AbstractGlycolic acid has distinctive pharmacologic properties that recommend it for topical use in a range of skin disorders: acne, aging and photoaging, keratoses, age spots. Future studies and more extensive and varied clinical trials would surely point out its therapeutic potential and extend its range of clinical applications.

AbstractBackgroundThere is a general perception that sexual interest and activity does not persist into later life, with consequential failure to appreciate the potential prevalence of sexually transmitted diseases in the elderly.MethodsRetrospective analysis of case notes of all patients over the age of 65 years who attended a Genito‐Urinary Medicine Clinic: 81 patients were identified over a 6 year period.ResultsReferral because of positive serology for syphilis accounted for 20% of patients and was the most common reason for presentation. Significantly, another 14% of patients attended for HIV testing. Of the 61 patients whose sexual history was available, 32 had sexual intercourse within the 6 months prior to attendance.CommentThis review confirms the need for the medical profession to be more aware of the sexual activity and the possibility of sexually transmitted diseases in this patient grou

AbstractAimThe aim of the study was to utilize the so‐called reversed Köbner phenomenon to treat psoriasis by dermabrasion.BackgroundThe background has been reports on long term remissions following other surgical procedures such as dermatomy or laser surgery.Methods11 psoriatics with localized, chronic heavily infiltrated plaques, which had not responded sufficiently to topical treatment with steroids and/or ditranol or tar, were treated with dermabrasion, using a diamond fraise.ResultsAll treated lesions healed within 3‐4 weeks without recurrence with the exception of a 0.5 X 0.5 cm minor recurrence in one patient. AbstractObjectivesIn this case‐control study we tested the hypothesis of an association between some psychosomatic skin diseases, attachment style and stress.Patients and methodsA total of 177 cases and 194 controls seen between November 1992 and November 1993 at the Istituto Dermopatico dell'lmmacolata (IDD) in Rome, were enrolled into the study. Cases were outpatients with first diagnosis of hyperidrosis, chronic urticaria, generalized pruritus or alopecia areata. Controls were outpatients seen in the same period of time with first diagnosis of pigmenled nevi, keratosis or mycosis. The presence and weight of life stress events were assessed by u standard precoded questionnaire based on the Schedule of Recent Experiences (SRE) and on the Life Experiences Survey (LES). The attachment style was assessed by a modified version of the Shaver and Hazan questionnaire about feelings in a love relationship. We calculated 3 scores for each individual and classified study subjects in 2 groups: 1) "free" (= secure attachment); 2) "not free" (not secure attachment: anxious‐ambivalent or avoidant). Questionnaires were self‐administered in the presence of a trained psychologist. Adjusted odds ratios (OR) were calculated using a multiple logistic function.ResultsNo association was found between the different stress scores and the skin diseases considered. The crude odds ratio for life stress events in the previous year was 1.4 (95% CI 0.8–2.7). After multiple adjustment for age, sex, marital status and education, the estimated OR was 1.6 (95% CI 0.8–3.0). One significant association emerged between the adult attachment style defined as “not free” and psychosomatic skin diseases: the adjusted OR was 4.0 (9

AbstractAimTo compare the frequency of questionnaire based diagnosis of atopic diseases to those of clinical findings and sensitization and to evaluate the reliability of self‐reported reactions to allergens with those confirmed by skin prick tests.BackgroundNot only airborne allergens but also environmental indoor and outdoor air pollution are regarded as risk factors for the development of respiratory diseases in children. Metals, soot, SO2, passive smoking and pollution from new building materials irritate both the skin and the respiratory mucosa and also increase the risk of atopic sensitization. A strong indication that atopic diseases are common in Sør‐Varanger community, an area heavily polluted by the nearby Russian smelting plants, prompted us to investigate this hypothesis.SubjectsThe clinical and immunological examination involved 424 out of 575 schoolchildren aged 7–12 years.ResultsA total of 36% of the subjects were atopic; i.e. atopic dermatitis was established in 23% and mucous membrane atopy in 18%. 44% were definite non‐atopies, leaving 14% not classifiable in either group and 6% latent atopies. Sensitization was confirmed by positive prick tests in 69% of children with a history of pollen allergy and 11% of those with a history of allergy to animal dander.ConclusionsSkin prick tests are of little value in the diagnosis of atopic dermatitis but of major importance for the confirmation of mucous membrane atopy. In cases of controversy between history, clinical findings and sensitization, it is difficult to decide between atopies and non‐atopies. AbstractObjectiveInstrumental colour readings (Minolta Chromameter CR200) were performed in order to quantify corticosteroid induced skin blanching.MethodsDifferent Halcinonide concentrations in the same vehicle ranging from 0.005% to 0.200% were tested on the volar part of the forearms. Besides dose‐response relationships we studied the influence of application time by applying the formulations under occlusion during 2, 12 and 18 h.ResultsAll evaluated formulations provoked a discernible blanching as indicated by the significant increase of the L* parameter and the decrease of the a* parameter. Discrimination between the different Halcinonide concentrations was possible only in the 2 h application experiment. With longer application times a similar blanching was recorded for all Halcinonide concentrations.ConclusionOur results indicate that a shorter application time may lead to a more sensitive skin blanching assa

AbstractObjectiveTo report the development of giant pharyngeal ulcers in a series of HIV‐positive patients after commencing zalcitabine therapy. We also present an algorithm for the clinical management of oral ulceration in HIV‐infected individuals.Design and subjectsProspective review of patients who were prescribed zalcitabine. a nucleoside analogue reverse transcriptase inhibitor of HIV, at a single UK centre.SettingHIV out‐patient clinic at the Chelsea and Westminster Hospital in London.Outcome measuredOro‐pharyngeal ulceration associated with zalcitabine therapy.ResultsWe report 3 cases of zalcitabine‐associated oral toxicity which manifested as atypical giant pharyngeal ulcers. The subjects were all male homosexual non‐smokers who were participating in the Medical Research Council Delta trial and had previously been randomised to receive didanosine or matching placebo in combination with zidovudine, before commencing open label zalcitabine.ConclusionsOral ulceration is a documented side‐effect of zalcitabine, which may produce larger atypical lesions in some individuals. AbstractBackgroundThe views about the increased prevalence of cutaneous infections in diabetic patients are in conflict.ObjectivesTo study the prevalence of cutaneous infections in diabetic patients.Methods100 non‐insulin dependent diabetics and 100 controls were studied. Complete skin examination and routine dermatologic tests were used in the diagnosis of cutaneous infections.ResultsThe overall prevalence of infections was higher in diabetic than non‐diabetic patients. The toe webs were the most commonly affected sites. AbstractBackgroundIn the literature, Kaposi's sarcoma‐rheumatoid arthritis association is reported and it has been suggested that genetic predisposition may play an important role. Numerous studies indicate that immunological deficiency may also have an important role in the pathogenesis of Kaposi's sarcoma, along with a series of others factors.Patients and methodsWe present two cases of Sardinian women with rheumathoid arthritis and Kaposi's sarcoma where the relationship between the drugs used for the treatment of the former (particularly gold salts) and Kaposi's sarcoma is highlighted. In addition, their genetic background is delineated.ConclusionOur findings suggest that genetic background associated with drugs plays a role in the pathogenesis of Kaposi's sarcoma in rheumatoid arthritis patient
