Among the many proposals currently being advanced to cope with the problem of rising hospital costs, two are clearly unacceptable: government takeover and transformation of the voluntary hospital into a profit–making, compctitivcly–priced, enterprise. Rather than any such broad hut impractical panaceas, the author suggests study and action in ten specific problem areas.

The New York City Medicaid Pharmacy program is the largest of its kind in the country. In 1968, 17.9 million dollars were paid to private licensed pharmacies, participating in Medicaid, for the delivery of pharmacy services to Medicaid enrollees. This sum represented 12 per cent of all funds paid for personal health care services provided by private health care practitioners in 1968. From the inception of Medicaid in May 1966, the pharmacy program has grown to include 2,330 private pharmacies, or 71 per cent of pharmacies available in New York City. An analysis made of a one–day sample of 22,000 prescriptions received for payment January 5, 1968, disclosed that the average Medicaid prescription cost was $3.59. The New York City Medicaid Pharmacy Watchdog System to monitor potential and alleged abuses is described. Professional pharmacy issues, such as generic drugs, formulary, and professional fees are discussed.

A survey of persons in treatment for psychiatric disorders with the general practitioners of an island showed a one–year prevalence rate of 51.5 per thousand population. This represents more than twice the rate of those who consult a psychiatrist in the same community during a year, even though psychiatric service is readily available, geographically accessible and of low cost. Thus, even under these conditions, the larger burden of treating psychiatric patients was being borne by the general practitioner. In nine of every ten patients, the problem was a psychoneurotic, psychophysiologic or personality disorder. Alcoholism was present in 28 per cent of all males considered to have psychiatric conditions. The psychiatric disorder rates were 50 per cent higher in the never–married and separated–divorced–widowed groups than in married people. Data on prevalence in relation to social class are provided also.

A method for estimating the effectiveness of systems designed to monitor the occurrence of events within a population is described. Specific application of this model to the analysis of a control program for the surveillance of infectious disease in a university hospital is presented. A reporting system with three sources—physicians, nurses, and bacteriology laboratory—was instituted. The effectiveness of the system ranged from 61 to S3 per cent over a period of 12 months. There was considerable variation among the three sources in terms of their relative effectiveness in identification of patients with infections. Under the circumstances described, the most effective two–source reporting system would have been the physicians' discharge reports and bacteriology laboratory records.

Successful home care of chronically ill and disabled people often depends upon that family member who bears primary responsibility for helping the patient and cooperating with the home–care team. This family member plays a role which we entitle “responsor.” We describe a population of responsors as observed by a home“care team during their clinical activities. Based on this, we offer a general analysis of the structure and function of the responsor role, and its complex relationships to patient and home“care team. The role seems worthy of further study, in the interests of the courageous people who fill it and who must he reckoned an important resource in the problem of medical manpower. The activity “home care” is best understood through a form of systems analysis of the relationships between patient, responsor and team, within a frame of reference provided by the illness and its demands for care, and also by the other purposes of the household.
