ABSTRACT.Severe combined immune deficiency is a rare disease involving the B and T cell compartments of the immune system. This paper reviews the local experience of nine such patients and relates the findings to those in the literature.Of the nine patients, three are alive — all having been successfully transplanted with sibling bone marrow. ABSTRACT.Nine hundred and eighty‐six 10 and 11‐year‐old children were screened for psychiatric disorder using a method developed by Rutter for a same‐age English population. The prevalence rate was 14%, with rural children showing a lower rate (10%) than urban (15%), and metropolitan (18%). Comparisons between Australian and English children suggest a higher rate of disorder among the former; significantly so in the case of girls (p<.02), with a significant increase in emotional disorder among boys (p<.02), and an increase in conduct disorder among girls but not at a level of statistical significance. ABSTRACT.An enzyme linked immunosorbent assay (ELISA) was used to determine whether cytomegalovirus (CMV) specific IgM antibody was present in the serum of 135 neonates suspected of having congenital CMV infection. The assay was compared with the fluorescent antibody (FA) technique for cytomegalovirus specific IgM using the IgM fraction separated from serum by chromatography, a technique which has been shown to be a sensitive and specific method of detecting CMV specific IgM antibody.Elevated optical density reactions with the ELISA technique were only obtained with the seven sera that were shown to contain CMV specific IgM antibody by the fluorescent antibody technique. Sera from 128 neonates that were negative by the CMV‐IgM‐FA technique gave low optical density ELISA readings.The ELISA appeared at least as sensitive and specific as the fluorescent antibody test for detecting cytomegalovirus specific IgM antibody in neona

ABSTRACT.We have modified the ELISA assay for human rotavirus antigen detection so as to eliminate false positive reactions which occur due to extraneous antibodies. This has been accomplished by the incorporation of a control serum which contains no rotavirus specific antibodies, detectable by complement fixation, into the ELISA assay. Using this method, 9.7% of specimens giving positive results by the standard ELISA method as described by other workers, can be recognized as false positives and discarded.

ABSTRACT.Glucose tolerance and insulin secretion were studied after intravenous glucose load in 10 normal and 27 children with homozygous β‐thalassaemia of whom 11 received 5–20 units of blood (1.25–5g total iron) and 16 received 50–60 blood units (12.5–40g total iron). Fasting blood glucose and insulin levels were similar in normal and thalassaemic children. Differences were found at 10 and 20 minutes of intravenous glucose administration both in glucose blood levels. (which were considerably higher) and insulin levels (which were significantly lower), in thalassaemic patients compared to normal children. Evidence of depression of pancreatic function was found in three out of 11 patients, with low and in three out of 16 patients with severe iron load. ABSTRACT.In the 4 years 1977 to 1980, 375 infants weighing 501–1500g were admitted to the neonatal unit of the Queen Victoria Medical Centre. Overall neonatal survival was 79%; 56% of infants weighing 501–1000g and 89% of infants weighing 1001–1500g survived. Four post neonatal deaths occurred in hospital. Perinatal factors associated with decreased survival were indicators of perinatal asphyxia, thus demonstrating the imortance of the prevention of this condition in the management of early preterm labour. The survival rate for infants born after Caesarean section was significantly higher than those born after vaginal delivery for the whole study group but not among the smallest infants weighing ≤ 1000g. The two most common causes of respiratory failure were hyaline membrane disease and recurrent apnoea of prematurity. Two hundred and forty‐seven (68%) infants required assisted ventilation, of whom 116 (67%) survived. Although the majority of infants could be ventilated successfully with low peak airway and positive and expiratory pressures, the prevention of air leak and bronchopulmonary dysplasia remain a therapeutic challenge. ABSTRACT.In the 18 months, January 1979 to July 1980, 149 infants who weighed ≤1500g at birth were cared for in the neonatal intensive care unit at Queen Victoria Medical Centre, Melbourne. During this period the neonatal survival rate for infants weighing 501–1000g was 57% and for those weighing 1001–1500g was 91%. Of the 123 neonatal survivors, four died within the first year after birth. One hundred and seventeen (98%) of 119 surviving infants have been assessed at follow‐up. At a mean age of 12.3 months (corrected for prematurity), 11 (9%) survivors had physical handicaps only and four (3%) had developmental delay, including one child who was also physically handicapped. Nine of the 14 children with a defined handicap were considered to have significant functional handicap. Therefore 108 (92%) survivors are able to function within the normal range. ABSTRACT.One hundred and ten twin infants between 26 to 36 weeks maturity admitted to a neonatal unit over a 3 1/2‐year period were studied for the effects of prolonged rupture of membranes (PROM) on the development of respiratory distress syndrome (RDS). Fifty‐eight infants were born following PROM, 24 developed RDS; in contrast 34 of the 52 infants who did not have PROM developed RDS. There was a marked difference of incidence of RDS between the presenting infants who had PROM and the non‐presenting twins of the same pregnancy with intact gestational sacs (P<0.0005). Among the presenting infants, those with PROM also had significantly lower incidence of RDS compared with those without (P<0.005).
