Neonatal physical therapy is an advanced practice, subspecialty area within pediatric physical therapy. Because of the structural, physiological, and behavioral vulnerabilities of neonates, pediatric physical therapists need postprofessional precepted training and experience before providing neonatal care. Theoretical frameworks, scientific basis, clinical training criteria, and a decision-making algorithm are described to guide pediatric physical therapists working in neonatal intensive care unit (NICU) settings. Clinical competencies for neonatal physical therapy practice are outlined according to roles, proficiencies, and knowledge areas. The unique role of the neonatal physical therapist is highlighted as a postural control and movement specialist within behavioral, environmental, and family contexts in the NICU. Handling infants in NICU settings is considered inappropriate for student physical therapists and physical therapist assistants and aides because each contact involves ongoing examination, interpretation, as well as multiple adjustments of procedures, interventions, and sequences to minimize risk to infants who may be potentially unstable. The guidelines are intended to provide a structure for clinical training, practitioner competency evaluation, and clinical paths for neonatal physical therapy services. A glossary of selected terminology is included in the Appendix.

摘要:
The purpose of this article is to provide an overview of facts and issues related to child abuse and neglect. Common signs and symptoms of child maltreatment, characteristics of the abuser, prevention of child abuse and neglect, and the role of the health care professional are reviewed. The Pennsylvania Child Protective Services Law is provided as an example of current legislation related to child maltreatment. Legislation varies somewhat from state to state, however, health care professionals are generally mandated to report suspected cases of child maltreatment. Health care professionals are in a unique position to affect the lives of children and their families and are encouraged to become familiar with specific legislation regarding child maltreatment in the states in which they practice.

When the cocaine epidemic began, predictions were made that the children of cocaine users would demonstrate devastating negative developmental sequelae. In infants and young children prenatally exposed to cocaine most frequently the neurobehavioral and neuromotor systems have been studied. Although clinically described as irritable, difficult to console, and jittery as infants, research findings have not been able to clearly describe a pattern of long-term developmental sequelae. The mechanisms of action of drug exposure on developmental outcome have shown to be more complex than originally suspected. Many factors, other than the drug use, can influence developmental outcome. In most studies of prenatal cocaine exposure, family and environmental factors rarely have been measured, although their influence on developmental outcome has been well documented in other populations. This review of literature summarizes the research on the early effects of prenatal exposure to cocaine and other drugs on neurobehavioral functioning and neuromotor development in an attempt to provide objective and scientifically based information to clinicians working with this population.
