Concepts of the pathogenesis of petit mal epilepsy, a childhood paroxysmal disorder characterized by the absence seizure and synchronous 3 Hz spike‐wave complexes in the EEG, have undergone considerable revision. The original idea of a “centrencephalic” disorder of brainstem origin is not supported by experimental evidence in animals or man, and has been replaced by the corticoreticular theory. The disorder is probably due to symmetric discharging foci in the cerebral cortex of the frontal lobes. The subcortical reticular formation participates by producing a desynchronizing drive which inhibits cortical paroxysmal activity. Incomplete myelination of the corpus callosum is the probable maturational factor which delays the clinical expression of petit mal epilepsy until after infancy. A study has been made of seven obese children and their mothers in discussion groups to explore their attitudes to obesity and its treatment.It'was apparent that members of the groups had very different problems associated with their obesity, and that treatment aimed at weight reduction alone would be inappropriate. The group discussions appeared useful in allowing children and their parents to express their feelings and problems, and gain insight into them. They also provided diagnostic insight into individual children and their families.

A pilot study of the families of 19 children with acute lymphoblastic leukaemia in remission for 12‐23 months is reported. Reactions reported by the families were most marked in parents. Tentative findings suggest that maternal depression and anxiety symptoms may provide an index of family strain. Fathers' job status markedly influenced the degree and timing of additional social and financial stress. Reactions in siblings tended to be minimised, denied or ignored. Problems tended to be most marked in the isolated nuclear family, especially when estranged from relatives and when there was pre‐morbid illness in another family member. Close relationships, the presence of teenage family members, and stable external factors such as hospital support appeared to minimise subsequent family disorder. Binovular male twins are described, one of whom was brought to attention at birth because of a covered anus and the other at age 6 weeks because of vomiting, dehydration and failure to thrive. He was found to have a partially covered anus.Investigation revealed the presence of bilateral renal hypoplasia with oligomeganephronia in twin II, whilst twin I had normal renal function and normal renal histology.The only previous report of this condition occurring in twins was in monozygotic twins where again only one was affected.The relationship to covered anus is obscure and may be fortuitous.The condition is believed to be an acquired developmental defect.

The haematology of forty‐four infants with necrotizing enterocolitis (NEC) has been reviewed. Normal values for neutrophil, band cell and platelet counts as well as coagulation profiles have been defined for normal term and preterm infants at varying postnatal ages. In NEC, the absolute neutrophil count was of no diagnostic or prognostic significance but serial counts were useful in following the course of disease process. Band cell and platelet counts were strong predictors of onset of disease and both were significantly different in infants who died compared with those who survived the disease. Disseminated intravascular coagulation occurred in 11% of cases of NEC, all of whom died. Hypovolaemic shock preceded NEC in 5 infants and 3 infants had preexisting polycythaemia. The radiographic manifestations of necrotizing enterocolitis (NEC) were reviewed in 44 infants. As the clinical presentation of NEC could often not be distinguished from sepsis and/or surgical abdominal problems, abdominal X‐ray is by far the most sensitive test for the presence of this disease. Nonspecific radiological findings which frequently accompanied or preceded the appearance of intramural gas include bowel distension, a foamy pattern and asymmetry of gas pattern. The presence of these findings is an indication to repeat radiological abdominal examinations as an adjunct in the early diagnosis of NEC. The radiological hallmark of NEC is intramural gas, though this may be absent in infants with fulminating disease. If pueumoperitoneum occurs, it is diagnosed almost always at the onset or within 24 hours of the disease. Portal venous gas may be a transient sign and is not as ominous as previously reported.
