Peripherally inserted central catheters (PICC) and extended catheters have become an acceptable norm within the medical community. In late 1991, the Kaiser Foundation Hospital in Fontana, California decided to take a serious look at these catheters. A task force led this project, developing a multiphased plan that began with raising awareness and moved on a continuum to full implementation. During 1992, more than 3,000 patient days were saved as a result of early referral to home care.

In this article, the authors share the results of a survey developed by a group of nurses in a 400-bed community hospital and distributed to 25 health care agencies throughout the northeastern New York region. The survey is comprised of six major categories, with questions pertaining to flushing and dressing protocols, types of vascular access devices used, and the availability of equipment and resources. Sixty percent of the agencies responded, including hospital, home care, hospice, and physician office settings. Major findings identified a lack of consistency in flushing and dressing protocols and in the types of equipment used. Implications for future practice will be presented along with the results of the survey.

The lack of published literature specific to the pediatric population (1 month to 18 years) was a major deterrent to the initiation of a peripherally inserted central catheter (PICC) and midline catheter program in a 260-bed pediatric hospital. The intravenous team assumed responsibility for the program initiated by the physicians. The following challenges were encountered: administrative issues, insertion-related discomfort, adverse clinical reactions, equipment inadequacies, catheter maintenance, and staff education. To date, the IV team has successfully placed 84 catheters and concludes that the procedure is invaluable for pediatric patients requiring an extended course of IV therapy.

Providing home intravenous care to patients with acquired immune deficiency syndrome (AIDS) in rural areas can be quite a challenge. The possibility of poor access to care, limited hospital resources, or a shortage of pertinent education available to the nursing staff may combine to make this specialized form of care difficult. In this article, these challenges are discussed and solutions are suggested.

Di-2-ethyl-hexyphthalate (DEHP) is a major component of polyvinyl chloride (PVC), which is the plastic of choice used to fabricate disposable medical devices. Most containers and tubings used to administer intravenous therapy are made with PVC, and DEHP controls the strength and pliability of these PVC products. Certain infusates have been shown to leach DEHP from plastic. The possibility of harmful effects to the human body is a concern, especially to infants and pregnant females. Carcinogenicity, hepatotoxicity, teratogenicity, and more recently, cardiotoxic effects have been documented, yet we continue to use these products that leach DEHP when alternatives are available. Exposure to this substance can be minimized with careful adherence to manufacturers' recommendations when administering substances that leach DEHP.

New drugs and therapies are continually emerging in an effort to delay the progression of human immunodeficiency virus (HIV)-positive status to acquired immune deficiency syndrome (AIDS). One such treatment is passive hyperimmune therapy (PHT), which was first researched and subsequently published in 1988. Passive hyperimmune therapy involves plasmapheresis of an asymptomatic HIV-positive donor with high p24 antibodies, no detectable p24 antigen, and a helper-inducer T-cell count greater than 400. The plasma is then pooled, sterilized, and administered to symptomatic HIV-positive patients as a monthly intravenous infusion in an effort to provide passive immunotherapy. In this article, an overview of PHT is provided, including benefits, adverse reactions, and other similar therapies available, so that the nurse who cares for HIV-positive patients can continue to be a significant source of information to them.

Patients with immunosuppression are especially susceptible to Mycobacterium tuberculosis and Mycobacterium avium-intracellularae, (also known as Mycobacterium avium Complex). Mycobacterium tuberculosis is the causative agent of pulmonary and extrapulmonary manifestations of tuberculosis, whereas the M. avium-intracellularae group has become recognized as a significant pathogen. Treatment problems center on multiresistant bacteria and poor patient compliance with prolonged treatment regimens. Lengthy multidrug treatment plans and special precautions against nosocomial transmission of these pathogens to patients and health care workers are necessary to limit the spread of these pathogens.
