Public law 99–457 requires professionals providing early intervention services to expand their practice to support families caring for children with disabilities. Family-centered early intervention has created the need for physical therapists to review their roles and intervention strategies in working with children with disabilities and their families. Effective family-centered early intervention, in part, depends on the therapist's understanding of issues surrounding families caring for children with disabilities and the therapist's ability to effectively collaborate with families in developing therapy options which are meaningful to families. This article provides an overview of factors which may impact on the family's capability to meet the needs of the child with disabilities. Ways in which physical therapists can expand their roles to enhance the family's capacity to care for the child are also outlined.

PL 99–457 and its reauthorization PL 102–119, the Individuals with Disabilities Education Amendments of 1991 have been instrumental in shifting the emphasis of early intervention programs from a child-centered to a family-centered approach. The child is to receive a multidisciplinary evaluation using observational and standardized instruments to determine eligibility for early intervention services. The physical therapist has the expertise necessary to evaluate physical development, including gross and fine motor and several areas of adaptive development in order to formulate functional outcomes. Parents are included in the evaluation process as interactive and collaborative members on the multidisciplinary team. Strengths and priorities of the family as related to enhancing development of the child are assessed. Major areas of strengths and needs of the family are identified which are translated into outcomes in the individualized family service plan. Family-centered intervention assists families in choosing their desired level of involvement in the intervention of their child. The individualized family service plan is based on the comprehensive evaluation of the child and family and outlines the services to be provided.

摘要:
Good clinical practice requires that both the family and the child's needs be addressed in any program of early intervention. This paper demonstrates how family systems theory offers a rich framework for understanding and working with families. Issues related to compliance were selected to illustrate the application of systems theory because of the importance of this concern in clinical practice.

This paper reviews research pertaining to the use of parent report measures in the assessment of infants and young children. Based on current research on the effectiveness of parent report measures and the evolution of intervention philosophy to a family-centered approach, it is suggested that parent report measures should become an ongoing strategy in the assessment process. Based on the assessments reviewed, The Infant Monitoring Questionnaires, the Minnesota Infant and Child Development Inventories, and the Kent Infant Development Scale, appear most appropriate for use in early intervention programs.

The Family Adaptability and Cohesion Evaluation Scale (FACES-MI) was used to describe family interaction in a sample of families whose children required intensive care at birth. Families represented a low-risk group that was homogenous in terms of socioeconomic status, marital status, and education. The Peabody Developmental Motor Scales (PDMS), Test of Visual-Motor Integration (VMI), and the Motor-Free Visual Perception Test (MVPT) were used to determine the incidence of developmental delay in the children. The convenience sample consisted of 45 children and their 38 parents. The children ranged in age from 6 months to 7 years old. Parents reported congruence and satisfaction on measures of cohesion and adaptability on the Family Adaptability and Cohesion Evaluation Scale, indicating a stable family sample. On qualitative analysis of all the mothers' perception of their children's developmental status there were two distinct groups. One group thought the children were developmentally appropriate, the other group was less certain. Frequency data using PDMS developmental quotients indicated that 38% of the children scored 1.5–2 SD below the mean in total fine motor, and 26% of the children scored 1.5–2 SD below the mean in total gross motor. Positive correlations were found between the eye-hand coordination subtest of the PDMS and the VMI and between the VMI and the MVPT. The results of the study support the need for both quantitative and qualitative evaluation of family function and parent perception as part of sequential developmental follow-up of all children who require intensive care at birth.
