摘要:
A case is presented of a patient with the chief complaint of low back pain, who subsequently was found to have lumbar metastatic angiosarcoma. This report emphasizes the need for a thorough medical evaluation in all instances of radicular low back pain. The characteristics of metastatic intramedullary spinal cord tumor are outlined.

Of the 3.3 of every 1000 persons in the United States who use a wheelchair, an estimated 3.3% per year have a serious wheelchair-related accident. Yet, only isolated case reports of fatal accidents have appeared. To obtain a better estimate of the incidence and nature of fatal accidents, a search was carried out of the death certificate database (1973-1987) of the National Information Clearinghouse of the Consumer Product Safety Commission; 770 wheelchair-related deaths were identified. The majority, 596 persons (77.4%), experienced a fall from their chairs or tipped over. Of 85 deaths (11%) caused by environmental factors, stairs were implicated in 51 (60.0%). Of 48 fatal burns (6.2%), 27 (57.3%) were related to smoking. Asphyxia owing to restraints occurred in 44 persons (5.7%) of all ages and caused 10 of the 17 deaths (58.8%) among persons 1-20 yr old. Wheelchair- related accidental death is uncommon (about 0.2% of serious accidents per year), but some types of accidents appear to be preventable.

Young rats subjected to bilateral brain lesions were subsequently tested for acquisition of eight puzzle-box problems. Lesions to 12 of the 37 brain sites investigated (anterior pretectal area, subthalamus, posterolateral hypothalamus, frontocingulate cortex, anterior thalamus, mediodorsal thalamus, ventromedial thalamus, parafascicular nucleus, mamillary bodies, cerebellum, olfactory bulb, and ventromedial hypothalamus) retarded puzzle-box learning. This study, combined with earlier investigation of 11 additional brain sites, show that 21 of the 48 total brain structures are involved in motor skill learning. These findings suggest that motor skill learning requires the integrity of a nonspecific mechanism and several specific mechanisms, such as vestibular-proprioceptive-kinesthetic discrimination mechanism and probably also response flexibility, visuospatial attentional, visual discrimination and place learning mechanisms.

It has been suggested that the nerve fibers to the first and second lumbrical muscles are relatively spared in patients with carpal tunnel syndrome compared with the fibers to the abductor pollicis brevis. Latencies and amplitudes of the first lumbrical and the abductor pollicis brevis (APB) muscles were compared with both wrist stimulation and midpalmar stimulation. The lumbrical latency at 12 cm and the APB at 8 cm were found to be similar at <4.3 ms. Amplitudes to the lumbrical were 0.6-8 mV compared with 4-13.6 mV for the APB. Three patients who had an abnormal sensory study, yet normal motor values to the APB, were found to have abnormal lumbrical latencies. This is an additional technique that could be useful for individuals suspected of carpal tunnel syndrome who have normal latencies and amplitudes to the APB.

Ankle or foot pain may be due to various clinical conditions. Injury or entrapment of the deep peroneal sensory nerve is part of the differential diagnosis; however, no technique studying the deep peroneal sensory conduction velocity has been described. We describe a technique that is simple and reproducible with averaging. We electrophysiologically studied the deep peroneal sensory nerve in 40 neurologically healthy adult subjects. The latency to onset of the sensory action potential was 2.9 ± 0.4 ms (range=2.1-3.6 ms). The latency to the peak of the sensory action potential was 3.6 ± 0.4 ms (range=2.7-4.2 ms). The conduction velocity was 42 ± 5 m/s (range=33-53 m/s). The amplitude was 3.4 ± 1.2 µV (range=1.6-6.6 µV). We conclude that the deep peroneal sensory nerve is readily accessible for electrophysiologic evaluation. It may be of value in diagnosing deep peroneal nerve pathology.

One of the causes for shoulder pain associated with hemiplegia is thought to be vigorous range of motion to the involved upper extremity. The objective of this study was to analyze the occurrence of pain in patients treated with one of the three exercise programs commonly used in the rehabilitation of hemiplegia: 1) range of motion by the therapist, 2) skate board and 3) overhead pulley. Of the 48 hemiplegic patients evaluated, 28 were assigned to one of the three exercise groups. Comparing the number of patients who developed pain in each group, there was a significant difference, with 8% of the patients in the range of motion by the therapist group, 12% of the patients in the skate board group and 62% of the patients in the overhead pulley group developing pain (x2=8.44) (P=0.014). The three groups did not differ in the side of involvement (P=0.57), extent of hemiplegia (P=0.25) or presence of subluxation (P=0.84). Use of overhead pulley has the highest risk of developing shoulder pain and should be avoided during rehabilitation of stroke patients.

摘要:
The primary statistical analysis is approached from the standpoint of what is required to publish in medical research journals. Descriptive and bivariate statistics cover the majority of medical research articles now published. Statistical guidelines for review of manuscripts are used to develop guidelines for analysis, including specifying the objective, the source of subjects and response rate, differences detectable with the expected sample size, appropriateness of statistics for one and two variables, method of presentation of results and conclusions and calculation of confidence intervals. Common mistakes to avoid include use of standard error of the mean instead of standard deviation, use of standard deviation with skewed data, failure to describe the statistical test used, multiple comparisons and failure to use special forms of t test and x2.
