PurposeThe aim of the study was to investigate the predictive value of the Movement Assessment of Infants (MAI) in early detection of neurological disorders in infants with extremely low birth weight (ELBW; <1000 g).MethodThirty-three infants were examined and predictions regarding neurologic status were made at the corrected age of four months using the MAI. Follow-up examinations took place at the corrected age of 12 and 24 months by a pediatric neurologist and a physiotherapist. A neuropsychological assessment using the Bayley Scales of Infant Development was completed at the corrected age of two years.ResultsFor predicting cerebral palsy (CP), the sensitivity was 64%; specificity 91%, and the positive and negative predictive values were 78% and 84%, respectively. For minor neurological disorders (MND), the value of sensitivity was 44%, specificity 71%, the positive predictive value was 50%, and the negative predictive value was 67%.ConclusionThe MAI assessment administered at the corrected age of four months is highly specific in predicting CP in the infants with ELBW.

PurposeThe purposes of this study were to: 1) determine if previously published descriptors of the supine to stand rising task in healthy individuals could be applied to the movements of persons with Prader-Willi Syndrome (PWS); and 2) assess upper extremity (UE), axial region (AX), and lower extremity (LE) movements among subjects with PWS compared with controls.MethodsNine subjects with PWS (seven–36 years of age) and matched controls were videotaped performing 10 rising trials. The UE, AX, and LE movements were classified using published descriptors. Occurrence frequencies of movement patterns, duration of movement, and the relationships among body region movement score, BMI, and age were determined.ResultsSubjects with PWS utilized developmentally less advanced asymmetrical rising patterns, took longer to rise, and demonstrated less within subject variability than controls.ConclusionsCategorical descriptors, with minor modifications, can be used to describe rising movements in persons with PWS. Knowledge of successful rising patterns may assist PTs when examining or planning intervention strategies for teaching the rising task.

PurposeThe purpose of this study was to gather expert consensus on physical therapist (PT) intervention after lower extremity botulinum toxin A (BtA) injection(s) for children with cerebral palsy (CP) and lower extremity spasticity. This study also examined differences in expert opinion on intervention for two groups of children with CP and differing levels of functional ability.MethodsThe Guide to Physical Therapist Practice was used to develop a questionnaire. Sixty-two therapists identified as experts in the field of pediatric physical therapy rated the importance of the three PT intervention components and the seven types of direct intervention from Neuromuscular Pattern 5A for both groups. The Wilcoxon signed rank test was used to identify differences in expert responses between the groups. Experts also ranked the importance of specific intervention strategies. Consensus criteria were used to determine the importance of interventions.ResultsAll three intervention components and four direct interventions (Therapeutic Exercise; Functional Training-Self-Care; Functional Training-Community and Work; and Prescription of Devices/Equipment) met consensus criteria for both groups. Significant differences between groups were found for Patient/Client-Related Instruction and Direct Interventions and for Therapeutic Exercise, Functional Training in Self-Care, Functional Training-Community and Work, and Electrotherapeutic Modalities, suggesting that these items were more important for children with greater functional abilities. Twenty (35%) specific intervention strategies met consensus criteria for one or both groups.Discussion and ConclusionsAll three intervention components and four direct interventions in Neuromuscular Pattern 5A are important in the PT plan of care after botulinum toxin A injection for children with CP and lower extremity spasticity. Functional ability influences therapists’ intervention choices.

PurposeThe purpose of this article is to review the literature related to the application of dual-task methodology in adults and children, particularly in the areas of gait and postural stability.Summary of Key PointsThe interaction between cognitive factors and motor performance recently has received considerable attention in physical therapy research. Researchers often use dual-task methodology to investigate the attentional demands of motor tasks or the effects of concurrent tasks on motor performance. The attentional demands of a task and the interference effects of concurrent tasks are influenced by a number of factors, including the performer’s age, level of skill, and the nature of the tasks involved. Even highly practiced activities, such as walking and postural control, are attention-demanding, especially in individuals with impairments.ConclusionsAn understanding of how attentional demands and other cognitive factors influence motor performance may be helpful to physical therapists in structuring intervention activities and in identifying children who have particular difficulty under dual-task conditions. By modifying both cognitive and motor task demands, therapists can tailor their interventions to provide appropriate challenges for children at different skill levels.
