A plan of management of neonatal obstructive jaundice is proposed for use in units specializing in the treatment of this difficult condition. Reliance is placed in needle liver biopsy used early (and repeatedly if necessary) to judge the rate of development of fibrosis in the liver and to reach a diagnosis. For die paediatrician the understanding of growth is a cornerstone of his or her discipline and the impact of disease and/or deleterious environmental factors upon the growing infant or child is the focus of attention.It is therefore surprising that the study of growth and nutrition has not received the attention and research that it deserves. Indeed most of the work on growth in international centres has been left to the endocrinologist who may or may not be informed concerning nutrition and indeed may have no experience in the realm of body composition — so necessary in the assessment of tissue growth. Even in units devoted to anthropometry, linear growth and the consideration of weight for age, awareness has not always been apparent concerning the great potential of laboratory methods. Such methods greatly augment relevant information. In the clinic, the measurement of body length, weight and bone age are important for the assessment of growth and represent the initial approach. However as pointed out years ago change in body size for age can produce psychological changes in the child (Drash, Greenberg and Money, 1968). A follow‐up of the natural history of childhood asthma at 17‐18 years in a randomly selected population of asthmatic and control children who had been studied clinically and physiologically at 7, 10 and 14 years of age, has been conducted by questionnaire. Of the sample still in the study at 14 years of age, 77% of the asthma population replied, representing 65% of the original sample. There had been significant amelioration of asthma in those children who still had asthma at 14 years of age. Of those children with episodic asthma, approximately half had ceased wheezing while the remainder had relatively infrequent episodes. Those children with chronic asthma continued to wheeze but in just over half, asthma appeared to be episodic. During a three year periodGiardia lambliawas found in duodenal juice of 154 children. Faecal examination alone would have led to a missed diagnosis in more than 50% of patients.Clinical features and mucosal damage seen in these 154 patients are reviewed. Diarrhoea, failure to thrive and abdominal distension were common and improved after treatment. Histological abnormality occurred in 70%, and disaccharidase depression in 48%. Mucosal damage responded to treatment in the majority. The improvement of clinical features and mucosal damage with treatment, suggests thatG. lambliawas a pathogen in most of these children.

A survey of 1,118 newborn infants showed an incidence of infection of 18% and that of serious infection 5%. Prematurity predisposed newborn infants to both trivial and serious infection. Surgical induction of labour predisposed newborn infants to trivial infection, whilst repeated maternal vaginal examinations and instrumental delivery predisposed to serious infection. Prolonged rupture of the fetal membranes did not predispose to infection. This fact and the fact that there was an increased infection rate (albeit of only minor infections) if antibiotics were used suggested that prophylactic antibiotics should not be used in newborn infants born from pregnancies complicated by this problem, except when indicated for other reasons.

Diagnosis of an inborn metabolic defect causing neonatal death is difficult in the absence of a family history. We report a baby who was lethargic and hypothermic from birth. He developed stiffness, fits, profuse sweating and vomiting and jaundice before transfer to the Children's Hospital 3 days after birth. The finding of a very low blood urea shortly before he died from pulmonary haemorrhage led to diagnosis of the disorder by enzyme assays of liver taken at necropsy.
