Twenty patients with acute herpes zoster participated in a double-blind placebo-controlled study using sympathetic nerve blocks. Ten patients received sympathetic nerve blocks using a local anesthetic and 10 received a placebo. The local anesthetic was effective in resolving acute herpetic neuralgia in 90% of the patients while the placebo was effective in 20%. This difference is significant at thep< 0.01 level. While the exact mechanism of action of the sympathetic blockade in relieving zoster pain is not understood, the immediate relief afforded by sympathetic blocks is, however, gratifying and favors its use when available.

The emotional status and marital adjustment of spouses of chronic low back pain patients were investigated. One hundred and seventeen spouses and patients were administered the MMPI, the Locke-Wallace Marital Adjustment Scale, and the Areas of Change Questionnaire as part of a comprehensive psychosocial assessment in a chronic pain treatment program. Twenty percent of the spouses reported clinically significant depressive symptoms and over 35% rated their marriages as maladjusted. Spouses' emotional distress was related to patients' emotional distress, and spouses' marital adjustment was significantly related to patients' level of functional impairment, specifically social interaction deficits. These results call for routine evaluation of spouses of patients with chronic low back pain and inclusion of families in the treatment of this chronic illness.

This study determined the efficacy of two modes of transcutaneous electrical nerve stimulation (TENS) as a nonmedicinal means of pain control in dysmenorrhea. Twenty-seven subjects (nine per group) were randomly assigned to either conventional TENS (Group 1), strong low rate acupuncture-like TENS (group 2), or a control group (Group 3). Each group received similar instructions, used the same TENS units and electrodes and also completed a posttreatment questionnaire. A mean decrease in pain of 72.2%, 51.3%, and 26.1% was observed for Groups 1, 2, and 3, respectively. Statistical analysis revealed that the mean decrease in pain and duration of pain relief between the conventional and control group was statistically significant. This study indicated that TENS can provide a safe and effective nonmedicinal means of pain relief for dysmenorrhea.

Pain is one of the most common and costly health problems in America. When pain becomes chronic, it becomes the disease, and the misapplication of therapies intended for acute pain may reinforce and perpetuate illness behaviors. Low back pain comprises greater than 50% of the presenting complaints in a busy, comprehensive pain clinic facility. In addition to standard medical treatment of patients with chronic low back pain, a review of many studies is presented indicating the benefit of early psychological intervention. Those groups utilizing concurrent psychological management experienced greater treatment success. The objectives are to have patients with chronic low back pain become aware of their emotional responses to painful stimuli, assume a more active role in controlling their pain, and become more productive members of society.
