Percutaneous catheters such as Hickman-Broviac catheters have provided a means of central venous access for the chronic delivery of hyperalimentation and cytotoxic therapy for 15 years. Totally implantable access devices, consisting of a subcutaneous reservoir connected to a catheter positioned in the central circulation, have become available in the past 5 years and provide an alternative for patients requiring access for periods in excess of three months. However, infectious complications with either system may put the patient at risk. The literature is reviewed to evaluate whether there is any evidence to suggest that infection rates with implantable systems differ from those reported with percutaneous catheters. Thirty-four reports satisfying specific criteria are included. Although the results suggest that implantable devices may be associated with a lower incidence of infection in the long-term, it is concluded that a controlled study is required to determine whether there is definitive evidence of benefit.

The effect of lidocaine on the pain induced by the intravenous infusion of potassium chloride (KCl) was evaluated. Six volunteer subjects received 10 mEq of KCl in 50 ml of 5% dextrose in water in one arm, and in the other arm received the same solution with 10 mg of lidocaine added. Each solution was infused over 1 hour, and the subjects and investigatorwere blinded. Subjects rated pain on a seven-point scale (1 is mild, 7 is severe). Pain was significantly less severe with the solutions containing lidocaine. Mean values were 3.17 with lidocaine and 6.17 without lidocaine (p<0.01). Lidocaine in a dose of 10 mg added directly to the infusion solution significantly reduced the pain induced by the I.V. bolus infusion of 10 mEq of potassium chloride.
