The Pediatric Evaluation of Disability (PEDI) was developed as a comprehensive functional assessment instrument for pediatric rehabilitation. The PEDI samples content in the domains of self-care, mobility, toileting, and social cognition. These content domains are evaluated by three separate scales: 1) functional skills/behaviors, 2) caregiver assistance of complex activities, and 3) environmental modifications and equipment. The PEDI can be used as a parent report/structured interview instrument or by professionals observing the child's functional behavior in a hospital, outpatient, or educational setting. This article describes the development edition (pilot) of the PEDI and includes a report of a content validity study with 31 expert reviewers. Overall, the content validity and feasibility of the PEDI were supported. Revisions made in the standardization version of the PEDI based on the content validity study are discussed.

A current method of defining motor delay, the cognitive referencing (CR) model, assumes that children who have similarly delayed motor and cognitive development are unlikely to benefit from motor therapy. In contrast, children with cognitive skills more highly developed than motor skills are identified as appropriate therapy candidates. This study compared the effects of motor therapy on two groups of children, 13 with higher cognitive than motor skill levels, and 20 with motor skills equal to, or higher than, cognitive skills. In contrast to expectations based on the CR model, students with equivalent motor and cognitive skills made significantly greater improvements on a perceptual subtest than did children with cognitive skills that were higher than their motor skills. The results of this study do not validate the practice of cognitive referencing. Rather, the results raise legal and ethical questions regarding the use of cognitive referencing as currently implemented, because of its apparent lack of validity.

The purpose of this study was to pilot procedures and measures for use in the assessment of the response of children with cerebral palsy (CP) to electromyography-triggered electrical muscle stimulation (EMC-EMS). Ten children, ages 51/2 to 16 years, who have spastic CP, participated in the study. Each received EMG-EMS to either the distal upper or lower extremities and a total body therapeutic exercise program. Gait, upper extremity function, range of motion (ROM), and overall motor development were assessed before and after eight weeks of treatment. Each child's posttest score was compared with his or her pretest score. The majority of the children accepted the EMG-EMS treatment and showed improvements in general motor function. When the improvements that each child made were analyzed according to the site of the EMG-EMS, both of the children receiving upper extremity stimulation showed more specific improvement in upper extremity ROM and function than did the seven children receiving lower extremity stimulation. All seven subjects in the lower extremity stimulation group showed positive changes in gait and lower extremity ROM measures, but this did not differ significantly from the improvements on these measures shown by the children in the upper extremity group. The use of EMG-EMS with children who have spastic CP may offer another approach to improve motor function. However, the results of this pilot study are inconclusive and suggest that a controlled study that compares therapeutic exercise alone to exercise with EMG-EMS is warranted.

摘要:
Three approaches used to alter posture and movement abilities in infants and young children are compared and contrasted. Included are neurodevelopmental treatment (NDT) and developmentally based programming, the two most commonly used approaches in the United States, and conductive education, a Hungarian approach that is being adopted in countries throughout the world. Each of these approaches differs in terms of critical features such as the population focus or intervention methods used. An understanding of these approaches can assist therapists in beginning to address issues of best fit between intervention approaches and specific child characteristics.
