Human subjects received irradiation of the skin overlying peripheral nerves with a helium-neon laser (1 mW. 632.5 nm. 20 Hz) for 20 s to each site. This treatment was accompanied by irradiation of the skin overlying paingul facial areas for 30–90 s according to a predetermined protocol. Control subjects received placebo treatment by an apparatus that looked identical to the laser apparatus but emitted no radiation. Laser or placebo therapy was repeated 3 times a week for 10 weeks. Subjects in the experimental group exhibited a statistically significant reduction in the intensity of pain as measured by the visual analog scale (p < 0.002) and the number of painful episodes. These results, combined with previous research. indicate that laser therapy may provide relief from some kinds of chronic pain.

Previous reseach has shown that the reaction to pain is influenced by ethnocultural group of origin. Key variables that have been shown to differ include attitudes toward pain. anxiety, and perceived self-management. Theoretrically, differences between groups need not be limited to ethnic origin. Other group norms also should be influential. In this study, 119 dental patients within Israel responded to a previously used attitude-toward-pain questionnaire, State-Trait Anxiety Inventory. Self-Control Shcedule, and Dental Self-Control Schedule. Patients were divided according to ethnic origin, social class, and sex. Results indicated that social class and sex were more powerful reference group than ethnocultural groups of origin in determining pain attitudes, state anxiety, and self-management. Ethnic origin was significantly related to trait anxiety and dental self-management, but less so than social class and sex.

Because of difficulties encountered with patient compliance using the standard visual analogue scale, a new nonvisual analogue scale has been devised for pain measurement. The new scale was found to give values for pain that correlated well with values given using the visual scale and was found to be more easily understood. The scale was considered to be simpler and more reliable than the visual scale heretofore in use.

Thirty lumbosacral spine films with posteroanterior and lateral views were reviewed following insertion of epidural catheters and injection of contrast medium. The characteristics identifying the normal epidural space are described and the radiological distortions resulting from laminectomy and other surgical procedures are also detailed. The radiological features accompanying inadvertent misplacement of the catheter into epidural vein, intrathecal space, and paravertebral tissue are also described.

Pain, depression, and illness behavior were investigated in a group of 56 elderly persons. Of the subjects, 76.7% reported having pain complaints. The pain group was significantly different from the pain-free group on three of the seven scales of the Illness Behavior Questionnaire. Botrh groups obtained relatively high scores on the Denial Scale, suggesting that denial was used to fend off fear of disease and death. The two groups were nondepressed on the Beck Depression Inventory.

Forty-two patients presenting to a university-based, multidisciplinary pain clinic with chronic pain in the head/neck region completed a scale assessing their belief systems on personal control of health behaviors. The patients were dichotomized into high and low groups on three scale factors (internality. powerful other, and channel and compared on measures of pain description and quality, health related behaviors, and psychological functioning. In particular, patients attributing health control to powerful others were older, less likely to report use of analgesies, and were more convinced of a somatic basis for their symptoms. Patients who attributed control of health behaviors to themselves obtained lower disease conviction scores and reported greater “other” medication use. Patients attributing control to chance factors were less educated and obtained higher disease conviction scores. The results of the study indicate considerable variation among chronic facial pain patients in belief systems regarding personal control of health behaviors and the clinical implications for differential pain management strategies are discussed.

Herpes zoster occurs following a reactivation of dormant varicella virus, which produces neurogenic changes in the nerve roots of spinial and cranial nerves. The symptomatology that follows includes pain and vesicular eruptions over the affected dermatomes. In an attempt to minimize the pain of postherpetic neuralgia that may follow the acute eruption of herpes zoster, a series of stellate ganglion blocks (for upper extremity, head and neeck lesions) or lumbar sympathetic blocks (lower abdomen and Iower extremity lesions) may be performed. When herpes zoster of the face. i. e., herpes zoster ophthalmicus occurs, contralateral hemiplegia may occur with this condition. This usually follows the migration of the virus from the dorsal root ganglia through the ophthalmic nerves into the carotid vessels. The resulting situation may produce neurological signs and symptoms, which if not accurately diagnosed. may be attributed to complications of the stellate ganglion block series or cerebrovascular accident. The treatment of this clinical syndrome consists of supportive care, intravenous heparin, acyclovir, and physical therapy.

Successful treatment of intractable deafferentation pain by dorsal root entry zone (DREZ) operation in a patient with chronic tibial nerve injury is presented. This 70-year-old man had a crush injury on his right foot, and intractable pain developed 2 months after ther injury. At operation, evoked potentials were directly recorded from the dorsal roots. After determining the root that presented maximum response to tibial nerve stimulations. DREZ lesions were made. The effectiveness of DREZ operation in root-intact deafferentation pain is discussed. And, the usefulness of teh directly recorded root potentials evoked by stimulation of the proximal protion of the injured nerve is emphasized.
