This study compared two ways of treating chronic muculoskeletal pain, using randomized assignment of 131 patients to one of two electrotherapy methods: electroacupuncture or acupuncture-like TFNS. For the latter a new device called Codetron (Trademark of FHM) was used which had many features to ensure strong and unhahituated stimulation. For both methods, frequencies of 4 Hz and 200 Hz were used to optimize release of endorphins and serotonin. Initial outcomes were evaluated using visual analogue scales for pain relief and activity improvements. The “initial” outcomes during treatment weeks were not significantly different for both electrotherapy methods. Follow-up data were obtained by telephone 4 to 8 months after cessation of therapy: these were significantly better for Codetron than for eleetroaeupuncture but we must remain cautious about telephone interviews.

A quick screening paper and pencil instrument was constructed tor rapid assessment of posttraumatic stress disorder (PTSD) in the chronic pain population. Preliminary indices of reliability and validity indicate that the new test, titled the Posttraumatic Chronic Pain Test (PCPT). consistently and accurately identifies patients at high risk from stress-related, posttraumatic chronic pain.

Children who had experience rating their headaches on a 0–5 rating scale were asked to draw pictures of themselves, which depicted various intensities of pain that they had experienced. The drawings were rated by blind independent raters for intensity perceived, and for features present in the drawings. Interrater agreement was calculated and a discriminant analysis was performed to determine whether there was a clustering of certain features present tor each intensity level examined. The results indicate that drawings of moderate pain are much more difficult to rate than no pain and extreme pain drawings.

Two hundred twenty-five chronic pain patients were studied following training in mindfulness meditation. Large and significant overall improvements were recorded post-intervention in physical and psychological status. These gains were maintained at follow-up in the majority of subjects. Follow-up times ranged from 2.5–48 months. Status on the McUill Melzack Fain Rating Index (PRI). however, tended to revert to preintervention levels following the intervention. Most subjects reported a high degree of adherence with the meditation techniques, maintenance of improved status over time, and a high degree of importance attributed to the training program. We conclude that such training can have long-term benefit for chronic pain patients.

Dezocine. one of a scries of new bridged aminotetraline analgesics possessing narcotic agonist and antagonist activity, was compared with morphine and placebo in a double-blind study of 162 postsurgical patients in pain. Patients in the four study groups were comparable in all demographic aspects. The patients receiving dezocine were given either 10 or 20 mg by intramuscular injection: the larger dose has not been studied previously in parenteral administration for postoperative pain control. At all times studied, up to 6 h after administration, dezocine 20 mg was significantly more effective in relieving pain than either de/ocine 10 mg. morphine 10 mg. or placebo. Dezocine 10 mg was not significantly different from morphine 10 mg. but was more effective than placebo. Side effects were minor in all groups, but nausea and vomiting were found to he highest in the placebo group, whereas the dezocine group had more dizziness and vertigo. We concluded that parenteral dezocine serves a useful role in the safe and effective management of moderate to severe postoperative pain.

We evaluated the effectiveness of a behavioral program in reducing headache recurrence in children with migraine using a multiple baseline design. Treatment consisted of electromyographic (HMG) biofeedback. relaxation training, and operant pain behavior management. The children studied were randomly assigned to four groups. Each group received the same treatment over nine sessions, but groups differed in the length of baseline assessment of headache occurrence: 3. 6. 9. or 12 weeks. When treatment was introduced, the results indicated statistically significant reductions in dependent measures of headache occurrence across groups. Thirteen of the 17 children who participated in follow-up maintained treatment gains 6 and 12 months after treatment ended. These results are viewed as supporting the usefulness of behavioral treatment as an alternative to medication in the management of pedialric migraine.

Single and multiple dose analgesic efficacy and side-effect profile of two tablets of a combination of codeine (6.8 mg. aspirin 250 mg. and acetaminophen 250 mg were compared in a double-blind crossover study in 50 patients with pain after third molar surgery with a combination containing the same aspirin and acetaminophen doses, but without the codeine.There was no measurable difference in analgesic efficacy after a single dose: no measurable difference was found over the first two postoperative days. even though a mean of 133 mg of codeine was taken in addition to aspirin and acetaminophen. The codeine combination resulted in significantly more nausea, but also in significantly, less infection of the operative site. No analgesic benefit of adding low doses of codeine (13.6 mg/dose) to an aspirin and acetaminophen combination was seen, and the threshold codeine dose which produces measurable benefit is not known.
