Abstract365 2nd through 4th graders completed a 3‐yr clinical trial on the caries‐preventive effect of rinsings every second week during the school year with 10 ml of an 0.2% neutral solution of sodium fluoride. All children received regular dental examinations and treatment in clinics established by the municipality in which the study took place. The trial was performed under double‐blind conditions. The caries increment on teeth erupted at baseline was 1.75 DMFS in the fluoride group and 1.83 DMFS in the placebo group (P>0.05; 95% confidence limits for percentage caries reduction: ‐20.7% and 29.5%). AbstractDMFS increments were determined in 1286 military cadets (19–20 yr old), at 6‐monthly intervals during an 18‐month clinical study. The cadets were randomly divided into two groups. The lest group (n= 787) brushed their teeth with an amine fluoride gel for 5 min once weekly in an unsupervised manner. The control group (n= 499) as well as the test group used a fluoride‐free dentifrice twice daily. In both groups, motivation in oral hygiene was given before commencing the study. AbstractThis study compared the oral health status of 600 6‐16‐yr‐old young persons: 297 from Heidelberg, Victoria, Australia (an area of recent water fluoridation and a predominantly market‐place delivery system) and 303 from Dunedin, New Zealand (an area with established water fluoridation and a long‐standing school based public delivery system). A greater proportion of Heidelberg subjects, in each of three age groups, were caries free. Level of dental plaque varied both with study area and with age group. Dunedin subjects in the younger age groups had lower mean levels of plaque than their Heidelberg peers. The New Zealand dental service delivery system was consistently closer, in each age group, to meeting defined dental needs than the Australian system. The similarities and contrasts between this study and the WHO‐ICS illustrate the importance for planners to consider carefully differences in local conditions which may affect service and morbidity

AbstractThis paper analyzes use of services, behavioral features and their intercorrelations among a random sample of 94 patients visiting University of Connecticut dental clinics. The data were collected in interviews and patient records. In care seeking the greatest percentage (34%) of respondents relied on the advice of friends, relatives or neighbors. This finding confirms the importance of the lay referring system. The influence of regular source of care was apparent when patients’frequency of visits in clinics exceeded considerably their visit frequency in the past. Even earlier irregular visitors made many visits during the past 12 months. The following dimensions of satisfaction were measured: difficulty in getting an appointment, general satisfaction with care, costs of care in general, convenience in treatment and satisfaction with oral health. The results show that opinions toward care and about dental health were dominantly positive ranging from 60 to 94% of all the answers. Costs were regarded as high by 54% of the subjects. There was a positive and significant correlation between degree of utilization and satisfaction with care. The correlation between satisfaction with oral health and number of teeth was non‐significant. AbstractThe caries experience and dental fluorosis of 84 Danish children, who had used fluoride tablets for 1–4 yr in the period 1976–80, were compared with those of a group matching in sex, age, place of living, and socioeconomic status. The average age of the children at the time of examination was 5.8 yr. A recording of mothers’attitudes to dental care, knowledge about tooth brushing, attitudes to candy, and number of teeth in the maxilla showed no difference between the fluoride tablet group and the non‐users’group. Moreover, there was no significant difference between the two groups with respect to dental caries. Abstract2000 persons from an a‐select sample of the Dutch population were sent a questionnaire in which the most important question was: “Did you have any questions on matters related to dentistry during the past year?” The question was by nature an open question, and the respondents were entitled to several answers. 640 questionnaires were sent back, of which 398 actually provided an answer to the request. Although the request was for questions only, there were also clear expressions of dissatisfaction. A coding system is described that was developed to classify the 873 answers, in which 751 were identified as questions and 122 as complaints. The problems are highly varied as to their contents. Most questions bear on the dentist's activities (35%). Most striking is that these questions can be answered by any dentist. The category on dental minded behavior accounts for 22% of the questions, that on various aspects of the dentist‐patient communication and on fear accounts for 17%, and the category on financial problems in dentistry accounts for 16%. Bearing in mind that the composition of the group of 640 respondents differs from that of the entire Dutch population, the results should be interpreted with due care. AbstractProsthetic needs and demands for care were investigated in a group of the elderly, residing in Romema, Jerusalem, Israel. Of the population aged 60 and above residing in the neighborhood, 34% (n= 116) were selected at random. These were examined under standardized conditions and prosthetic status and denture requirements were recorded. The findings indicate that the prosthetic status was poor and the treatment needs extensive. In all, over 83% of the study population were edentulous, the majority in both arches. Although 86.3% of the edentulous subjects possessed dentures, over 86% of these subjects needed some form of prosthetic treatment. About 60% needed new full or partial dentures and over 25% had dentures whose fit and retention could be improved by rebasing, relining or minor adjustments.

AbstractFour psychometric scales: Corah Dental Anxiety Scale (CDAS), a modified version of the Geer Fear Scale (GFS), the Health Locus of Control Scale (HLCS), and a Mood Adjective Check List (MACL), were studied with regard to their implications for dental fear. Swedish versions given to groups of fearful and non‐fearful dental patients as well as non‐patients showed satisfactory metric properties. Mean scores obtained were in most cases similar to those reported for original American versions, even though some indications of cultural differences were observed. AbstractTwo simple microbiological tests for quantitating salivary Streptococcus mutants levels were compared with each other and evaluated for suitability for mass screenings and private practice. Both tests use mitis salivarius medium with bacitracin (MSB) and are selective for S. mutans. One test estimates colories grown on agar (MSBA) and the other estimates colonies grown in broth that adhere to glass (MSBB). Both are scored from 1 (lowest) to 5 (highest). Children (293) in grades 5 and 6 in Tallahassee, Florida (nonfluoridated) were tested for S. mutants levels. Scores of children by both tests were significantly similar (χ2, regression and Kappa statistical analysis). Correlation coefficients (regression) between scores and DMFS increments of the previous 4 yr were 0.35 (MSBA) and 0.26 (MSBB). Both tests were very good in identifying children with low caries increments, but positive scores did not correlate well with high caries increments. Sampling and interpreting took 2 min (MSBA) and 1 min (MSBB). Cost of MSBA/child was $1.15, and MSBB/child $1.25. These tests are economical and suitable for mass screenings to identify low risk populations who do not require preventive treatment

AbstractA population of 163 10th grade, 15‐yr‐old students of a Jerusalem high school were surveyed for caries prevalence, cariogenic dietary habits, dental knowledge and socioeconomic variables. A mean level of 7.75 DMFT was found, confirming results of previous studies which indicate a general increase of caries prevalence in Israel over the past two decades. None of the independent variables measured constituted significant predictors of caries prevalence.
