&NA;This introductory article provides an historical overview of special care unit (SCU) research, policy, and practice issues. It highlights findings from the National Institute on Aging Collaborative Studies on Special Care Units for Alzheimer's Disease. Its major conclusions are that policy, research, and practice issues are intertwined and that future efforts should focus on providing high quality care for persons with dementia residing in a variety of living and care arrangements.

&NA;In addition to the benefits that accrue to residents and their family members, research in an Alzheimer's disease residential care facility can be a rewarding experience for researchers and practitioners. This collaborative effort requires considerable flexibility, sensitivity, and willingness to devote the time and energy to enter and understand each other's world. This article reports on the experience of one applied researcher who had an opportunity to work with staff, residents and family members associated with an Alzheimer's disease residential care facility, and found the benefits worth the effort.

&NA;Staffing and administrative issues are among the most serious problems facing nursing homes and assisted living facilities in the United States. Staffing and administrative practices are critically important to organizational performance in terms of finance, human resources, level of quality, and satisfaction. In this article, we highlight the importance of five organizational systems as they relate to organizational performance: (1) environmental design, (2) programs, (3) information systems, (4) human resource management, and (5) leadership. A fundamental challenge facing researchers and practitioners today is translating research into practice. We describe a collaborative research strategy to better inform researchers, practitioners, and policy makers about how to simultaneously improve special care unit performance and expand our knowledge base.

&NA;This article interprets and reports on some of the findings from two NIA‐funded studies of special dementia care units. A focus in the analyses and the interpretation was the degree to which behaviors that are problematic to others change, accompanying differing amounts (minutes) of nursing aide input. Also examined was the degree to which such change, if any, was greater in special care units (SCUs) than in traditional units. Results indicate that there is more “bang for the buck” in SCUs. That is, additional aide time was found to have more impact in special care units than in traditional units, at least insofar as a reduction in problematic behavior is concerned.

&NA;The Agency for Healthcare Research and Quality reports that 40 percent of the aged will utilize a long‐term care (LTC) facility during their lives. With a potential doubling of elderly LTC residents by 2030, the need to train, recruit, and retain qualified health care professionals and paraprofessionals in LTC will escalate. This article presents results of a workforce initiative, LEAP for a 21st Century LTC Workforce. Developing effective LTC leaders and staff members benefits residents' quality of life. Unique to LEAP is its comprehensive approach on development of nurse leadership and communication skills, career enhancement for nursing assistants, and development of workforce teams. Initial program evaluation indicates significant value to residents, staff members, and organizations, strengthening retention, work effectiveness, and positive work climate.

&NA;This article explores advances over the past decade in understanding the role of the physical environment in supporting individuals with dementia. On a global scale, there is growing evidence that smaller and more homelike units are associated with more positive outcomes and that noise and distractions lead to less positive resident outcomes. At a more discrete level of the setting, there is also evidence that certain individual features (personalized orientation cues, direct visibility of the toilet, and increased lighting) have a positive impact of specific outcomes such as orientation, continence, and eating. However, much of existing environmental research is plagued by significant methodologic limitations that need to be overcome for the field to make significant advances in the future.

&NA;The unifying goal of psychosocial interventions in nursing homes is to enhance quality of life. The paths taken to arrive at this goal are diverse and reflect the complex nature of providing care to persons with dementia. Some types of interventions have been rigorously studied, whereas others are compelling because of anecdotal reports of their efficacy. The authors provide examples of programs across the spectrum of care that fall into four general categories: (1) programs that focus on individualization of care; (2) programs designed to promote functional independence; (3) programs that utilize “technology;” and (4) programs that utilize alternative therapies.

&NA;Activities for persons with dementia have “graduated” for beyond the past picture of basket weaving and large groups of residents packed into a room with one activity staff person shouting to be heard. It is exciting to witness the evolving therapeutic qualities of activities. But best of all, I applaud the respectful and warmly acknowledged thanksgiving for activity staff‐persons, and their creative, life enriching, person centered and fun‐loving opportunities of shared wellness.
