AbstractHuman papiliomaviruses (HPVs) are recognized as likely viral agents responsible for genital precancer lesions and squamous cell cancers in both men and women. Nevertheless, their role in genital carcinogenesis is not entirely clear. This review aims to present current knowledge in experimental, biological and clinical work of diseases associated with HPV. There are many other agents, both non‐viral and viral, which might act synergistically or separately with HPV in a multistep tumorigenic process. AbstractThis article critically reviews both established and experimental treatment modalities for anogenital (AG) warts. Efficacy, cost, safety, advantages and disadvantages of each therapy are directly compared and treatment algorithms are suggested. Treatment of AG warts in pregnant women and prepubertal children is discussed. As AG warts are a sexually transmitted disease (STD) prevention of infection and the need to screen and treat patients and their sexual partners for other concurrent infections is stressed. The majority of patients with AG warts also have subclinical or latent HPV infection/associated disease of the AG tract. In this regard alt such women should be screened for the presence of cervical intraepithelial neoplasia (CIN).

AbstractToxic epidermal necrolysis leads to extensive exfoliative epidermal slough, fever, systemic toxic reactions, conjunctivitis and severe mucous membrane involvement. As it evolves many other organs can be affected. Whether or not toxic epidermal necrolysis is the most severe form of erythema multiform is still a subject of discussion. The pathophysiological events involved are not well understood. Indirect evidence suggests a hypersensitivity reaction, but the search for potential immunological mechanisms has resulted in little data to support this hypothesis. Drug reactions remain the most common associated factor. Twenty‐two patients, 3‐84 years old, have been included in this retrospective study. In 15 patients, clinical evidence points to drugs as the most important cause of toxic epidermal necrolysis. Recent infections were implicated in three patients and ulcerative colitis and lymphoma in one case each. Symptomatic therapy included fluid replacement, nutritional support and local treatment. Steroids were administered in 13 patients, followed by plasmapheresis in three. The mortality rate was approximately 27%. Elderly patients and patients with extensive lesions were at greater risk. AbstractWe have studied the clinical, histopathologic and histochemical features of pilornatricoma in a series of 22 cases. In early lesions, basophilic cells constituted the main tumor cell population and underwent four ways of maturation. Calcification was observed in 16 cases and ossification in two lesions. Melanin pigment was found in seven specimens and hemosiderin in six. Glycogen granules were present in most tumor parenchyma and less frequently in the stroma. Mitotic figures were frequent but cellular atypia was not observed.

AbstractThis retrospective study of 28 patients with pruritus and hyperpigmentation located on the upper half of the back, without involvement of other skin areas, includes 8 cases of macular amyloidosis, four neurodermatitis and 16 compatible with what has been described as notalgia paresthetica (NP). With the exception of the amyloid deposition, we did not find histopathological data that differentiated between macular amyloidosis and NP, since in both processes we found necrotic keratinocytes and melanophages. The differential diagnosis and the current nosological situation of the hyperpigmentation localized on the upper back are reviewed. A diagnostic protocol is proposed.

AbstractEight children, aged less than 5 years, with atopic dermatitis (AD) and food‐induced contact urticaria are described. These cases illustrate the clinical importance of food‐induced contact urticaria. The symptoms can be easily provoked and imitated in children under 4 years of age using the ‘Skin Application Food Test’ (SAFT). This test includes the application of foods on the skin, in the form they are consumed. The clinician should pay attention to clinical symptoms of immediate‐contact reactions, easy to recognize. The common skin symptoms are swollen face, in particular the lips, and itching or pain in the mouth and (contact) urticaria. Another dominating symptom is evident refusal of foods. Non‐immunological irritation reactions are also common in these children. Confusion with irritation reactions is first avoided of all by strict selective interpretation of the history told by the mother; and second, both the SAFT and the CAP RAST are positive only in immune‐media

AbstractAs a non‐invasive method, ultrasound is a suitable diagnostic procedure for routine in vivo examination of the human skin in dermatology. In this study 40 healthy test subjects were examined with a 20‐MHz ultrasound scanner in order to determine the thickness and morphology of normal skin at 51 different sites. B‐mode ultrasonography permits us to measure both the individual layers of the skin and deeper structures and to assess their definition and echogenicity. In both sexes the greatest skin thicknesses were found on: the shoulder‐blade (mean thicknesses (mm), males 3.07/ females 2.47); the upper back (m, 3.04/f, 2.85); the chin (m, 3.04/f, 2.69); the lower lip (m, 2.66/f,2.50) and in the region of the lower back (m, 2.64/f, 2.39). Particularly thin skin was found on: the upper eyelid (m, 0.59/f, 0.64); the lower eyelid (m, 0.99/f, 0.86); in the groin (m. 0.97/f, 0.90); in the armpit (m, 1.01/f, 1.09) and on the forearm (m, 1.13/f. 1.00 mm). AbstractIn order to evaluate the effect of Cyclosporin–A (CyA) at doses of 5 mg/kg per day in atopic dermatitis we studied clinical effects, immunoglobulin production, lymphocytes subset, immunphenotype and local cytokine production in seven patients before and after 3 months CyA therapy. Clinically CyA was very effective in eliminating pruritus, erythema and vescicles with no significant adverse side effects. Using flow cytometric analysis we demonstrated that CyA treatment restored an overexpression of circulating CD25‐positive cells. Similar results have been recorded with HLA‐DR expression. CyA normalized the number of CD29 cells which were reduced in the blood of patients before starting treatment.
