Treatment gardens are designed in response to the dynamic physical, cognitive, social, emotional, and spiritual needs of individuals with Alzheimer's disease. Intended to meet specific treatment goals established by staff, the garden extends the care setting of a facility to include the outdoors as part of the nurturing and supportive environment. This article focuses on the application of a system of imaging elements to create naturally mapped outdoor environments that support independent and self-initiated use, engagement with the outdoor setting, and social interaction among cognitively impaired residents, staff, and family members.

This article discusses the ways in which the design of the physical environment of a residence for people with Alzheimer's disease can help sustain the social functioning of the residents. Spaces for programmed activities should have visibility, variety, and familiarity. More important are the spaces for non-programmed social engagement, where most community building takes place. These spaces, which are often the circulation paths in a facility should be well-traveled, pleasant, and open to other spaces.

This intrinsic case study explores the relationship between the physical setting and observed affect in participants of a dementia-capable adult day center. It is found that the setting in which the affect is observed has a small but statistically significant relationship to the enduring affects of engagement and awareness and also to the more fleeting affect of pleasure, even when one considers the level of activity engagement and degree of social interaction in which those participants are involved. It is suggested that the physical setting may play an even more important role in facilitating positive quality of life assessments (such as pleasure and engagement) than it plays in mitigating negative behaviors.

In residential care settings for people with Alzheimer's disease, kitchens located adjacent to dining and living spaces have been successfully used to help create homelike households of 8–15 people. Residents, family members, and staff value these settings because they result in the formation of extended families that slow functional decline, encourage social interaction, and improve life quality.This article summarizes design approaches and postoccupancy research on several generations of kitchens in residential care facilities that incorporate design concepts first used at Woodside Place in the early 1990s. It recognizes that reestablishing home is problematic for older persons and particularly confounding for people with Alzheimer's disease. Older people will be better able to reestablish home in a residential care setting if the complex personal, social, and emotional issues they encounter are considered as we make places that offer freedom, choice, and the opportunity for each person to be themselves.

The family caregiving literature suggests that, at the moderate stage of impairment from dementia, family members who provide dementia care often need to manage troublesome behaviors, such as catastrophic reactions (emotional outbursts) and wandering. As a result, families are introduced to a demanding situation of unpredictability and the need for constant vigilance. It stands to reason that care burden reaches its highest level during the moderate stage of Alzheimer's disease, partly due to the emergence of disruptive behaviors.1
