This is a retrospective study of 53 chronic pain patients who were treated at the Boulder Pain Control Center. Only one measure of treatment success was examined: the cost benefit of that treatment to two workman's compensation insurance carriers. This was accomplished by using a model that projected, in our study population, an average cost benefit of $238,515 per patient. The social value of determining such measures in chronic pain patients is discussed.

Twenty cancer pain patients and 20 matched chronic benign pain patient controls were compared on dimensions of painintensity, affect, expectationsof treatment. Cancer patients perceived their pain as more unpleasant and used words associated with higher sensory intensity than did chronic benign pain patients. Lower levels of emotional distress and higher expectations of relief were reported by cancer patients than by the controls. While affect was unrelated to pain variables for cancer pain patients, affect was consistently related to pain variables and expectations of relief from treatment for chronic benign pain patients.These results suggest that while there is a close interplay between affect and pain perception for chronic benign pain patients, affect and pain perception appear to be distinct for cancer pain patients. These findings imply the necessity for differential treatment of pain complaints in these two patient populations.

Low-intensity, high-frequency transcutaneous electrical nerve stimulation (TENS) is generally better tolerated by most patients than acupuncture-like TENS. In clinical practice, its pain-relieving effect has appeared to be a rather localized phenomenon. Since it is difficult in some instances to apply counter irritative stimulation at or close to the painful site, this study explores the potential of contralateral low-intensity TENS for pain alleviation. Changes in subjective sensitization to thermally induced experimental pain are used as the criterion of TENS effectiveness. Results show that contralateral TENS produces approximately half the pain-reducing effect of ipsilateral stimulation. It appears to be a viable alternative method of pain control in patients for whom conventional modes of TENS application are problematic.

Research has indicated that a diversity of physical (e.g., drugs, bite-plate) and psychological (e.g., biofeedback, individual and group psychotherapy) interventions will alleviate the symptoms of myofascial pain dysfunction (MPD) syndrome in 70 to 80% of patients. The purpose of the present study was to evaluate the effects of two relaxation training methods that are less costly and have fewer side effects than other methods, and to compare these with a previously reported realistic placebo method. The results indicated that both relaxation training methods produced about equivalent effects. Both were more effective than the realistic placebo method, while at the same time having several apparent advantages over other methods in terms of cost, side-effects, and generalization and maintenance of treatment effects.

This study evaluated the efficacy of training in cognitive strategies for attenuating pain during orthodontic banding. Twenty-five patients, aged 8 years to 15 years-10 months, were randomly assigned to one of three groups: distraction training, reframing training, and irrelevant training control. Assessment of dental pain included heart rate, self ratings, and ratings by the orthodontist and his assistants. The results support the efficacy of cognitive strategy training in attenuating clinical pain. Compared with the controls, patients taught the cognitive strategies had lower heart rates and were rated as having less pain during the first half of the banding. Indices of pain during the procedure were correlated positively with self-reported general sensitivity to pain, and correlated negatively with motivation for braces.

Twenty-two courses of oral methadone were given to control moderate or severe pain in 19 children with cancer. Of these, 21 courses gave adequate pain control for periods of 5 to 267 days (median 24 days). In 16 courses, methadone was continued until death or until the pain resolved; in five remaining courses, a change to parenteral narcotics was ultimately required. Toxicity was minimal. A safe starting dose of methadone appears to be 0.1 mg/kg given every 4 hours, or the equivalent total daily dose given less frequently, with escalation as required to achieve and maintain adequate pain control.
