摘要:
&NA;Caregivers for individuals suffering from severe or terminal progressive degenerative dementia must consider the irreversible nature of this condition and adjust appropriately the treatment strategies. Maintaining or enhancing the quality of life should be a goal of care even in advanced dementia. This goal requires attention to the availability of meaningful activities, appropriate management of simultaneous infections and eating difficulties, and effective treatment of behavioral symptoms of dementia. Providing care on a special care unit (SCU) has advantages over care on traditional nursing home units. Advance directives should be formulated as soon as possible to guide end‐of‐life care of each individual with dementia.

&NA;Persons with dementia and their caregivers struggle with a myriad of difficult ethical decisions. As dementia progresses and the affected individual no longer has decisional capacity, family members and professionals must work together to achieve the good. Each stage of Alzheimer's disease raises its own unique set of moral quandaries. The purpose of this column is to provide a perspective on these many issues in the hope that continuing discussion will be of benefit to decision makers. There are issues that allow no single answer, yet it is imperative to think through all issues with accuracy and care.
