PurposeTo determine the effect of hyperthyroidism on both myofiber number and myosin heavy-chain isoform composition within the palpebral orbicularis oculi muscle in rabbits.MethodsFour New Zealand White rabbits were made hyperthyroid by injection of 3,3,3′-triiodothyroinine intraperitoneally every other day for 1 month. Four rabbits were used as control animals. After 1 month the rabbits were euthanized, and the eyelids were excised and sectioned in a cryostat. The sections were immunostained to determine the presence of fast, slow, and neonatal myosin heavy-chain isoforms. To determine alterations in myofiber number, differential counts of myofiber number and the cross-sectional areas of the muscle fibers were performed with the use of computerized morphometry.ResultsThe orbicularis oculi muscle in the palpebral portion of the eyelids from hyperthyroid rabbits had significantly fewer myofibers compared with control eyelids, predominantly as the result of a loss of myofibers in the preseptal region. The remaining fibers showed continued expression of fast myosin but upregulated coexpression of slow myosin isoform.ConclusionsHyperthyroidism led to reduced orbicularis oculi muscle in the rabbit model and an alteration in the myosin heavy-chain isoform composition. This finding may help explain the clinical finding of eyelid retraction in patients with Graves orbitopathy.

PurposeTo show the efficiency of the ear helix sandwich graft as a simple method for reconstruction of full-thickness eyelid defects.MethodsA prospective case series of 13 patients requiring full-thickness upper or lower eyelid margin reconstruction was evaluated. Defects were reconstructed with a helical composite sandwich graft, and all patients were assessed for functional and aesthetic results and surgical complications.ResultsAll patients were satisfied with their functional and aesthetic results. Three patients had marginal skin loss on the composite graft, which was treated with daily dressings. Donor site complications occurred in two patients; one had a local infection that was treated with daily dressings and the other had wound dehiscence treated with marginal debridement and primary closure.ConclusionsThe helical composite sandwich graft is a good match for eyelids in terms of color, texture, and contour. The use of the helical composite sandwich graft is a simple, effective, and safe procedure for the reconstruction of full-thickness defects of the eyelid margins.

PurposeTo identify and summarize the available published literature describing histologic features of congenital blepharoptosis and to consider the evidence for its classification as either a dystrophy or a dysgenesis. A brief overview of etiologic factors is included to highlight the controversy surrounding the cause of the condition.MethodsOnline searches of the current medical literature were performed with the use of search engines provided in British University libraries. Keywords used were ptosis, blepharoptosis, and birth defect, combined with eyelid. Manuscript copies were obtained and translated where necessary.ResultsA summary of data found in more than 35 published papers, including theories of etiology (muscular versus neural, genetic versus sporadic, and environmental factors) and evidence for the classification of congenital blepharoptosis as a dysgenesis and as a dystrophy. Study design and data interpretation are commented on where appropriate.ConclusionsThe scarcity of both affected tissue and normal fetal tissue for histologic examination has limited the understanding of the development and classification of congenital blepharoptosis. Each study analyzed had small specimen numbers, highlighting the need for a review of all available data. The available histologic data appear to support the classification of congenital blepharoptosis as a dysgenesis rather than a dystrophy. It is predicted that a greater understanding of the etiology and histology of this condition may distinguish further subtypes. It is also anticipated that a greater understanding of this condition will allow developments in preventive and therapeutic approaches.

PurposeTo present a case of epibulbar Fordyce nodules, with a referral diagnosis of primary tumor.MethodsCase report.ResultsA 38-year-old woman was referred for ocular oncology consultation because of a conjunctival lesion in the right eye. She had had a buccal mucous graft to treat recurrent pterygium 18 years earlier. The lesion consisted of multiple small, yellow granules over a pink, thickened mucosa from the 12 to 3 o’clock meridians. Excisional biopsy revealed multiple subepithelial sebaceous glands consistent with Fordyce nodules.ConclusionsFordyce nodules are a possible late benign complication of buccal mucous grafts.
