AbstractTrimalar fractures of the zygoma create a fracture dislocation of the lateral and inferior orbital walls. These are frequently associated with orbital and ophthalmic injury. A surgical approach to trimalar fracture repair should provide adequate exposure to the orbit and flexibility in dealing with associated orbital trauma. Surgical techniques for repair of trimalar fractures are reviewed. A new technique is presented that provides optimal orbital exposure and minimizes facial incisions.

AbstractIn this series of 24 patients with 28 canalicular lacerations, bicanalicular silicone intubation is found to be an effective technique with 95% anatomic success. No statistically significant difference is noted in successful repair when comparing proximal, mid, and distal lacerations. There are no recognized cases of iatrogenic injury to either the injured or the uninjured canaliculus in this series. The repair of all canalicular lacerations is advocated.

AbstractComputed tomography (CT) is far superior to plain radiography or xeroradiography in the detection of orbital foreign bodies and in defining altered anatomic relationships in this region. We present a case in which CT was instrumental in the management of orbital cellulitis in a patient who had forgotten that an eye prosthesis had been inserted in the past. We stress the importance of CT in our case and discuss its usefulness in other reported clinical situations involving the orbit.

AbstractTo avert complications of blepharoplasties the surgeon must first communicate with the patient. Once a rapport is established, a thorough preoperative evaluation is performed. It is imperative to determine the expectations of the patient and to communicate the realistic results that can be achieved from surgery. Strict adherence to proper surgical technique is paramount for good postoperative results. If complications occur, correct diagnosis of the etiology of the problem usually leads to a satisfactory resolution.

AbstractChildren often experience extremely high levels of psychological stress in the operating suite because of anxiety associated with the separation from their parents, the unfamiliar and intimidating environment, the presence of strangers in unusual attire (masks, caps, and gowns), the act of being forcibly restrained, and the forceful pressing over the mouth and nose of an anesthetic mask that emits a foul-smelling gas and that seems to restrict breathing in a manner reminiscent of suffocation. The recovery room can be very frightening for children because of the separation from parents and the frequently overpowering scene of human injury and suffering. If the personnel in the operating suite are trained in child development, and if they are encouraged to practice elementary “preventive psychiatry” and to use common sense, then the psychological morbidity resulting from the experience in the operating suite can be minimized. Arrangements ought to be made for children undergoing relatively simple and short operations to go to a place different from the “intensive-care” type of recovery room.

AbstractFive cases of orbital varices are presented, demonstrating a surgical approach with the use of vascular clips. Three of four cases were clipped at the apex of the orbit, with no recurrence. A fourth case was clipped two-thirds of the way back in the orbit, with no progression of the varix and significant improvement of the patient's symptoms. A fifth case failed because of extensive hemorrhaging. Follow-up ranged from 2 to 9 years.
