AbstractUnder‐reporting of asthma and inappropriate management at school can hamper a child's academic progress. The aim of this study was to assess asthma reporting and crisis management, following implementation of a Primary School Asthma Programme.The intervention covered 12 primary schools with 5093 students in an area of high unemployment with a large proportion of the community from a non‐English background. The programme included School Asthma First Aid Kits, training workshops for school staff and individual Crisis Management Plans for students with asthma. Registration of students with asthma at school increased from 6.2% before the intervention in 1989 to 12.7% in 1992. School Asthma Crisis Plans were completed appropriately by the child's doctor and parents and returned by 68% of the students with asthma. Teachers' asthma knowledge and confidence with the management of acute asthma at school improved following asthma education workshops. AbstractTo test the hypothesis that cow's milk protein intolerance (CMPI) might cause or contribute to gastro‐oesophageal reflux (GOR), 10 of 14 infants with abnormal GOR on prolonged oesophageal pH monitoring who had failed to respond to conventional antireflux therapy were placed on a hypo‐allergenic diet for 1 month. In no child was there significant improvement in pH monitoring indices, and only two showed any symptomatic improvement. Therefore, in these children, CMPI did not appear to contribute to GOR. It is probable that CMPI is rare as a cause of passive GOR, although our results do not exclude CMPI as a cause of active vomit

AbstractBone marrow transplantation (BMT) has had an increasing role in the treatment of acute nonlymphoblastic leukaemia (ANLL). A review of patients transplanted between May 1975 and August 1991 was undertaken in order to evaluate problems and outcome, and examine the role of both autologous and allogeneic BMT at various stages in the treatment of ANLL. Forty‐seven patients received either an allogeneic (n= 24) if a suitable donor was identified or autologous (n= 23) BMT if there was no allogeneic donor. Conditioning therapy consisted of total body irradiation (TBI) and cyclophosphamide (n= 14) or busulfan and cyclophosphamide (n= 33) with or without melphalan. Graft‐versus‐host disease prevention was with methotrexate (n= 9), methotrexate and cyclosporin (n= 10) or T cell depletion (n= 5). Minimum follow‐up for surviving patients was 24 months (median 65 months). For patients transplanted in first remission there was a 0% and 9±6% transplant‐related mortality, a 37±11% and 33±12% relapse risk and a 63±11% and 63±12% event‐free survival for autologous and allogeneic BMT, respectively. In second remission, there was a 14% mortality, 50±20% relapse risk and 43±19% event‐free survival for allogeneic BMT. No patient transplanted in relapse survived. In patients who survived greater than 24 months, late side effects were noted in all 8 (100%) patients given TBI and in 2 of 19 (10.5%) given busulfan. These data indicate that both allogeneic and autologous BMT are associated with significant event‐free survival when used both as initial therapy after complete remission has been achieved with conventional chemotherapy or in second remission. AbstractSubureteric Polytef injection is one of the alternatives now available for the management of vesico‐ureteric reflux in children. The results of the treatment by one surgeon, over a 2 year period, were studied prospectively in 47 children (60 refluxing units). Following a single injection, reflux was abolished in 49 (82%) ureters and of the 11 failures, 10 ureters showed a reduction of the grade of reflux while one remained unchanged. A repeat injection was performed in nine of these ureters with success in five, persistence in two, and two children are awaiting a follow‐up cystogram. The conservative success, with the addition of a second injection, was 90% (54/60). Of the four with unresolved reflux, two have low grade reflux and are free of infection, and two have had a ureteric reimplant, one of which was after a single injection, at the request of the parents. AbstractThis is a retrospective review of 185 short children who were tested for growth hormone (GH) secretion using the L‐dopa‐propranolol provocative test. One hundred and thirty‐three children were deemed to have passed the screening test when a GH concentration of greater than 15 miu/L was elicited after stimulation. Fifty‐two failed the screening test, of which 33 were diagnosed as having growth hormone deficiency (GHD) when they had inadequate growth hormone response to insulin‐induced hypoglycaemia. The other 19 were low‐responders since they showed adequate GH response to insulin tolerance test (ITT). The low‐responder rate to L‐dopa‐propranolol provocative test among short children who are not GH deficient was 12.5%.
