In a recent study on gait training in young children with disabilities, gait velocity and the Gross Motor Function Measure (GMFM) failed to identify changes in locomotor status that could be discerned by trained observers from video records. This led to the development of a clinical measure that would be sensitive to changes in the period preceding the onset of independent gait. The first part of this paper describes the development of the SWAPS (Supported Walker Ambulation Performance Scale), and the second part presents the results from a preliminary testing of its psychometric properties. Three experts were asked to determine the criteria they would use to determine changes in the walking performance of nine children with spastic cerebral palsy. After examining two series of video records taken at four-month intervals, the SWAPS was proposed. It consists of four dimensions: A) support, B) posture, C) quality of steps, and D) quantity of steps. These four dimensions are weighted so that support is worth 40% and the other three dimensions 20% each of the total score (100%). Each dimension is scored on a four-point Likert scale. The interrater reliability of the SWAPS was studied by having four physiotherapists score video records of the walking performance of the nine children. The intraclass correlation coefficient calculated for the total SWAPS score was 0.95, indicating a high-interrater reproducibility. Construct validity was also studied by correlating the SWAPS scores to the Gross Motor Function Measure (GMFM). A Spearmanpof 0.68 computed between the SWAPS and GMFM scores indicates a good relationship between the performance measured by the two scales. There are limitations to the present study: the development of the scale was made a posteriori using data recorded before its development in a small convenience sample, and the same sample of subjects was used for both the development and the testing of the scale. Despite these limitations, the present results are encouraging, and valuable information can be drawn from this preliminary study. Suggestions to improve the scale in future investigations are discussed.

Many physical therapists in educational settings still resist “role release” with claims of legal or ethical restrictions, despite 30 years of advocacy for transdisciplinary teamwork. Laws from 51 states and territories were analyzed to determine how physical therapy practice is defined. Laws delineated practices and procedures that physical therapists commonly use and some they may not use. Some laws also defined roles of physical therapy support personnel. Laws specified that only a licensed physical therapist may use the title, physical therapist, or claim to provide physical therapy services. No law specified that only a physical therapist may use certain procedures, but guidance was given for responsible delegation of responsibilities. Implications for physical therapists working with educational teams and the rationale for role release are discussed.

To further analyze the interobserver agreement in the use of a new protocol for Structured Observation of Motor Performance (SOMP-I), 35 infants between one and six months of corrected age were assessed by two physiotherapists. Twenty-seven preterm and full-term infants who consecutively attended a follow-up clinic were interspersed with five neonatally healthy infants and three infants admitted for physiotherapy. The protocol was designed to assess the level of motor development and the degree of deviation from the motor performance described at the achieved level of development. The assessment of levels of development showed good interobserver agreement when measured by the percentage agreement method (≥ 80%), by kappa statistics (≥ 0.62), and as the Spearman rank-order correlation coefficient (≥ 0.80). The majority of the assessments of degree of deviation showed agreement values from fair to good. Low-interobserver-agreement values were found for the items describing the sitting position (66%) and for the trunk in the supine and prone positions (66%). Thus, good interobserver agreements were achieved for most items on the SOMP-I.

Posture and balance are integral parts of voluntary movements. The purpose of this study was to investigate the effect of two different treatment interventions, neurodevelopmental treatment (NOT) and practice, on postural control of children with cerebral palsy during a reaching task. The subjects were eight children 10 to 15 years of age with cerebral palsy. They were treated daily for five days with NDT and five days with practice. Posture was assessed using kinematic analysis and videography. Total displacement of the head (TDH) and shoulder (TDS) during the reaching movement was used to assess postural displacement before and after treatment. In addition, postural alignment was evaluated and scored from a videotape using a modified version of the Posture Assessment Scale (PAS). No significant differences were found for either TDH or TDS for either of the treatment conditions. There were no significant differences in modified PAS scores for either treatment. However, when one subject's extreme data values were eliminated from the pooled data for the modified PAS, there was a significant improvement in postural alignment after NDT but not after practice. NDT seemed more effective than practice for improvement of postural alignment during a reaching task. Postural alignment rather than displacement of the head and shoulder in space may be more amenable to NDT and a more relevant variable for postural assessment.
