ABSTRACT.Factors useful in predicting the likely course of childhood asthma have been documented from a 14‐year prospective study of a randomly selected group of children with onset of wheezing before 7 years of age. The children were selected from the community in a manner to ensure that they were representative of the whole spectrum of wheezing in childhood. Features associated with significant asthma in early adult life included multiple periods of prolonged wheezing in the first 2 years of life; onset of eczema by 2 years; very frequent wheezing and the presence of eczema, barrel chest deformity, reduced FEV1, and a number of strongly positive skin tests to common environmental allergens at 10 and 14 years. Age of onset of wheezing had no prognostic significance. Adequacy of long term treatment did not seem to have any effect on the course of asthm

ABSTRACT.Recent warnings have been reported in the Press about the lack of improvement in Australia's perinatal mortality rate and the relative decline in Australia's position among the countries with the most favourable perinatal mortality rates. Such warnings are based on a false interpretation of recent and past perinatal mortality statistics. Australia's perinatal mortality rate has declined and is continuing to decline in recent years. Further, there is no evidence to suggest that Australia's perinatal mortality position has altered over the last fifteen years and continues to be very favourable by international standards.

ABSTRACT.A retrospective study of case notes was undertaken at the Royal Children's Hospital Melbourne, over a five‐year period 1974–1978 inclusive, to determine the need for long term anticonvulsant medication for infants who had convulsions in the neonatal period following birth asphyxia and/or trauma. Anticonvulsants were generally ceased before discharge from hospital.Of 38 infants available for follow up, 30 had no recurrence of convulsions by 12 months of age. Of the remaining 8 infants, one had a febrile convulsion, one infant remained on treatment because of a family history of convulsions and later convulsed with fever, one infant who convulsed at six weeks of age and five infants with major neurological sequelae who convulsed beyond the neonatal period, were recommenced on anticonvulsants.Twenty seven of the 38 infants were developmentally normal, and 11/30 had major neurological sequelae (five of these 11 had subsequent convulsions).If an infant ceases convulsing, and is behaving normally at discharge, long term anticonvulsants are not necess

ABSTRACT.A low osmolar modular defined formula diet has been successfully used in the treatment of 46 patients for a total treatment time in excess of 15 years. The diet is unique in that each constituent can be varied independently of any other, thus giving maximum flexibility in management.The diet is well tolerated in circumstances when the gastro‐intestinal tract is unable to tolerate other nutrients. ABSTRACT.The Transcutaneous Bilirubinometer was evaluated on 95 occasions on 66 infants who were clinically jaundiced but not receiving phototherapy. Meter readings were obtained from seven different body sites within 30 minutes of sampling blood for serum bilirubin estimation. A cephalocaudal progression of dermal icterus was demonstrated. Meter readings from forehead, sternum, thigh, upper and lower back correlated closely with serum bilirubin (r ≧ .93). The 95% confidence limits for forehead and sternum were ± 52.8 μmol/l. Multiple regression analysis using all 7 body sites increased the correlation coefficient to .97 and decreased the confidence limits to 44.4 μmol/l. However, we would recommend the forehead and sternum as the most reliable sites for meter readings with acceptable accuracy and precision. Meter readings from preterm and low birth weight babies tend to over‐estimate serum bilirubin. ABSTRACT.Thirty primigravidae took part in a prospective study of attitudes to the fetus, carried out by means of a series of semi‐structured, tape‐recorded interviews. The degree of development of the fetus was grossly underestimated in the first trimester. By mid‐pregnancy their ideas about the fetus had become more accurate and all gave realistic descriptions by 36 weeks gestation. Although most women had difficulty in believing that the fetus really existed in early pregnancy, a minority already had an established relationship with the fetus by 8–12 weeks of gestation.
