar branch is slower than that in the 2–3 palmar branch. Median and ulnar sensory conduction velocities are not well correlated in the same hand in the same individual, so comparison of these nerves for diagnostic purposes is subject to some risk. Palmar and digital conduction velocity in the same nerve may not be identical and the median nerve may have asymptomatic palmar slowing of no clinical significance. The conclusion that abnormality is present must take these factors into account.

It has been hypothesized that myotonic dystrophy is caused by a generalized disorder of membrane aminergic and petidergic receptors. A single uncontrolled study has suggested that the affective symptomatology and muscular disability of this disorder might be improved by tricyclic antidepressant treatment. In the present study, 12 adult patients with myotonic dystrophy (age range 18–55 yr) were treated with imipramine and placebo in a double-blind crossover paradigm. Both grip and percussion myotonia were significantly improved by imipramine treatment independent of change in depressive symptomatology. Serum drug levels revealed nonlinear elimination kinetics for imipramine in this patient population.

Lithium carbonate (LiCO3) was used to treat 10 brain-injured patients with severe, unremitting, aggressive, combative, or self destructive behavior or severe affective instability. Five patients had a dramatic response that resulted in significant improvement in their participation in a rehabilitative program. One other patient had a moderate response. A seventh patient improved dramatically, but regressed after 7 wk. Three other patients had neurotoxic side effects that precluded continued use of the medication. Two of them were simultaneously taking neuroleptic agents. These case reports provide further evidence that LiCO3can be a useful medication in the treatment of aggressive behavior and affective instability after brain injury, but that it has significant potential for neurotoxicity in this population, particularly when used in conjunction with neuroleptic agents.

One hundred women responded to a questionnaire dealing with night backache during pregnancy. All the women were in the second half of their pregnancy. Sixty seven per cent of the women reported discomfort and/or backache during the night. We hypothesize that hypervolemia combined with obstruction of the inferior vena cava, caused by the enlarging uterus, is the underlying pathomechanism leading to night backache. Patients with inadequate venous collateral circulation may develop excessive pressure within the venous system, the vertebral bodies and overdistension of venous channels distal to the occluded zone. These changes may lead to hypoxemia, metabolic disturbances, irritation of unmyelinated nerves and result in night backache.

A longitudinal study of the ambulation progress was done on 102 patients in the first 9 wk after an operation for repair of hip fractures. The time taken to achieve a stable walking pattern from frame to a cane was recorded for every patient. Results showed that 82.2% of the patients were able to walk with a cane at 4 wk postoperatively. Of the remaining 17.8% of the patients, 7.7% were able to walk with a cane at 6 wk whereas 10.1% were not able to walk with a cane at 6 wk. Several factors including the age, sex, mental state, the type of operation and the preoperative ambulation level of the patients were studied to predict the ambulation progress of the patients. The most important factors affecting the ambulation progress were the age and the preoperative ambulation level of the patient. Because of the general trend in the aging of the population, an increase in the number of hip fractures is expected. The result of this study may help in predicting the prognosis and provide guidance for restoring the function of the patients.

Thirty white male New Zealand rabbits underwent forelimb amputation below the elbow under sterile operating conditions. Fifteen of the amputation stumps were dressed in a soft, bulky dressing while the remaining 15 rabbits had their stumps immobilized in a rigid plaster dressing. On each of the postoperative days 3, 6, 9, 12 and 15, three rabbits from each group were killed and the amputated limb disarticulated and fixed in formalin. Then, histological examination of the suture line and subcutaneous tissue was carried out by a pathologist to determine the effects, if any, that the type of immobilization has on wound healing. Based on microscopic evaluation of wound approximation, interstitial edema and presence of granulation tissue, there was no significant difference between the stumps immobilized in a plaster dressing and those dressed with a soft, bulky dressing.

The process of data entry and initial analysis to locate data errors is described. Basic terms are defined and a simple method of entering data by using word processing software is illustrated. Data checking is done by using visual check of the raw data. Statistical programs are then used to locate possible data errors by finding data points (outliers) that are very different from the average. Special graphic output of statistical programs, scatterplots and box and whisker plots can be used to further locate questionable data. Examples of data entry forms and annotated step by step data cleaning with the use of inexpensive programs for personal computers are presented.
