Both institutional and ambulatory techniques have been utilized in the detoxification of addicts from narcotics. This study was designed to provide empirical data on attrition from one outpatient program for hard-core heroin addicts. The background and treatment characteristics of 86 addict-patients were analyzed. Education, marital status, and the abuse of drugs concurrently with heroin were the only variables which were found to be associated with whether an addict remains under treatment until the detoxification process is completed. A multivariant analysis isolated one variable, the concurrent abuse of other drugs with heroin, as the most potent predictor of remaining in treatment.

The New York City Podiatry program is discussed, with a brief review of the podiatry program of the New York City Department of Welfare prior to the inception of Medicaid. Emphasis is given to the functions of the New York City Health Department in setting standards of care, monitoring, and enforcement of high quality podiatric care. Notwithstanding the complexities and difficulties built into the Medicaid programs through legislation for efficient and effective administration, podiatry through the Medicaid program has made a significant contribution to the well-being of thousands of persons for whom podiatric care previously was inaccessible. Furthermore, the standard setting and audit responsibilities and functions of the Health Department Medicaid administration portends a relevant impact on improving podiatric care services in New York City for all patients.

In 1968, the Federal government passed an amendment to the Social Security Act requiring licensing by the states of nursing home administrators. An Advisory Council was established to study the problem and establish guidelines for state legislation. As of January 1, 1970, 25 states had passed licensure laws, and an additional 20 had bills before their legislatures. In this report, the state laws are examined to determine the variations as between states, the extent to which the laws deviate from the Model Law and expressed intent of the Congress, and the problems involved in licensure. Although this specific legislation deals only with the nursing home administrators, it raises issues regarding licensing in the medical and health care field, from the professional specialist to the newly developing occupations in the paramedical areas.

A.M.A. data, from 1968, are examined to determine the impact of foreign medical graduates (FMGs) on the geographic distribution of physicians in the United States. Doctor/ population ratio data indicate that FMGs have increased the disparity in the availability of physicians' services among states and between urban and rural areas. Thus it is shown that an increase in the number of physicians does not necessarily help to relieve imbalances in the geographic distribution of physicians.

Persons making decisions about health care personnel are confronted with an array of measurements that allegedly determine "need." This paper examines selected categories of indicators; one category' analyzes those which compare numbers of health personnel to number of persons; a second category analyzes health personnel need as indicated by economic factors; a third category examines personnel need as indicated by factors of geographic distance. There are both advantages and disadvantages to using any one indicator, and there is greater reliability in using more than one indicator. Most indicators used can be derived from available information. The indicator utilized should fit the health need. A plea is made for caution to be employed in declaring need for health manpower.

This is a study of the role and functional relationships of the public health nurse (PHN) as perceived bv other health care workers. Two hundred and twenty-seven subjects, comprising subgroups of medical students, nursing students, PT students, public health nurses, nursing faculty, academic physicians, and preventive medical staff, used a specially constructed 64-item rating instrument to describe their perceptions of the PHN. Factor analysis of these data yielded five interpretable factors, which were subsequently termed, 1. Denial of Incompetence, 2. Interpersonal Skills, 3. Physician Dominance, 4. Induced Role Ambiguity, and 5. Effectiveness. The mean factor scores on these factors for the seven subject groups evidenced two types of profiles, the first PHN convergent (signed +, +, −, −, + , on the five factors respectively) and the second physician convergent (signed −, −, +, +, −, on the five factors). In general terms, the PHN convergent profile, which was common to the student nurse, PHN, and preventive medicine staff groups, represented a positive view of the PHN and the physician convergent profile, which characterized the medical student and academic physician groups, was negative. The nursing faculty group profile was physician convergent except for its disavowal of Physician Dominance. These discrepancies in describing the PHN arc discussed in terms of their implications for interorganizational and interprofessional conflict, and their relationship to the health care education system are considered.

A group of family nurse practitioners in rural Appalachia was observed, and their patient contacts were analyzed. It is concluded that nurses can assume the role of providing some primary medical care in a rural area, that they can care for a large proportion of patient complaints without consulting a physician, and that increased training and experience enable nurses to care for a larger percentage of patients without a physician's direct aid. There are indications that at least a minimum of formal instruction in some of the arts of the physician would enable these nurses to perform their duties in an optimal manner. There are clear implications that formally trained nurse practitioners working as "physician associates" could increase the effectiveness of oui health care systems.

This is an examination of the complex interrelationship of public fiscal policy with service delivery patterns, manpower utilization, and allied health professions education, with particular focus on the problems of transition from primarily custodial services to community-based systems of care. Excessive long-term costs and clinical ineffectiveness of custodial care are discussed as these negatively affect early implementation of preventive services, comprehensive care, and collegiate curricula for non-traditional, middle level manpower. A plea is made for public sector cost-accounting procedures based on longitudinal assays of clinical results. Thus the lack of viable community alternatives to hospitalization increases both morbidity and the size of custodial requirements, preempting the very monies necessary to inaugurate more efficient indigenous programs. Resolution to the dilemma is postulated through: 1. The deployment of interdisciplinary care teams; 2. Development of undergraduate trained associate professionals; and 3. Reversing our traditional style of affording chronic care by returning to the family (and, indirectly, to the community) its historical primacy in the basic nurturing of chronic patients, further supported by accessible neighborhood care centers.
